[The "Nephrology outpatient Triage": an organizational model for the ambulatory care of patients with advanced renal disease].

The clinical course of outpatients with advanced chronic kidney disease requires a close monitoring by the nephrology team, in order to identify emerging clinical problems promptly and prevent subsequent complications. With the aim of improving the outpatient management in our clinic dedicated to advanced renal failure, we implemented the "Nephrology Clinic Triage" (NCT). This organizational model is coordinated by the nephrologist and supported by nurses. In case the outpatients, or their caregivers, have clinical problems or need advice, they can easily get in touch with a nephrology nurse b

From Patient Voice to Organizational Adaptability: A Critical Narrative Review and Conceptual Model for Personalized Care.

Hospital routines can elicit patients' views without providing a reliable route for acting on them. The aim of this article is to develop a positive organizational model in which patient voice provides the basis for tailoring care, while professional responsibility remains with the clinical team and healthcare organization. We conducted a critical narrative review with conceptual synthesis, drawing on a previously assembled corpus of 105 full-text publications and a targeted update completed on 10 August 2026. The analysis compared patient input, organizational responses and limits of transfer

Meta-analytic structural equation modeling for exploring workplace friendship, well-being, and organizational commitment.

BACKGROUND: Workplace friendships, which are informal relationships in the workplace, foster mutual trust, assistance, and emotional support and create a sense of belonging among coworkers. Employees with positive perceptions of their job responsibilities, psychological needs, and work environment experience happiness, satisfaction, and an overall sense of well-being. A heightened sense of well-being in the workplace positively affects employee satisfaction with job tasks and the work environment, reinforcing employees' sense of belonging and organizational commitment. OBJECTIVE: The study pr

Organizational cynicism and job burnout: a moderated mediation model of work alienation and leader-member exchange.

BACKGROUND: In organizational management practice, understanding the intrinsic link between employees' negative perceptions of the organization and their state of emotional exhaustion, as well as identifying the transmission mechanisms and boundary conditions, has become a key issue in mitigating the risk of job burnout. However, the mechanisms through which organizational cynicism relates to work alienation and job burnout, in addition to the moderating role played by the leader-member exchange in this relationship, still lacks clear theoretical explanations and empirical tests. METHOD: Base

Organizational determinants of burnout among hospital-based nurse practitioners: A structural equation modeling analysis.

BACKGROUND: The nurse practitioner (NP) workforce is rapidly expanding in hospital settings, yet limited evidence exists on how organizational practice environments influence NP burnout. Identifying modifiable workplace factors is critical to improving NP well-being and retention. OBJECTIVE: To examine the associations between dimensions of the NP practice environment and burnout among hospital-based NPs. METHODS: This cross-sectional observational study used data from the 2024 Penn Nurses4All Study. The analytic sample included 1405 NPs working in hospital settings. The independent variable

Sepsis Team Organizational Model to Decrease Mortality for Intra-Abdominal Infections: Is Antibiotic Stewardship Enough?

Introduction. Sepsis is an overwhelming reaction to infection with significant morbidity, requiring urgent interventions in order to improve outcomes. The 2016 Sepsis-3 guidelines modified the previous definitions of sepsis and septic shock, and proposed some specific diagnostic and therapeutic measures to define the use of fluid resuscitation and antibiotics. However, some open issues still exist. Methods. A literature research was performed on PubMed and Cochrane using the terms "sepsis" AND "intra-abdominal infections" AND ("antibiotic therapy" OR "antibiotic treatment"). The inclusion crit

Integrated Hospital-Territory Organizational Models and the Role of Family and Community Nurses in the Management of Chronic Conditions: A Scoping Review.

Background and Objectives: One of the challenges of modern healthcare systems, in terms of economic and organizational sustainability and the impact on patients' quality of life, is the progressive increase in chronicity and care complexity. In this scenario, hospital-community integration models represent possible strategies to ensure the continuity of care, reduce readmission rates, and improve clinical outcomes. This study aims to map integrated care models for patients with chronic diseases, with active involvement of the family and community nurse, describing their functions and associate

Organizational dehumanization and gig workers' outcomes: a moderated mediation model of psychological distress and family recognition of work.

INTRODUCTION: Platform work is increasingly organized through algorithmic rules and standardized performance systems that may leave workers feeling treated as replaceable instruments rather than as full persons. Drawing on the Job Demands-Resources (JD-R) model, this study examined whether psychological distress mediates the associations between organizational dehumanization and gig workers' flourishing and commitment to service quality, and whether family recognition of work moderates this process. METHODS: Data were collected from married food-delivery and ride-hailing platform workers in C

Personal, biomechanical, psychosocial, and organizational risk factors for carpal tunnel syndrome: a structural equation modeling approach.

This longitudinal study aimed at exploring the direct and indirect relationships between organizational, psychosocial, biomechanical, and personal factors and carpal tunnel syndrome (CTS) in French workers. Between 2002 and 2005, 3710 workers were included in the Cosali cohort. Between 2007 and 2010, 1611 workers were re-examined using the same standardized clinical protocol. Subjects free from CTS at baseline were studied (804 men and 563 women). We used structural equation models to examine the relationships between incident CTS cases and organizational (machine-paced work or work pace depen

[A multi-disciplinary approach to diabetic foot patients--an organizational model for the treatment of leg complications in diabetic patients].

Diabetes mellitus is the major cause of non-traumatic limb amputations in the Western world. In the diabetic foot patient, 85% have developed a leg ulcer prior to limb amputation. Due to the complicated nature of therapy in such patients, a multi-disciplinary approach is warranted. In this review, we describe an organizational model which provides an immediate solution to the factors involved in the evolution of a diabetic foot. At the end of 2002, The Wolfson Medical Center administration decided to centralize the diabetic foot patients into one unit situated in the Department of Medicine C.

How does servant leadership affect unethical pro-organizational behaviors: examining a moderated mediation model.

BACKGROUND: Servant leadership in the healthcare sector can lead to negative outcomes in certain contexts, such as encouraging unethical pro-organizational behavior (UPB) for the benefit of the organization. The purpose of this study is to examine the mediating role of identification with leader and the moderating role of positive reciprocity belief in the relationship between servant leadership and UPB, drawing on social exchange and social identity theory. In this context, the aim is to highlight not only the generally accepted positive outcomes of servant leadership in literature but also i

Access to pulmonary rehabilitation in real-world territorial care: the APTA friendly network as a structural and organizational model.

BACKGROUND: Pulmonary rehabilitation represents a cornerstone of chronic respiratory disease management, supported by robust evidence demonstrating improvements in functional capacity, symptoms and health-related quality of life. Nevertheless, its implementation in real-world territorial respiratory care remains limited and highly heterogeneous, suggesting the presence of systemic and organizational barriers beyond clinical efficacy. METHODS: This mixed-methods, expert-based study combined structured expert discussion and targeted survey data collection among pulmonologists actively involved

The Fast-Track Program: A new Organizational Model to Enhance Living Donor and Pre-emptive Kidney Transplantation.

Living donor kidney transplantation (LD KTx), especially when performed pre-emptively, remains the gold standard for treating end-stage renal disease (ESRD). In Italy, however, this resource remains underutilized due to significant organizational hurdles and protracted evaluation timelines. To overcome this limitation, a Fast-track program (FTp) was developed in our Center, aiming to perform the clinical work-up of donor and recipient candidates pairs (DR) in five consecutive days with dedicated schedules on outpatient basis. The results so far obtained in twenty DR evaluated with the FTp indi

A hospital-based HTA model to assess the economic and organizational impact of intravenous-to-subcutaneous monoclonal antibody transitions in oncology.

OBJECTIVES: Subcutaneous (SC) formulations of monoclonal antibodies (mAbs) allow faster and less invasive administration than intravenous (IV) routes, with comparable efficacy and safety. Beyond clinical equivalence, SC administration may reduce hospital resource use, improve patient experience, and increase treatment capacity, contributing to the sustainability of oncology care. This study aimed to develop and apply a hospital-based, multidisciplinary model to assess the economic and organizational impact of IV-to-SC transitions in oncology.
