Tissue response to aqueous drainage in a functioning Molteno implant.

The tissue reaction to a functioning Molteno implant has been studied by light microscopy and by scanning and transmission electron microscopy. The material was obtained from an aphakic 83-year-old human eye which required enucleation because of intractable bullous keratopathy, despite well controlled intraocular pressure. The tissue response around the silicone rubber tube was that of simple fibroblastic activity. Around the implant, however, there was fibrous tissue in which necrotic cell debris was prominent. No significant inflammation was observed in this tissue, but the choroid beneath t

Subconjunctival silicone oil drainage through the Molteno implant.

To describe silicone oil drainage from the vitreous cavity to the subconjunctival space through a Molteno implant. A 52-year-old aphakic man with a Molteno implant inserted after lensectomy, vitrectomy and intravitreal silicone oil injection visited our hospital. The Molteno tube was located in the anterior chamber with two silicone oil drops on it. Corneal dellen, induced by marked bleb-like elevation of the conjunctiva was noted during follow-up and excision biopsy was attempted. The conjunctival elevation consisted of innumerable microdrops of silicone oil enclosed by inflammatory subconjun

The effect of adjunctive mitomycin C in Molteno implant surgery.

PURPOSE: The purpose of the study is to assess the effect of adjunctive intraoperative mitomycin C (MMC) in Molteno drainage device implantation for patients with recalcitrant glaucomas. METHOD: Forty-nine eyes of 49 patients who underwent one-stage, single-plate Molteno device implantation with adjunctive intraoperative MMC (0.5 mg/ml) for 3 to 5 minutes (MMC group) were compared to a historic control group of 51 eyes of 51 patients (control group) who received one-stage, single-plate Molteno device implantation without MMC. Success (survival) was defined as an intraocular pressure (IOP) bet

Clinical experience with the Molteno dual-chamber single-plate implant.

A Molteno dual-chamber single-plate implant was placed in 40 consecutive patients, who were followed for a minimum of 6 months. This implant is identical to the single-plate Molteno implant, except for the addition of a pressure ridge to the upper surface of the episcleral plate, which allows the episcleral tissues to function as a temporary, pressure-sensitive valve, limiting the escape of aqueous in the early postoperative period. Confirming earlier studies, we found that use of the modified implant reduced immediate postoperative hypotony, eliminated choroidal hemorrhage, and, by obviating

Long-term results of primary trabeculectomies and Molteno implants for primary open-angle glaucoma.

OBJECTIVE: To describe the long-term outcomes of primary trabeculectomies and primary Molteno implants performed in cases of primary open-angle glaucoma at Dunedin Hospital. METHODS: Prospective comparative case series of 718 eyes (500 patients) and 260 eyes (195 patients) that had trabeculectomy and Molteno implants, respectively, at Dunedin Hospital as the first drainage operation for primary open-angle glaucoma between 1976 and 2007, and followed up for a mean of 7.7 (range, 0.0-28.0) and 5.0 (range, 0.0-27.4) years, respectively. RESULTS: The probability of intraocular pressure (IOP) con

Prevention of early hypotony associated with Molteno implants by a new occluding stent technique.

BACKGROUND: A major complication of unvalved drainage tube implants is hypotony. Occluding stents to internally block the tube, but which lie exposed in the inferior fornix, have been described. The authors report modifications of this technique. METHODS: A nonexposed 3-0 Supramid suture is used to internally block the tube, with the end placed subconjunctivally. Small venting slits are made in the extrascleral portion of the tube to provide early control of intraocular pressure. Molteno implants were placed in 32 eyes of 29 patients using these modifications. RESULTS: Mean preoperative intr

Comparison of the double-plate Molteno drainage implant with the Schocket procedure.

A randomized, prospective study was undertaken to compare the results of the double-plate Molteno implant with those of the anterior chamber tube shunt to encircling band (ACTSEB) procedure, described by Schocket, for eyes with complicated glaucoma. One surgeon performed all the procedures. For statistical analysis there were 21 eyes in the ACTSEB group and 19 eyes in the Molteno group, with at least 6 months of follow-up (mean follow-up, 26 months; range, 6 to 49 months). Eyes with aphakia/pseudophakia, uveitis, previous penetrating keratoplasty, previous unsuccessful trabeculectomy, and neov

Clinical experience with the single-plate Molteno implant in complicated glaucomas.

Ninety patients with medically uncontrollable glaucomas and poor surgical prognoses received single-plate Molteno implants (Optomat Supplies, Dunedin, New Zealand) without postoperative adjunctive systemic antifibrosis therapy. Eleven patients had insufficient follow-up for assessment of intraocular pressure (IOP) or visual acuity outcome. The initial Molteno implant procedures in the remaining 79 patients were successful (IOP less than or equal to 21 mmHg with at least 6 months' follow-up) in 26 (63%) of the 41 patients older than 12 years of age with non-neovascular glaucomas in aphakia/pseu

Molteno implant for control of glaucoma in eyes after penetrating keratoplasty.

Seventeen patients (17 eyes) underwent implantation of a single plate Molteno implant for medically uncontrollable intraocular pressures after penetrating keratoplasty. Most of the eyes had extensive peripheral anterior synechiae, and 16 of 17 (94%) were pseudophakic or aphakic following keratoplasty. Other glaucoma procedures had been performed previously on 13 eyes: argon laser trabeculoplasty (one eye), trabeculectomy (seven eyes), transpupillary argon laser cyclophotocoagulation (three eyes), and cyclocryotherapy (three eyes). Follow-up ranged from 5 to 28 months (mean, 13 months). Three e

Single stage Molteno implant with combination internal occlusion and external ligature.

A variety of techniques have been devised to reduce postoperative hypotony following a single-stage Molteno implant procedure. Although they effectively reduce hypotony, these techniques may result in a prolonged increase of intraocular pressure (IOP). I present a technique, consisting of placement of a 5-0 nylon internal occlusion suture combined with an external polyglactin (Vicryl) ligature around the Molteno tube, to prevent both hypotony and elevated IOP after implantation.

The long-tube single plate Molteno implant for the treatment of recalcitrant glaucoma.

One hundred cases of recalcitrant glaucoma were operated with a long tube single-plate Molteno implant (LSMI). At the median follow-up of 15 months an intraocular pressure of less than or equal to 19 mm Hg was obtained in 64 of the 87 eyes (73%) which have a minimum follow-up of 6 months (secondary non-neovascular glaucomas = 19; congenital glaucomas = 26; secondary neovascular glaucomas = 12). The interval probability (percentage +/- standard error) of obtaining a successful result (IOP less than or equal to 19 mm Hg) was 79% (+/- 13) at twelve months from surgery, and 53% (+/- 24) at the eig

Retinal detachment adherent to posterior chamber IOL after Molteno implant surgery.

A patient with refractory glaucoma 1 year after cataract extraction and trabeculectomy had Molteno implant surgery. Three days after surgery a kissing choroidal effusion and retinal detachment adherent to the posterior chamber IOL were detected. Repeated choroidal taps were unsuccessful. Removal of the Molteno implant, vitrectomy, and silicone oil injection were required to reattach the retina.

The use of releasable sutures in Molteno glaucoma implant procedures to reduce postoperative hypotony.

We used releasable sutures to minimize immediate postoperative hypotony and flat anterior chamber in 19 cases of refractory glaucoma requiring insertion of a single-plate Molteno implant in a one-stage procedure. A slip knot using 7-0 nylon suture was fashioned around the Molteno tube under a lamellar scleral flap. In 18 of the 19 eyes, anterior chamber depth was normal immediately after surgery.

[Surgical treatment of chronic aphakic glaucoma with the Molteno plastic implant (author's transl)].

Chronic aphakic glaucoma can be surgically corrected with the Molteno plastic implant, which consists of a silicone tube attached to a plastic base. The technique described here avoids complications such as hypotony, blocking of the filtering mechanism and fibrosis of the filtration bleb, which can arise in conventional glaucoma operations. Out of 31 eyes with aphakic glaucoma 29 had normal intraocular pressure within six to 72 months without any medication whatsoever.

Molteno implant versus trabeculectomy with adjunctive intreoperative mitomycin-C in high-risk glaucoma patients.

PURPOSE: The Molteno implant is one of the most widely used drainage devices for the management of complicated glaucoma. Antiproliferative agents such as mitomycin-C (MMC) have markedly improved the outcome of glaucoma filtering surgery. This case control study compares the results of these two techniques in the management of high-risk glaucoma patients. METHODS: Forty-three consecutive patients with complicated glaucoma who underwent glaucoma filtration surgery with intraoperative application of 0.3 mg/ml of MMC were matched with a control group of 43 glaucoma patients who previously had tub
