Posterior interosseous nerve palsy can occur following a Monteggia fracture by a variety of mechanisms. In this case report of a 35-year-old man the nerve became wrapped around an anterolaterally dislocated radial head in the course of an unsuccessful attempt as closed reduction. Cadaveric dissections demonstrated how reduction of the radial head can be blocked by an enwrapped nerve. Obviously open reduction of the fracture with exploration of the nerve is the treatment of choice.

PURPOSE: Monteggia-variant fractures involve a fracture of the proximal ulna combined with a radiocapitellar dislocation and/or additional injuries to the radial head. These injuries are associated with severe outcomes, high complication rates, and frequent need for revision surgery. This study aimed to investigate strategies for early revision following failed operative treatment of these injuries.
METHODS: We retrospectively included all patients over 18 years of age who underwent revision surgery within the first 3 months after failed initial treatment of a Monteggia-variant fracture due t

A one-year-old, intact female Sphinx cat was presented with severe lameness in the right front limb following a fall. Orthopedic examination revealed grade four lameness with hematoma and severe pain around the elbow. Radiologic examination revealed a type I Monteggia fracture. The ulnar fracture was treated with plate osteosynthesis, while the radius head dislocation was fixed with a positional screw inserted between the ulna and radius. The cat was kept on cage rest for eight weeks. Recheck radiographs were taken four and eight weeks after the operation. At the time of the second follow-up e

The eponym Monteggia fracture dislocation originally referred to a fracture of the shaft of the ulna accompanied by anterior dislocation of the radial head that was described by Giovanni Battista Monteggia of Italy in 1814. Subsequently, a further classification system based on the direction of the radial head dislocation and associated fractures of the radius and ulna was proposed by Jose Luis Bado of Uruguay in 1958. This article investigates the evolution of treatment, classification, and outcomes of the Monteggia injury and sheds light on the lives and contributions of Monteggia and Bado.

The Monteggia fracture is one of the pitfalls of conventional diagnosis of upper limb trauma. Through a retrospective study of 20 cases diagnosed at the Department of Orthopaedic Surgery and Traumatology, University Hospital Center, Ibn Sina, Mohamed V University, Rabat, Morocco, between 2010 and 2014, we have tried to do an update on the management of Monteggia fractures either at of paraclinical exams or the surgical management. We support the idea that the dislocation of the radial head should be sought systematically to any isolated fracture of the ulna, for not to miss fracture Monteggia

BACKGROUND: High-energy trauma may result in uncommon open injuries around the elbow joint. The management of these injuries can be difficult.
CASE DESCRIPTION: Fourteen patients were treated between 1999 and 2003 and their injuries consisted of Monteggia fracture-dislocations combined with segmental fractures of the ulna or fractures of the forearm bones and/or various more complex trauma such as neural injuries, bone comminution and severe soft tissue injuries around the elbow. Eight of them (57%) were multiply injured with severe additional injuries. All patients underwent surgery within f

Open reduction of the radial head and reconstruction of the annular ligament has been advocated for the Monteggia fracture dislocation in children who present more than a month after injury. Three patients with an anterior Monteggia lesion were treated by open reduction of the radial head which was held in place by a Kirschner wire passed from the humerus to the radius. No attempt was made either to repair or reconstruct the annular ligament. The patients were aged between 2 and 6 years, the delay between injury and reduction was between 6 and 8 weeks, and the length of follow up was 5 years f

We reviewed 36 consecutive patients with Monteggia fracture-dislocations of the forearm; 28 had been treated within 24 hours and 8 had been referred a week or more after the initial injury with persisting or recurrent dislocation of the proximal radio-ulnar joint after treatment elsewhere. We treated 15 of the 16 complete fractures and 3 of the 11 incomplete fractures of the ulna by operative fixation. All the early fractures and six of the eight late referrals had good or excellent results. The two poor results were in patients with malalignment and dislocation of the radial head persisting f

In an analysis of 29 cases of three different Monteggia-type fractures in children, the mechanism of injury was a hyperpronation force on the extended elbow. The follow-up period ranged from one to three years with a mean of 18 months. Closed reduction of these fractures was always difficult to maintain because the lesions are unstable. Closed reduction was successful in 25 of the 29 cases and open reduction was required in four. A slight residual dorsal angulation remained in 18 cases with fracture of both bones of the forearm, but healing in this position was acceptable.

INTRODUCTION: Neglected Monteggia fracture-dislocations in children remain challenging due to adaptive deformities of the ulna, chronic radial head dislocation, and soft tissue contractures. While early recognition ensures excellent outcomes, delayed cases often require complex reconstructive surgery. This study presents the clinical and radiological outcomes of a case series involving corrective oblique ulnar osteotomy in neglected Monteggia fractures (NMF), open reduction of the radial head, temporary transcapitellar fixation, and annular ligament reconstruction (ALR).

BACKGROUND: Although many pediatric Monteggia fractures can be treated nonoperatively, the presence of any residual radiocapitellar subluxation following ulnar reduction mandates a more aggressive approach to restore and maintain ulnar length. In younger children, restoration and maintenance of ulna length may be achieved through intramedullary fixation of the ulnar shaft.
DESCRIPTION: A Steinmann pin or flexible intramedullary nail is introduced percutaneously through the olecranon apophysis and advanced within the medullary canal to the ulnar fracture site. If necessary, the ulnar length an

Monteggia fractures and olecranon fracture dislocations represent complex injuries with distinct patterns of bony and soft tissue involvement. Fractures of the proximal ulna and olecranon process may lead to disruption of the proximal radioulnar joint and/or ulnohumeral joint. The keys to treatment are recognition of the pattern of injury and formation of an algorithmic surgical plan to address all components of the injury process. Complications are common and may be related to the injury spectrum itself and/or inadequate fracture alignment or fixation.

OBJECTIVE: To investigate the classification and treatment of Monteggia equivalent fractures in children.
METHODS: A retrospective analysis was made on the clinical data of 35 cases of Monteggia equivalent fractures between January 2008 and January 2012. There were 17 boys and 18 girls with an average age of 7 years and 5 months (range, 1 year and 2 months to 14 years and 11 months). The causes of injury were tumbling injury in 25 cases, falling injury in 3 cases, and sport injury in 7 cases. The disease duration from injuries to admission ranged from 1 hour to 16 days (median, 28 hours). Acc

A rare combination of an ulnar fracture with posterior dislocation of the head of the radius and intraarticular fracture of the distal radius concomitant with an ipsilateral navicular fracture is presented. To the best of our knowledge, no such case has been reported. The mechanism of this fracture combination is discussed with respect to its role in producing the Monteggia type II fracture. Treatment consisted of a combination of closed and open reduction with external fixation.

A lateral Monteggia fracture in a child is reported. Between 1941 and 1977, only 16 such cases were reported in the literature. Of these, 2 cases needed open reduction of the fractured ulna. The present case needed osteotomy of a nearly healed fracture of the ulna and reconstruction of the orbicular ligament to stabilise the reduction of the radial head. No similar case has previously been described in a child. Prompt diagnosis and early reduction should yield good results by closed methods of treatment, but open reduction is necessary if the diagnosis is missed and treatment is delayed. Even

INTRODUCTION: Infected non-union of long bones represents one of the most challenging problems in orthopedic surgery. Conventional two-stage approaches involving debridement, antibiotic spacers, and subsequent fixation are associated with high morbidity and prolonged treatment duration.
MATERIALS AND METHODS: We present a prospective case series of 10 patients with infected non-union of various long bones (tibia, femur, humerus) treated with antibiotic cement-coated intramedullary nails (AC-CIMN). The nail was coated with polymethylmethacrylate mixed with culture-sensitive antibiotics in a si

INTRODUCTION: Neglected Monteggia Fractures (NMF) in children typically result from missed diagnoses at initial assessment, and may lead to pain, instability, and functional impairment. The optimal treatment strategy remains controversial. This study evaluates surgical outcomes and prognostic factors in pediatric NMF.
METHOD: A retrospective review was conducted of children under 14 years treated for NMF (>4 weeks postinjury) from 2010 to 2024 at a tertiary pediatric referral center, with a minimum follow-up of 12 months. All patients underwent open reduction of the radial head, with or witho
