The neuropsychology of mood disorders.

Cognitive dysfunction is central to our understanding of mood disorders in terms of patient experiences, Diagnostic and Statistical Manual of Mental Disorders criteria, and psychological models. In this article, we highlight key findings from studies that have used neuropsychological tests and functional neuroimaging techniques to explore cognitive dysfunction in patients with depression and mania. In particular, we focus on affective processing bias, abnormal response to negative feedback, and decision making. Results are discussed in the context of current conceptualizations of dysfunctional

The epidemiology of mood disorders.

This review provides an overview of the epidemiology, risk factors, and genetic epidemiology of mood disorders in adults and children. The magnitude and impact of mood disorders in the community outweighs that of most other chronic diseases. Although there is substantial knowledge regarding the sociodemographic risk factors for mood disorders, our understanding of the pathogenesis and classification still is evolving. Comorbidity of mood disorders with anxiety disorders and substance abuse has been documented widely. Whereas substance abuse and mood disorders seem to be independent etiological

Atypical antipsychotics in mood disorders.

Bipolar disorder is ranked as the sixth most important worldwide cause of disability. Current treatment is based chiefly on lithium and/or anticonvulsants, of which sodium valproate is the most widely used. A significant minority of patients fail to respond fully to current treatments, particularly those with mixed mania and/or rapid cycling. Many patients are unable to tolerate the side-effects of current therapy in the long term, and adverse effects may contribute to the high rate of noncompliance observed in bipolar disorder. The shortcomings of current treatments are reflected in poor outc

Women's issues in mood disorders.

Since the introduction of antidepressants in the 1950s, it was assumed for the next several decades that there were no special reasons to look at the application of these medications to women. In the past half-century, particularly in the past decade, since the advent of the selective serotonin re-uptake inhibitors (SSRI), a series of specific foci have developed. Firstly, there appear to be differences in the degree of response to particular antidepressants between the genders. Secondly, there is data concerning hormonal effects of particular relevance to women, i.e. prolactin, which separate

Psychobiology of postpartum mood disorders.

Postpartum mood disorders are common. The clustering of mood-disorder episodes after birth compels a search for factors particularly potent during childbearing. In this article, the complex relationships between the dynamic postbirth physiological environment and mood disorder are discussed. Available studies show a lack of evidence that serum levels of gonadal hormones account for mood disturbance in women. However, substantial amounts of data demonstrate their ability to modulate other neuroendocrine systems. Alterations in hypothalamic-pituitary-adrenal (HPA) axis function attributable to c

Clozapine in severe mood disorders.

BACKGROUND: A growing literature suggests that the atypical antipsychotic agent clozapine may be effective in schizoaffective and psychotic mood disorders. METHOD: To evaluate the efficacy and tolerability of clozapine in severe mood disorders, we reviewed published studies on clozapine in schizophrenia, bipolar disorder, schizoaffective disorder, major depression, and organic disorders with psychotic or major affective syndromes, identified through the MEDLINE data base. RESULTS: Patients in manic or psychotic phases of schizoaffective or bipolar disorder were significantly more likely to r

[Modern aspects of mood disorders].

Mood disorders, particularly depression, now represent a major world health problem which will most likely continue increasing. From a clinical standpoint, the synthesis of new antidepressant agents of various chemical structures and the advent of the thymoregulators (lithium salts) has allowed a more flexible, better individualized, therapeutic approach to the affective disorders. However, the widespread use by general practitioners of the antidepressants, alone or in combination with other psychotropic drugs, often with mitigated success, brings to psychiatric consultation patients whose sym

Mood reactivity and affective dynamics in mood and anxiety disorders.

The application of ecological momentary assessment (EMA) in community settings provides a powerful opportunity to obtain measures of emotional reactivity to daily life events, as well as emotional dynamics in real time. This investigation examines the association between emotional reactivity to daily events and emotional experience in mood and anxiety disorders in a large community-based sample. Two-hundred and 87 participants with a lifetime history of bipolar I disorder (BPI; n = 33), bipolar II disorder (BPII; n = 37), major depression (MDD; n = 116), anxiety disorders without a mood disord

Sleep Disturbances in Mood Disorders.

The article provides an overview of common and differentiating self-reported and objective sleep disturbances seen in mood-disordered populations. The importance of considering sleep disturbances in the context of mood disorders is emphasized, because a large body of evidence supports the notion that sleep disturbances are a risk factor for onset, exacerbation, and relapse of mood disorders. In addition, potential mechanisms for sleep disturbance in depression, other primary sleep disorders that often occur with mood disorders, effects of antidepressant and mood-stabilizing drugs on sleep, and

Sleep deprivation in mood disorders.

Growing clinical evidence in support of the efficacy and safety of sleep deprivation (SD), and its biological mechanisms of action suggest that this technique can now be included among the first-line antidepressant treatment strategies for mood disorders. SD targets the broadly defined depressive syndrome, and can be administered according to several different treatment schedules: total versus partial, single versus repeated, alone or combined with antidepressant drugs, mood stabilizers, or other chronotherapeutic techniques, such as light therapy and sleep phase advance. Normal N400 in mood disorders.

Individuals diagnosed with major depression have been characterized as having a variety of cognitive problems based on a number of behavioral and psychophysiological measures, but it is not clear whether there is a consistent language processing abnormality in depression. Three studies sought to determine whether diverse mood disordered samples show abnormal semantic processing, as indexed by a failure to show increased N400 event-related brain potential amplitudes to passively viewed incongruent, relative to congruent sentence endings. Individuals with major depression (N = 50) or dysthymia (

Mood variability in anxiety disorders.

BACKGROUND: To investigate whether patients with anxiety disorders have more variable mood than control subjects. METHODS: Twenty-eight patients with anxiety disorders and 28 controls were assessed with the State-Trait Anxiety Inventory-Trait form (STAIT), Beck Depression Inventory (BDI), Mood Disorder Questionnaire (MDQ), and TEMPS-A questionnaire for temperament. Participants used Visual Analogue Scales (VAS) to rate low, high and anxious moods, morning and evening, for 7 consecutive days. Mood Disorders in Neurologic Illness.

Patients with neurologic illness frequently develop secondary mood disorders that are broadly categorized as unipolar or bipolar. Accurate diagnosis is essential because the treatment of unipolar disorders is markedly different from that of bipolar disorders. Aggressive treatment of mood disorders improves quality of life, reduces morbidity and mortality, and may prevent worsening of both psychiatric and neurologic disease. Antidepressants and psychotherapy are both effective for patients suffering from depressive disorders. Choice of antidepressant depends on the patient's particular symptom

Neuroimaging studies of mood disorders.

Neuroimaging studies of major depression have identified neurophysiologic abnormalities in multiple areas of the orbital and medial prefrontal cortex, the amygdala, and related parts of the striatum and thalamus. Some of these abnormalities appear mood state-dependent and are located in regions where cerebral blood flow increases during normal and other pathologic emotional states. These neurophysiologic differences between depressives and control subjects may thus implicate areas where physiologic activity changes to mediate or respond to the emotional, behavioral, and cognitive manifestation

Nosology of chronic mood disorders.

In attempting to assess the complex relationship of chronic depression with other conditions to which it may be a predisposition, an accompaniment, or a consequence of, a few concluding caveats may be useful. It must be recognized that chronic depression has a wide variety of different presentations and causes. Current definitions of major depressive disorder and dysthymic disorder may create a misleading distinction of what may be two aspects of the overall course of a depressive disorder. Furthermore, much of what is considered to be comorbidity of other disorders with chronic depression act

Inpatient treatment of mood disorders.

The present work provides a short-term, intensive, and integrative model for the assessment and treatment of mood disorders within a hospital setting. This model was developed and implemented at the Milwaukee Psychiatric Hospital in a specific inpatient program, a 15-bed unit for patients suffering from dysthymia, cyclothymia, major depression, and bipolar disorders. This paper describes the program and its philosophy, the target population, and assessment and treatment interventions. The data collected included pre- and posttreatment measurements of psychopathology and cognitive and emotional

[Drug-induced organic mood disorders].

Firstly, we reviewed the drugs (ex, corticosteroid, interferon) which were thought to be related to organic mood disorders in the literature, and discussed a number of problems with assessment of drug-induced mental disorders. Next, we investigated 1) the antidepressant-induced switch rate from depression to mania and 2) the latency from administration to manic onset in three groups (Major depression: single episode, Major depression: recurrent, Bipolar disorder: depressed). Our data showed, 1) the switch rate was 17.2, 14.3, 32.0% and 2) the latency to onset was 69.9, 65.9, 37.1 days, respect

The spectrum of mood disorders.

The problem of classifying mood disorders still lacks an acceptable solution. Interrater reliability studies have shown that this problem is more prevalent in mood disorders compared with other psychiatric conditions. Historically the problem originates from the distinction between Geisteskrankheiten and Gemütskrankheiten (mental and mood disorders) because the domain covered by the word mood has been progressively reduced. The fact that neurotic disorders are "emotional disorders" further complicates this field. The words anxiety and depression refer to a disorder, to a symptom, and to everyd

[Comorbidity between eating disorders and mood disorders: review].

OBJECTIVE: Comorbidity between eating disorders (ED) and mood disorders is a major issue when evaluating and treating patients with anorexia nervosa (AN) or bulimia nervosa (BN). In the literature, estimated comorbidity rates of mood disorders in subjects with ED differ widely across studies. Obviously, it is difficult to compare results from various sources because of differences in methods of assessment of depressive symptoms and in diagnostic criteria for both ED and mood disorders. Furthermore, few studies have included control groups, and, since mood disorders are among the most frequent
