Brainstem injury in motor vehicle crashes.

OBJECTIVE: This is a descriptive study of the frequency and risk for brainstem injury by crash type, belt use, and crash severity (delta-V). NASS-CDS electronic cases were reviewed to see whether the transition from vehicles without advanced airbags and seat belts and side airbags and curtains to vehicles with the safety technologies has influenced the risk for brainstem injury. METHODS: 1994-2013 NASS-CDS was analyzed to determine the number of brainstem injuries in nonejected adults (15+ years old) in vehicle crashes. Crashes were grouped by front, side, rear, and rollover. The effect of be

Suicide by motor vehicle "accident" in Queensland.

OBJECTIVE: Around the world, a substantial proportion of motor vehicle crash deaths are recognized as "hidden" suicides. This project sought to progress understandings of drivers who used a motor vehicle to die by suicide in Queensland, Australia, during the period 1990 to 2007. METHODS: Data for this study were derived from the Queensland Suicide Register and forensic crash investigation case records. Analysis focused on life circumstances and events preceding the death, physical and mental illnesses, past suicidality, and indication of suicide intent (e.g., suicide notes or statements). RE

Motor vehicle collisions with large animals.

Motor vehicle collisions MVC with large animals are a worldwide problem. In this review, we aim to analyze the mechanisms and patterns of human injuries caused by MVC with large animals and various ways to prevent them. Reported studies on large animals that can cause such accidents include the moose, camels, deer, and kangaroos. The moose causes a typical rear-and downward deformity of the roof of the car. The camel falls on the roof of the car causing cervical and head injury to the occupants. Injuries caused by kangaroos and deer are usually mild. Injuries may be caused by direct collision

Psychiatric consequences of motor vehicle accidents.

Psychiatric complaints are frequent following motor vehicle accidents and may be major predictors of persistent pain and other complaints. Outcomes are not related closely to the nature or severity of any medical injury. Psychiatric problems often are unrecognized and untreated. There is a need for more behaviorally inferred routine care, early recognition of complications, and the use of psychological and pharmacological interventions.

The motor vehicle collision injury syndrome.

OBJECTIVE: The objective of this study was to examine the central nervous system changes that may occur after acceleration/deceleration injuries in motor vehicle accidents. BACKGROUND: Occupants of motor vehicles involved in a collision often develop a disabling syndrome consisting of head, neck, and back pain; impaired short-term memory and concentration; fatigue and a loss of stamina; poor balance; and a change in personality. Injury victims experience a loss of motivation, emotional lability, and a decrease in libido. The major features of this injury syndrome are subjective, and there usu

[Motor vehicle accidents and eye injuries].

The medical records of 24,632 patients treated in our surgical emergency service over a 3-year period were reviewed to determine the frequency and characteristics of ocular trauma caused by motor vehicle accidents (MVA). MVA-related injuries accounted for 13.9% of all visits to the service and involved 1106 of the patients (33%), of whom 77% were young males. At least 1 pathological finding was found in 858 (77.6%) and 169 (15.2%) were admitted. 16 patients sustained very severe ocular injuries which resulted in poor vision.

Alcohol, the elderly, and motor vehicle crashes.

To document the prevalence of alcohol or drug use among elderly drivers admitted to a Level 1 trauma center after motor vehicle crashes, charts from 180 drivers age 60 years or older who were admitted to an urban Level 1 trauma center after motor vehicle crashes were retrospectively reviewed. Overall, 14% of the patients had a positive blood alcohol screen; among men, 21% had a positive screen. Only 55% of the patients were discharged to home. Only one patient ( < 1%) had a toxicology screen positive for another drug abuse. Alcohol/drug abuse counselling was offered to only one patient. These

Infant involved in a motor vehicle accident.

The authors present and discuss the differential diagnosis for a 7-month-old infant who was seen in the ED after having been involved in a motor vehicle accident. The infant was subsequently found to have an odontoid fracture. Strict attention to the mechanism of injury is emphasized for appropriate evaluation of this patient's condition. An infant who becomes airborne in a car that is extensively damaged deserves an aggressive workup. In a child of this age, examination for subtle or even quite significant injury is difficult. Therefore the focus should be on the potential for injury. This ar

Fetal death in motor vehicle accidents.

This study represents a series of cases of fetal death secondary to maternal involvement in a motor vehicle accident. During the period 1982 to 1985, nine cases were reported to the county coroner's office. In approximately 50% of the cases, other injuries to the victim, excluding injuries related to the pregnancy, were minor. Evidence of fetal distress was not always apparent at the time of initial evaluation. In all cases, placental abruption was documented. The injury mechanism was generally impact with the steering wheel. At the time of the accident, none of the patients were using seat be

Motor Vehicle Crash and Hospital Charges in Front- and Rear-Seated Restrained and Unrestrained Adult Motor Vehicle Occupants.

UNLABELLED: There are reports that historically higher mortality observed for front- compared to rear-seated adult motor vehicle (MV) occupants has narrowed. Vast improvements have been made in strengthening laws and restraint use in front-, but not rear-seated occupants suggesting there may be value in expanding the science on rear-seat safety. METHODS: A linked 2016-2017 hospital and MV crash data set, the Crash Outcomes Data Evaluation System (CODES), was used to compare characteristics of front-seated (n = 115,939) and rear-seated (n = 5729) adults aged 18 years and older involved in a MV

Trauma caused by three-wheel motor vehicles--an unrecognized epidemic?

A retrospective study of 54 consecutive cases was done to evaluate severity of trauma caused by three-wheel motor vehicles. Skeletal trauma predominated (22 of 54 patients [41%]). Two deaths were reported (4%), as were numerous other injuries. Men were injured more frequently (46 of 54 [85%]) than women. Riders seldom wear protective helmets (two of 54 [4%]), thus contributing to severity of injury. The injury pattern differs from that reported in motorcycle accidents. Specific recommendations are made that address these problems.

Obesity and vehicle type as risk factors for injury caused by motor vehicle collision.

BACKGROUND: This study sought to describe variations in the risk of motor vehicle collision (MVC) injury and death by occupant body mass index (BMI) class and vehicle type. We hypothesized that the relationship between BMI and the risk of MVC injury or mortality would be modified by vehicle type. METHODS: This is a retrospective cohort study of occupants involved in MVCs using data from the Crash Injury Research and Engineering Network and the National Automotive Sampling System Crashworthiness Data System. Occupants were grouped based on vehicle body style (passenger car, sport utility vehic

Vehicle factors and outcomes associated with hand-out-window motor vehicle collisions.

BACKGROUND: Patients can experience devastating injuries when an upper extremity travels out of a window during a motor vehicle collision. These injuries are typically very severe and may even result in loss of the affected limb. The purposes of this retrospective study were to determine the clinical fate of these extremities and analyze for common factors during the collisions. METHODS: A retrospective chart review of 20 patients between 2003 and 2010, evaluating the patient's age, arm involvement, number and types of surgeries, as well as the Mangled Extremity Severity Score (MESS) was cond

Audit of Motor Vehicle Accidents at a Trauma Center.

Background Motor vehicle accidents (MVAs) are the leading cause of accidental deaths in India. An audit of trauma cases is required in order to improve hospital policy and patient care, as well as to change the attitude and perspective of healthcare staff. Methods A retrospective observational study was performed on MVA trauma victims admitted to a tertiary trauma care center. Parameters included mean age with range, gender distribution, length of hospital stay (LOS), anatomical location of injuries, and the percentage of age groups and number of patients undergoing a nonoperative approach ver

Causes of fatalities in motor vehicle occupants: an overview.

Injuries from motor vehicle collisions are frequently encountered in routine forensic practice. While the most common lethal events involve blunt force trauma with injuries to the head and neck, chest, abdomen, pelvis and limbs, review of the literature and case files shows that a wide variety of other fatal situations can occur that may involve sharp force and penetrating trauma, incineration, drowning, asphyxia, organic diseases and combinations of these. The following overview details potential factors that may contribute to death following vehicle crashes.
