Furosemide impairs nasal mucociliary clearance in humans.

Furosemide, a potent diuretic, affects ion and water movement across the respiratory epithelium. However, the effects of furosemide, as clinically used, on mucociliary clearance, a critical respiratory defense mechanism, are still lacking in humans. Fourteen young healthy subjects were assigned to three random interventions, spaced one-week apart: no intervention (control), oral furosemide (40mg), and furosemide+oral volume replacement (F+R). Nasal mucociliary clearance was assessed by saccharine test (STT), and mucus properties were in vitro evaluated by means of contact angle and transportab

Mucociliary clearance patterns following endoscopic sinus surgery.

Detailed endoscopic evaluations of mucociliary transport were performed to define qualitatively the clearance patterns from the ethmoid cavity, sphenoidal sinus, and related nasal structures following sinus surgery for inflammatory disease. Forty patients participated in this clinical study. They were selected to represent a spectrum of disease severity and surgical outcomes. Powdered graphite was used as an insoluble tracer medium, and videodocumentation allowed multiple sites to be examined during a single study session. Mucociliary clearance was not uniformly observed to be linear, but was

Tacrolimus impairs airway mucociliary clearance of rats.

OBJECTIVES: Tacrolimus (TAC) is the most widely used immunosuppressive agent after lung transplantation. Considering that the ciliary beat frequency (CBF) mainly depends on the cytoplasmic calcium concentration and that TAC can affect this due to its binding with the intracellular immunophilin FKBP12, we hypothesized that TAC could also impair the airway mucociliary clearance of rats. METHODS: Sixty rats were divided into two groups (n = 30 each): Control = water; TAC = tacrolimus. After 7, 15 or 30 days of treatment, ten animals from each group were euthanized and the following parameters we

Does menopause effect nasal mucociliary clearance time?

The aim of this study was to investigate the effect of menopause on nasal mucociliary clearance time by comparing the results of premenopausal and postmenopausal women. A total of 60 women met the criteria and were divided into two groups: premenopausal women (n = 30) and postmenopausal women (n = 30). Nasal mucociliary clearance time of these women was measured and compared. Moreover, the correlation between nasal mucociliary clearance time and menopause duration in postmenopausal women was evaluated. Statistical analysis was performed using the SPSS (Statistical Package for the Social Scienc

Mucociliary clearance impairment after zygomaticomaxillary-orbital fractures.

OBJECTIVE: The purpose of this study was to examine the influence of zygomaticomaxillo-orbitalis (ZMO) fracture and its symptoms on mucociliary transport (MCT). STUDY DESIGN: The study encompassed 144 patients who sustained ZMO fracture. A saccharine test was conducted in every patient both on the side of fracture and on the unaffected side. The results were analyzed in connection with the patients' age, sex, degree of injury, method of treatment, time since fracture, and duration of surgery. RESULTS: It was shown that MCT was considerably impaired on the affected side compared with the cont

Primary ciliary dyskinesia and nasal mucociliary clearance.

In this study, nasal mucociliary clearance is investigated in patients with primary ciliary dyskinesia, patients with other respiratory tract infections and healthy persons. Mucociliary clearance is established with a drop of albumen labelled with 99m-Tc. From this study it appears that measurement of mucociliary clearance in the nose can serve as a reliable screening method. If a patient with respiratory tract infections shows a normal mucociliary clearance rate there is no primary ciliary dyskinesia, and a biopsy of ciliated epithelium is not necessary. If, after repeated examinations, a nor

Mucociliary clearance in early simple chronic bronchitis.

Tracheobronchial mucociliary clearance was measured in 37 patients with early simple chronic bronchitis. A non-invasive radio-aerosol technique with inhalation of 99Tcm labelled 5-microns polystyrene particles followed by assessment of radio-nuclide distribution by posterior gamma-camera scans was employed. The mucociliary clearance of the bronchitics was significantly lower than clearance of a non-smoker control group. An interim analysis of the effect of treatment with a surfactant stimulating drug, ambroxol, suggested a dosage-dependent tendency to a faster mucociliary clearance than seen i

Role of mucociliary clearance system in respiratory diseases.

Mucociliary clearance system is the primary innate defense mechanism of the lung. It plays a vital role in protecting airways from microbes and irritants infection. Mucociliary clearance system, which is mediated by the actions of airway and submucosal gland epithelial cells, plays a critical role in a multilayered defense system via secreting fluids, electrolytes, antimicrobial and anti-inflammatory proteins, and mucus onto airway surfaces. Changes in environment, drugs or diseases can lead to mucus overproduction and cilia dysfunction, which in turn decrease the rate of mucociliary clearance

Impairment of lung mucociliary clearance in pigeon fanciers.

Lung mucociliary clearance was measured in 15 pigeon fanciers. The study group was subdivided into two: a precipitin-positive group (n = 10; mean +/- SEM age 45 +/- 5 years) with circulating blood precipitins and a precipitin-negative group (n = 5; mean +/- SEM age 40 +/- 3 years) without. Clearance was measured using an objective, noninvasive radioaerosol technique. The data for both groups were compared with those of matched control groups of healthy subjects. The mean +/- SEM area under the tracheobronchial retention curves (AUC) over the 6-h observation period was 257 +/- 27 %h for the pre

Nasal mucociliary clearance in patients with diabetes mellitus.

Fifty insulin-dependent diabetics and 50 non-diabetics without a history of nasal disease have been studied for nasal problems including mucociliary function complicated by diabetes mellitus. For the diabetics, the mean value of nasal mucociliary clearance was considerably decreased and this was more often associated with dry noses but increased pH-values, both of which were higher than those for non-diabetics (p less than 0.05). As suggested in earlier literature, more aggressive nasal moistening therapy must be recommended for those patients presenting with these conditions in the light of s

The effect of vidian neurectomy on nasal mucociliary clearance.

The physiological factors controlling mucociliary transport are largely unknown. Nasal mucociliary clearance was measured using the saccharin test in seven patients who had previously undergone vidian neurectomy for vasomotor rhinitis. Persistent denervation was suggested by impaired tear secretion. The control group consisted of thirty-four patients with vasomotor rhinitis who had not had surgery. There was no significant difference in clearance times between the two groups, suggesting that mucociliary transport is well preserved in the absence of neural control.

Clearance of middle ear effusions by the mucociliary system.

There are two extreme types of middle ear effusion leading to hearing loss (a) a rubber-like effusion seen in secretory otitis media and (b) a water-like effusion seen in serous otitis media. The possibility is considered that the degree of crosslinking in these two extreme cases is the basis of an altered mucus transport rate that leads to an accumulation of effusions and hence impaired hearing. It has been shown (King et al., 1974) that the requisite rheological property for transport activity is not unique to mucus structural macromolecules but is found with other polymeric systems that are

Improving mucociliary clearance in chronic obstructive pulmonary disease.

Patients with COPD usually experience mucus hypersecretion as a result of airway inflammation and response to noxious stimuli. These in turn lead to worsening airway resistance, impaired airflow, increased work of breathing, dyspnoea and exercise intolerance. Mucus hypersecretion may also lead to increased exacerbations and poor health related quality of life (HRQL). Institution based pulmonary rehabilitation programs incorporating airway clearance techniques have been shown to improve HRQL, reduce dyspnoea and improve exercise tolerance but are often difficult to provide due to restricted acc

Control of Mucociliary Clearance in Airway Host Defense.

Respiratory infections remain a major threat to global health. To counter these insults, the respiratory system has evolved multilayered defense strategies that integrate anatomical and mechanical barriers with both innate and adaptive immune responses. Mucociliary clearance (MCC) serves as a cornerstone of airway host defense, whereby coordinated ciliary beating at the luminal surface of the airway epithelium drives the efficient removal of inhaled particles and pathogens. This process provides robust, broad-spectrum protection against both routine environmental exposures and emerging respira

Editorial: Mucus and the mucociliary interface: continuity and clearance.

Vitamin D deficiency affects a large part of the population of elderly people, especially women, who live in moderate climate countries due to a reduced amount of vitamin D in the diet (small sea fish consumption) and reduced content of 7-dehydrocholesterol, which causes decreased skin synthesis. The lowest seasonal concentration of 25(OH)D3 is usually observed during winter and spring. Sun exposure influences 25(OH)D3 concentration more strongly in men than in women. Sociodemographic factors that increase the risk of vitamin D deficiency in the elderly include poor environmental conditions, l

A modification of the saccharine test for nasal mucociliary clearance.

The saccharine test for assessing mucociliary clearance in the nose is the most practicable method. As the used sodium salt of saccharine is well water soluble, there is suspicion of interference with the very delicate two-layer-system of mucus or transport merely in the periciliary fluid, thus bringing about methodical errors. This is avoided by using inert tracers such as radioactively tagged ion exchange resin particles, which on the other hand must be observed with expensive equipment. A synthesis of both methods is demonstrated by using saccharine loaden ion exchange resin particles. The
