Periodontitis and multimorbidity: A conceptual framework.

Periodontitis is a chronic inflammatory disease that has been consistently linked to a range of chronic conditions, including diabetes, cardiovascular disease, and neurodegenerative disorders. Multimorbidity, defined as the co-occurrence of two or more chronic conditions in an individual, shares complex and multifactorial pathways with periodontitis, with both increasing in prevalence with age. However, the nature of their relationship, whether driven by shared risk factors or causal interactions, remains unclear. Several independent conceptual models have been developed to identify the determ

[Multimorbid patients in endocrine surgery].

Endocrine surgery in multimorbid, frail and geriatric patients is increasing, is often urgent and characterized by special risk constellations. Successful parathyroid gland surgery nearly always results in a marked improvement, irrespective of the specific risk profile of the patient. Except for critical intubation and mediastinal interventions in the risk profile, surgery of the thyroid glands is predominantly beneficial and justifiable even in frail patients. For surgery of the adrenal glands and for gastroenteropancreatic neuroendocrine tumors (GEP-NET), the expected extension of resection,

The evolving concept of multimorbidity and migraine.

Migraine presents with high prevalence and similar clinical course with different disorders such as neurological, psychiatric, cardio- and cerebrovascular, gastrointestinal, metabolic-endocrine, and immunological conditions, which can often cooccur themselves. Multifaceted mechanisms subtend these comorbidities with a bidirectional link. First, a shared genetic load can explain the cooccurrence. Second, comorbid pathologies can promote disproportionate energetic needs, thalamocortical network dysexcitability, and systemic transient or persistent proinflammatory state, which may trigger the act

Multimorbidity patterns in people with HIV.

PURPOSE OF REVIEW: With the progressive aging of populations of people with HIV (PWH), multimorbidity is increasing. Multimorbidity patterns, that is groups of comorbidities that are likely to co-occur, may suggest shared causes or common risk factors. We review the literature regarding multimorbidity patterns identified with data-driven approaches and discuss the methodology and potential implications of the findings. RECENT FINDINGS: Despite the substantial heterogeneity in the methods used to identify multimorbidity patterns, patterns of mental health problems, cardiovascular diseases, met

Understanding multimorbidity: insights with graphical models.

BACKGROUND: The use of graphical models in the multimorbidity context is increasing in popularity due to their intuitive visualization of the results. A comprehensive understanding of the model itself is essential for its effective utilization and optimal application. This article is a practical guide on the use of graphical models to better understand multimorbidity. It provides a tutorial with a focus on the interpretation of the model structure and of the parameter values. In this study, we analyze data related to a cohort of 214,401 individuals, who were assisted by the Local Health Unit o

Multimorbidity, behaviour and QoL in ageing dogs.

Older dogs frequently develop multiple chronic conditions, accompanied by behavioural and functional changes that may adversely affect their quality of life (QoL). However, the relative contributions of multimorbidity, behavioural manifestations, and functional performance to owner-perceived QoL remain incompletely understood. This cross-sectional study analysed anonymous online questionnaire responses from the owners of 789 dogs aged ≥ 8 years. Information on chronic health conditions, behavioural manifestations, functional performance, and owner-perceived QoL was collected using a structured

Multimorbidity in Atrial Fibrillation: Impact on Outcomes.

BACKGROUND: Multimorbidity is common in patients with atrial fibrillation (AF); however, the impact of the number and type of comorbid conditions on outcomes remains uncertain. METHODS: This cohort study included patients with new-onset AF from a Midwest region between 2013 and 2017. Eighteen chronic conditions at the time of AF were classified into groups: cardiometabolic, other somatic, and mental health. Cox regression determined associations between the number of each condition type with death, ischemic stroke/transient ischemic attack, and congestive heart failure, stratified by age. RE

Corneal Neurotization in Elderly Patients with Multimorbidity.

PURPOSE: Corneal neurotization (CN) is a surgical technique to improve corneal sensitivity and the ocular surface in eyes with neurotrophic keratopathy. It has been predominantly advocated in patients with congenital corneal hypesthesia. Neurotrophic keratopathy is however much more frequent at an older age and in often multimorbid patients. We report our experience with corneal neurotization in patients with multimorbidity. METHODS: Seven eyes of 5 patients (f:m = 2:3, median age 70 ± 12 years) suffering from neurotrophic keratopathy due to Diabetes mellitus (n = 3), post Herpes Zoster kerat

Robustly measuring multimorbidity using disparate linked datasets.

BACKGROUND: Measurement of multimorbidity, the co-occurrence of two or more conditions in the same individual, is highly variable which limits the consistency and reproducibility of research. METHODS: Using data from 172,563 UK Biobank (UKB) participants and a cross-sectional approach, we examined how choice of data source affected estimated prevalence of 80 individual long-term conditions (LTCs) and multimorbidity. We developed code-list-based algorithms to determine the prevalence of 80 LTCs in (1) primary care records, (2) UKB baseline assessment, (3) hospital/cancer registry records, and

[Perioperative care about a patient with multimorbidity].

The assumption of accelerated postoperative recovery according to the ERAS (Enhanced Recovery After Surgery) method is good condition of the patient prepared for surgery and gently operated on. Application of ERAS in postoperative care in the situation of patient with multimorbidity requires an individual approach and greater pre-operative preparation. During the healing process, the operation causes a systemic inflammatory reaction in the body, which is proportional to the size of the surgery trauma. There is fluid movement between the intravascular and interstitial spaces, increased protein

Multimorbidity in bronchiectasis: a systematic scoping review.

INTRODUCTION: Multimorbidity, the coexistence of two or more chronic conditions, has been extensively studied in certain disease states. Bronchiectasis aetiology is complex and multimorbidity is insufficiently understood. We performed a scoping review, summarising the existing literature and identifying deficits. METHOD: A literature search of the electronic databases PubMed, CINAHL and EMBASE was conducted following PRISMA guidelines. Observational, interventional, qualitative, randomised control trials and systematic reviews were included. The main objective was to identify prevalence, prog

Multimorbidity and mortality: A data science perspective.

BACKGROUND: With multimorbidity becoming the norm rather than the exception, the management of multiple chronic diseases is a major challenge facing healthcare systems worldwide. METHODS: Using a large, nationally representative database of electronic medical records from the United Kingdom spanning the years 2005-2016 and consisting over 4.5 million patients, we apply statistical methods and network analysis to identify comorbid pairs and triads of diseases and identify clusters of chronic conditions across different demographic groups. Testosterone Deficiency, Weakness, and Multimorbidity in Men.

The purposes of this study were to evaluate the association between total testosterone (TT) deficiency and weakness on multimorbidity in men. Analyses were performed to examine the prevalence of multimobidity among young, middle-aged, and older men, with and without testosterone deficiency. Multivariate logistic models were also used to determine the association between age-specific TT tertiles and multimorbidity, adjusting for key sociodemographic variables, as well as a secondary analysis adjusted for grip strength. Multimorbidity was more prevalent among men with testosterone deficiency, co

Tobacco and alcohol use disorders: Evaluating multimorbidity.

There is growing interest in applying the multimorbidity model for mental health disorders - i.e. the interactive effects of co-occurring diagnoses. Alcohol use disorder (AUD) and tobacco use disorder (TUD) often occur together, but distinctive correlates of their co-occurrence have not been studied. Veterans treated by the Veterans Health Administration (VHA) nationally in FY 2012 with diagnoses of both AUD and TUD were compared to those with only AUD or only TUD on socio-environmental factors, medical and psychiatric diagnoses, and service use. Multimorbid effects were defined as those in wh
