Munchausen Syndrome by Proxy (MSBP), or Fabricated or Induced Illness (FII), is a form of physical and emotional child abuse and maltreatment that remains frequently under-detected due to a significant lack of clinical awareness. This case report aims to highlight the challenges of diagnosing MSBP, its impact on victims, and the ethical implications for healthcare professionals. A 37-year-old woman charged with filicide of two of her offspring and deliberate intoxication of her remaining 3-year-old boy was diagnosed with MSBP. Despite the tragic outcome for two siblings, the timely interventio…

Childhood-onset systemic lupus erythematosus (cSLE) is a chronic inflammatory multisystem autoimmune disease that requires multiple differential diagnoses. Munchausen by proxy syndrome (MBPS) is a form of child abuse, where a caregiver intentionally creates a medical history and induces or fabricates signs or disease in a patient. To our knowledge, there is no case report of MBPS mimicking cSLE diagnosis. We reported herein a 9-year-old male patient, with a history of multiple hospitalizations due to seizures with altered levels of consciousness. The mother reported malar rash, photosensitivit

Munchausen syndrome by proxy is an increasingly reported insidious disorder in which illness in a child is fabricated and/or induced by the parent. Over a 5-year period at North Carolina Children's Hospital 10 such children were identified after having presented to the Pediatric Surgical Service. In reviewing this experience, we have identified two patterns of presentation. Apnea, seizures, and cyanosis comprised the pattern most frequently seen in infants. A history of persistent diarrhea and vomiting, although seen in two infants, was the more common pattern in older children. As they got ol…

The term Munchausen Syndrome by Proxy describes a pattern of physical abuse in which a perpetrator produces or fabricates illness in a victim. Victims are almost always children. We alternatively call this behaviour medical child abuse. Our report describes five recent cases. Two presented with infantile apnoea, one with alleged hypoventilation, one with recurrent vomiting and one with recurrent infections. Three of the perpetrators were mothers, two were fathers. In three cases diagnoses were confirmed by covert video surveillance. Contrary to popular beliefs, this behaviour is not rare and i…

Fifty six children who had been victims of fabricated illnesses and 82 of their 103 siblings were studied. In addition to the index fabrication, 64% of index children had had other illnesses fabricated by their mothers. Twenty nine per cent of the index children had a history of failure to thrive and 29% a history of non-accidental injury, inappropriate medication, or neglect. Seventy three per cent of the index children had been affected by at least one of these additional problems. Eleven per cent of the siblings had died in early childhood, the cause of death not being identified. Thirty ni

A five-year-old girl presented with profound growth failure, lethargy, vomiting and acidosis. A diagnosis of Munchausen syndrome by proxy was made after the demonstration of egg albumin, diphenhydramine and phenothiazine metabolites in her urine. Growth improved dramatically, but a subsequent child in the family died of sudden infant death syndrome.

A case of the girl who underwent multiple hospitalizations is presented. Gastrointestinal disorders were seen in the infancy together with skin rash of allergic type, hypoglycaemia without any clear reason in the fourth year of age, and polyuria with hyponatremia and hypokalemia since the sixth year of age. Mother's lack of concern was unexplainable in view of the deteriorating child's health. Samples of the urine and faeces supplied by the mother have shown the laboratory findings suggesting that potassium chloride was added to the faeces and natrium hydrocarbonate--to the urine. Urine collec

Munchausen syndrome by proxy is a kind of child abuse in which affected children are often hospitalized for long periods and endure repetitive, painful and expensive diagnostic attempts. We present herein two toxicologically confirmed cases of Munchausen syndrome by proxy. Case 1 is a 16-month-old male who had fever, peripheral cyanosis, tremor, and reported cardiac arrest. Symptoms recurred in the hospital when the mother administered fluids. Toxicology detected 3.5 ng/ml mercury (Hg) in the fluid and 9.4 microg Hg/g creatinine in the urine. Case 2 is a 14-year-old female who had irregular bl…

Fifty four children were studied 1-14 (mean 5.6) years after fabrications of illness had been identified. Thirty of the 54 children were living in families with their biological mothers and 24 were with other family members or in substitute families. Further fabrications were identified for 10 children who had been living with their mothers and there were 'other concerns' for a further eight children. Thirteen children residing with mother and 14 not residing with mother at follow up had a range of disorders including conduct and emotional disorders, and problems related to school, including d

Background: Munchausen syndrome by proxy (MSBP) or factitious disorder imposed on another (FDIA) is a bizarre psychiatric entity, consisting of the fabrication of symptoms and alteration of laboratory tests by a caregiver. It is considered a serious form of child abuse. Alarm signs are frequent medical visits and strange symptoms that are never objectified during hospitalization. Methods: This case sets a bright light on how difficult the diagnosis and management of FDIA is and the severe consequences this disease has on a defenseless child. Results (Case report): A boy who is 3 years 8 months…

Two children were thought to have an atypical gastroduodenal motility disorder because of the history and clinical course; both had received parenteral alimentation because of claims of inability to tolerate enteral feedings, and both continued to have unusual medical problems during parenteral alimentation. Both children had motility studies that were interpreted by a pediatric gastroenterologist to be "abnormal" and "diagnostic" of a motility disorder, but each was eventually shown to have a behavioral abnormality related to Munchausen syndrome by proxy.

Clinical history and inappropriate insulin secretion during hypoglycemic episodes permit the diagnosis of hyperinsulinism. We report 2 cases … ercent (2 of 250) of all cases of hyperinsulinemic hypoglycemia in our unit have been identified as Munchausen syndrome by proxy. Atypical disease history should raise the question of factitious hypoglycemia.

A four-month-old boy with pseudo-seizures was extensively investigated for fits and gastro-oesophageal reflux and subsequently treated for presumed epilepsy. It was finally established that his mother was inducing the "seizures" by asphyxiation and subsequent interviews established that information concerning the parents had been falsified.

BACKGROUND: Child abuse is a major public health crisis and is on the rise. Dermatologists are frequently involved in its evaluation in differentiating abusive injuries from accidental injuries and in excluding pathological conditions that may mimic abuse. Battered child syndrome or physical abuse is not only a common form of child abuse but can also result from a rarer form of child abuse known as Munchausen syndrome by proxy. In this form of abuse, mother, who is the usual perpetrator, induces an illness or abuses the child for her own self-serving psychological needs.
METHODS: We report tw…

Factitious Disorder Imposed on Another (FDIA), formerly known as Munchausen Syndrome by Proxy, is a rare form of abuse where parents induce or fabricate symptoms in their child, exposing them to unnecessary and potentially harmful medical treatments. In over 90% of cases, the perpetrators are mothers. A father was put on trial for the murder of his three-month-old daughter and was eventually convicted of repeatedly inducing respiratory crises, ultimately causing her death. Investigations began after multiple visits to the Emergency Department for respiratory episodes reported by the father but…

Munchausen syndrome by proxy (MSBP), also known as fabricated or induced illness in a child by a caretaker, is a form of abuse where a caregiver deliberately produces or feigns illness in a person under his or her care, so that the proxy will receive medical care that gratifies the caregiver. The affected children are often hospitalized for long periods and endure repetitive, painful and expensive diagnostic attempts. We present an analytically confirmed case of MSBP by alimemazine. A 3-year-old boy was brought repetitively to a Pediatric Emergency Department by his mother because he presented…

Renal and urologic problems in pediatric condition falsification (PCF) or Munchausen by proxy (MSP) can result in serious diagnostic dilemma. Symptoms of hematuria, pyuria and recurrent urinary tract infections have occasionally been described. However, MSP presenting as azotemia has not been previously reported. We describe the case of an unfortunate boy who had to undergo unnecessary hemodialysis for persistent hyperkalemia and azotemia before a final diagnosis of the falsification of investigations by the parents was made.
