BACKGROUND: Adrenal myelolipoma is a rare and benign, hormonally inactive tumor frequently discovered incidentally. Because of the increasing rate of detection of adrenal myelolipoma, use of the correct diagnostic examination and treatment, with respect to surgical excision or regular controls, is continually gaining importance. We report herein on the largest series of surgically treated patients with adrenal myelolipoma from a single institute.

Myelolipoma is a benign tumor composed of mature adipose tissue and hematopoietic tissue. It was mainly found in the adrenal grand, but there have been reports of extra-adrenal locations. Only 7 cases of mediastinal myelolipoma have been reported ever. We have experienced a case of mediastinal myelolipoma surgically resected from a 55-year-old-man. PURPOSE: To correlate clinical and pathologic patterns of myelolipoma with the computed tomographic (CT) appearance.
MATERIALS AND METHODS: All cases of myelolipoma accessioned by the Armed Forces Institute of Pathology from 1981 through 1997 were reviewed. CT images were scored for the location and size of each myelolipoma and the presence of calcification, hemorrhage, fat, and pseudocapsule. In a 25-year-old female in adrenocortical carcinoma with a myelolipoma and areas of ossification was removed by operation. The carcinoma did not penetrate the myeloadipose structure. The finding of an actively growing bone within the stroma of the adenocarcinoma can be explained by the local action of androgens produced by the tumour tissue.

INTRODUCTION: Adrenal myelolipoma is an invariably benign neoplasm of the adrenal gland that is the second most common primary adrenal incidentaloma following adrenocortical adenomas. It is composed of elements of adipose tissue and extramedullary hematopoiesis. Hypotheses on stem cells and hormonal factors have been formulated regarding its pathogenesis that is still obscure. Despite its benign behavior, adrenal myelolipoma is clinically relevant as it might cause significant difficulties in the differential diagnosis of adrenal tumors.

We report a case of adrenal myelolipoma occurring in a 56-year-old man. The patient complained of lumbago and gross hematuria in October, 1981 and consulted a local doctor who pointed out a right ureteral stone and downward deviation of the left kidney. This is a case report of an adrenal myelolipoma, accidentally diagnosed during a work-up for bladder tumor. A 67-year-old male presented with the chief complaint of gross hematuria. He was subsequently diagnosed as having a bladder tumor, which was resected transurethrally and was found to be a transitional cell carcinoma of Grade 2 and Stage pT2. During further examination for metastasis computed tomography (CT) scan revealed a round tumor (approximately 5 cm in diameter) in the left adrenal. A tentative diagnosis was reached based on the scan, and surgery was undertaken to remove the tumor.

A 12-year-old female spayed, Silken Windhound dog was presented with a 3-month history of lethargy and cervical and lumbosacral spinal pain. No significant abnormalities were noted on CBC or serum biochemical assays. Magnetic resonance imaging of the spine demonstrated a soft tissue mass within the ventral and right epidural space at the level of the L7 vertebra. During surgery, a pale brown mass was identified within the epidural fat. Cytologic and histopathologic examinations demonstrated that the mass was composed of adipose tissue and hematopoietic elements, consistent with a myelolipoma.
