Effect of aspirin on late preconditioning against myocardial stunning in conscious rabbits.

OBJECTIVES: The goal of this study was to investigate the effect of three different doses of acetylsalicylic acid (aspirin) (ASA) on the late phase of ischemic preconditioning (PC) against myocardial stunning. BACKGROUND: Although recent evidence indicates that the late phase of ischemic PC is mediated by cyclooxygenase-2 (COX-2), the effect of nonsteroidal anti-inflammatory drugs (NSAIDs) that inhibit COX-2 activity on late PC has not been evaluated; ASA is the most widely used NSAID. Therefore, we determined whether ASA impedes the development of late PC. METHODS: Conscious rabbits underwe

[Stimulation of the delta 1-receptors help to decrease reperfusion induced myocardial stunning].

In vivo or in vitro administration of a omega 1 receptor agonist d-SKF 10,047 (1 mg/kg intravenously or 10 mg/l in vitro) promoted an increase in the resistance of isolated perfused rat heart to ischemia/reperfusion injury. Both in vivo and in vitro stimulation of omega receptors prevents development of reperfusion contracture and release of creatine kinase and increases the developed pressure, double product, +dP/dt, and -dP/dt in the left ventricle. Activation of omega receptors has no significant effect on the occurrence of reperfusion arrhythmias ex vivo. Stimulation of cardiac sigma recep

Myocardial stunning caused by sympathetic nerve injury after an operation on cervical vertebrae.

In this case, electrocardiographic inverted T waves appeared after cervical laminaplasty and echocardiogram showed temporary wall motion abnormality. Myocardial metaiodobenzylguanidine (MIBG) uptake was obviously reduced in the same area where the wall motion abnormalities appeared in the echocardiogram, although no abnormalities were detected with myocardial thallium scintigraphy and coronary angiography. The myocardial stunning was caused by injury to the sympathetic nerves from a surgical procedure on the cervical vertebrae.

Rare giant T-wave inversions associated with myocardial stunning: report of 2 cases.

Prominent T-wave inversions are well recognized electrocardiographic signs that can occur in acute myocardial infarction (AMI). However, the giant negative T waves may be associated with myocardial stunning without AMI.This case report describes 2 patients without AMI who developed rare giant T-wave inversions measuring up to 35 mm in depth and QT prolongation after admission to hospital. While 1 patient presented with acute pulmonary edema, the other patient presented with severe chest pain at rest and transient ST elevation. The giant T-wave inversion with QT prolongation may be caused by myo

Kappa-opioid receptor antagonism improves recovery from myocardial stunning in chronically instrumented dogs.

We tested the hypothesis that the selective kappa-opioid receptor antagonist nor-binaltorphimine (nor-BNI) improves recovery from myocardial stunning. Ten dogs were chronically instrumented for measurement of heart rate, left atrial, aortic and left ventricular pressure (LVP), and the maximum rate of LVP increase (LV dP/dt(max)) and decrease (LV dP/dt(max)), coronary blood flow velocity and myocardial wall-thickening fraction. Regional myocardial blood flow was determined with fluorescent microspheres. Catecholamine plasma levels were measured by high-performance liquid chromatography, and bet

Does the pre-ischaemic administration of sevoflurane reduce myocardial stunning? A porcine experimental model.

BACKGROUND: In a porcine model, the cardioprotective effect of sevoflurane was studied with regard to the preservation of myocardial contractility (myocardial stunning) after a myocardial ischaemic insult. METHODS: Twenty-seven pigs were randomized to receive either a dual 4% sevoflurane inhalation period as a supplement to pentobarbital anaesthesia or pentobarbital anaesthesia only before a 15-min ischaemic insult on the left anterior descending coronary artery. The ischaemic period was followed by 180 min of reperfusion. Myocardial contractility was assessed by myocardial sonomicrometry. R

Emergency use of extracorporeal membrane oxygenation in a patient with post-cardiotomy myocardial stunning.

Myocardial stunning refers to reversible postischemic myocardial dysfunction persisting after reperfusion. We report a patient who presented with biventricular failure, but exhibited neither definite electrocardiographic change nor elevation of serum cardiac enzyme levels in the post-cardiotomy period. The presentation was indicative of myocardial stunning. The patient's hemodynamics could not be maintained even with infusion of high doses of catecholamines. Extracorporeal membrane oxygenation successfully restored myocardial function. In conclusion, when the hemodynamic status of a patient wi

A review on fueling recovery: the role of metabolic modulation in non-obstructive myocardial stunning.

Non-obstructive myocardial stunning is characterized by transient myocardial contractile dysfunction despite preserved coronary perfusion. Although increasingly recognized, it remains a diagnostic and therapeutic challenge in cardiology. This review synthesizes current insights into reversible cardiomyopathies, explores the potential of antimetabolic and anti-ischemic agents in their management, and highlights emerging therapeutic avenues. Greater understanding of these mechanisms may improve patient outcomes in this underexplored area.

Effect of intradialytic exercise training on hemodialysis-induced myocardial stunning: a pilot-controlled trial.

BACKGROUND: Hemodialysis (HD) can lead to left ventricular (LV) transient regional wall motion abnormalities (RWMAs), due to segmental hypoperfusion, better known as myocardial stunning. Repeated episodes of HD-induced ischemia contribute directly to the development of heart failure and increased mortality in patients receiving HD. Intradialytic exercise (IDE) training is capable of exerting favorable effects on the cardiovascular system. However, its impact on HD-induced myocardial stunning remains currently unknown. METHODS: In this prospective controlled study, 31 patients participating in

Haemodialysis-induced myocardial stunning in chronic kidney disease - a new aspect of cardiovascular disease.

Chronic haemodialysis (HD) patients are already primed by a large number of structural and functional peripheral vascular and cardiac abnormalities to experience demand myocardial ischaemia. Transient myocardial ischaemia may lead to left ventricular (LV) dysfunction that can persist after the return of normal perfusion. This prolonged dysfunction is known as myocardial stunning. Repetitive episodes of ischaemia can be cumulative and have been shown to lead to prolonged LV dysfunction (in patients with ischaemic heart disease). Conventional HD itself is a sufficient cardiovascular functional s

Differences in myocardial stunning following coronary artery occlusion in conscious dogs, pigs, and baboons.

To determine whether myocardial stunning differs among dogs, pigs, and baboons and is reproducible within species, we examined the effects of 10-min coronary artery (CA) occlusion (CAO) on 9 conscious dogs, 12 minipigs, and 6 baboons. During 10-min CAO, systolic wall thickening in the ischemic zone fell similarly in dogs (-108 +/- 5.6%), pigs (-102 +/- 1.8%), and baboons (-107 +/- 5.7%), but blood flow fell more (P < 0.05) in the subepicardium in pigs (0.07 +/- 0.01 ml.min-1.g-1) and baboons (0.07 +/- 0.02 ml.min-1.g-1) than in dogs (0.18 +/- 0.03 ml.min-1.g-1). Myocardial perfusion and oxygen consumption in reperfused noninfarcted dysfunctional myocardium after unstable angina: direct evidence for myocardial stunning in humans.

OBJECTIVES: To positively establish the diagnosis of myocardial stunning in patients with unstable angina and persistent wall motion abnormalities after reperfusion by coronary angioplasty. BACKGROUND: Although myocardial stunning is thought to occur in several clinical conditions, definite proof of its existence in humans is still lacking, owing to the difficulty of measuring myocardial blood flow (MBF) in absolute terms. METHODS: We studied 14 patients with unstable angina due to proximal left anterior descending coronary artery disease who presented persistent anterior wall motion abnorma

Individualised dialysate temperature improves intradialytic haemodynamics and abrogates haemodialysis-induced myocardial stunning, without compromising tolerability.

BACKGROUND/AIMS: Haemodialysis-induced myocardial stunning is associated with intradialytic hypotension, increased likelihood of cardiovascular events and death. Dialysis at 35°C reduces stunning, but adverse thermal symptoms limit technique adoption. This study investigated whether individualised body temperature dialysis improves haemodynamic stability and abrogates stunning. METHODS: Randomised crossover study of 11 patients compared LV regional wall motion abnormalities (RWMAs) at 37°C (HD(37)) and body temperature ('individualised', HD(ind)). Regional systolic function was quantitatively

Extensive myocardial stunning showing transient regression of prolonged T wave inversion and prolonged sympathetic denervation.

A 69-year-old woman was admitted to the hospital with palpitations. Although left ventriculography showed extensive akinesis except in the basal hyperkinetic segment, coronary angiography showed normal coronary arteries. 123I-metaiodobenzylguanidine (MIBG) accumulation was obviously reduced in the anteroseptal, apical and inferior areas. Inverted T waves developed on day 3 and disappeared on day 104 after transient regression. Echocardiography showed normal left ventricular motion two weeks later. Ergonovine provocation test showed no vasospasm and thallium-201 showed no perfusion defect on da

The Deceptive Presentation of Myocardial Stunning: A Case of Suspected Stroke Preceding In-Hospital Cardiac Arrest.

Myocardial stunning is a reversible form of cardiac dysfunction that can follow cardiac arrest. We report a case where its presentation was masked by a primary neurological complaint. A 67-year-old male with cardiovascular risk factors presented to the emergency department with acute profound confusion and amnesia, initially suspected to be an acute ischemic stroke. Within one hour, his condition deteriorated, and he suffered an in-hospital cardiac arrest due to pulseless ventricular tachycardia, requiring prolonged resuscitation. Post-return of spontaneous circulation, comprehensive neuroimag

Sensitization of reperfused myocardium to subsequent coronary flow reductions. An extension of the concept of myocardial stunning.

The purpose of the present study was to evaluate the response of briefly ischemic and reperfused myocardium to subsequent moderate reductions of coronary arterial flow. In mongrel dogs, a carotid to left anterior descending coronary shunt was constricted to produce moderate coronary flow reductions (50-60% of control) and to thereby reduce regional systolic thickening (measured by echocardiography or sonomicrometry). First, we demonstrated an abnormal response of reperfused myocardium to subsequent flow reductions. We performed two episodes of coronary shunt stenosis, with an intervening 5-min

Better characterization of dipyridamole-induced myocardial stunning by systolic wall thickening. A gated perfusion SPECT study.

AIM AND PATIENTS: The aim of the present study was to assess the additional value of systolic wall thickening to myocardial perfusion in diagnosing myocardial stunning induced by dipyridamole infusion. We selected 52 ischemic patients (43 males; mean age 65.5 ± 7.64), with CAD documented by angiography. Ischemia was defined as a summed difference score ≥ 5. All patients underwent a 2-day gated perfusion SPECT protocol. The patients received a dose of 740 MBq of 99mTc-tetrofosmin after stress and at rest. RESULTS: The post-stress LVEF was significantly lower than rest LVEF (48.3% ± 14.5% vs. 5
