From Nail Biting to Osteomyelitis: A Pediatric Case of Distal Phalanx Infection Secondary to Onychophagia.

Onychophagia, commonly referred to as nail biting, is a body-focused repetitive behavior that occurs most frequently in children and adolescents. Although often perceived as a harmless habit, repetitive trauma to the periungual tissues can compromise the integrity of the cutaneous barrier and increase susceptibility to secondary bacterial infection. Extension of infection into deep soft tissues with associated bone involvement is uncommon, particularly within the pediatric population. We present the case of a 10-year-old female patient who arrived at the emergency department with progressive i

Prevalence, risk factors and potential implications of nail biting in adults with congenital heart disease.

BACKGROUND: Nail biting is a habit with potential health implications. Patients with congenital heart disease (CHD) are at increased risk of infections, with endocarditis being a particularly significant concern. Our aim was to establish the magnitude of this modifiable potential risk factor, namely nail biting, in a contemporary cohort of adult with CHD (ACHD). METHODS: Our prospective study included consecutive, unselected ACHD patients, categorised on nail-biting habits, analysing background demographics, health records, and behavioural factors. RESULTS: Of the 310 patients enrolled, 101

Drug Treatment of Trichotillomania (Hair-Pulling Disorder), Excoriation (Skin-picking) Disorder, and Nail-biting (Onychophagia).

BACKGROUND: Trichotillomania (TTM), excoriation (or skin-picking) disorder and some severe forms of onychophagia are classified under obsessive-compulsive and related disorders. There are different interacting neurotransmitter systems involved in the pathophysiology of impulse-control disorders, implicating noradrenaline, serotonin, dopamine, opioid peptides and glutamate, hence investigators focused on drugs able to act on these transmitters. Our aim was to critically review the efficacy of the drugs employed in impulse-control disorders. METHODS: We searched for controlled drug trials to tr

Purulent Pericarditis Due to Paronychia in a 16-Month-Old Child: A Nail-Biting Story.

Purulent pericarditis is a rare infectious disease with significant mortality, even in the modern antibiotic era. The presenting signs can often be subtle and patients can deteriorate rapidly with cardiac tamponade. We report a previously healthy 16-month-old female who developed purulent pericarditis associated with paronychia and sepsis caused by methicillin-sensitive Staphylococcus aureus. In addition to antibiotic treatment, she required emergent pericardiocentesis for cardiac tamponade, followed by two surgical interventions including full median sternotomy incision and partial pericardie

On the value of nonremovable reminders for behavior modification: an application to nail-biting (onychophagia).

The authors examined the effectiveness of a novel behavior modification method for dysfunctional and impulsive habits, based on nonremovable reminders (NrRs). NrRs were implemented by having participants wear nonremovable wristbands designated to constantly remind them of their resolution to quit the targeted habit (nail-biting). Participants were 80 nail-biters who resolved to quit. The NrR approach was contrasted with an aversion-based behavioral modification technique. Recovery was assessed after 3 and 6 weeks of treatment and in a 5-month follow-up. The NrR method was associated with lower

A comparison of relaxation training and competing response training to eliminate hair pulling and nail biting.

A 17-year-old female with a 14-year history of hair pulling and nail biting was treated with relaxation training and competing response training. While introduction of relaxation training produced an initial decline in hair pulling, there was a gradual recovery of the maladaptive behavior to baseline levels. The competing response procedure consisted of clenching fists for 3 min whenever hair pulling or nail biting occurred or were likely to occur. By the use of the competing response hair pulling and nail biting decreased, eventually to zero. Follow-up over a 2-year period revealed that the t

Nail biting and picking as a possible cause of longitudinal melanonychia. A study of 6 cases.

Examination of individuals affected by onychotillomania led us to describe a new cause of longitudinal melanonychia (LM) of the finger-nails. Damage of the finger-nails was caused both manually and by chewing. The nail cuticles were usually pushed back, and biting caused pressure damage of the base of the nail. It is likely that finger-nails respond similarly to toe-nails and the matrix melanocytes can be stimulated by trauma. Such stimulation can apparently persist long after cessation of the trauma. Histological examination of the nail plates demonstrated that the pigment was melanin. In the

A randomized controlled trial of a novel self-help technique for impulse control disorders: a study on nail-biting.

Nail-biting is currently classified as an impulse control disorder not otherwise specified. Although seldom targeted as a primary symptom, nail-biting is often associated with somatic complications and decreased quality of life. The present study assessed the effectiveness of an innovative self-help technique, titled decoupling (DC). DC aims at attenuating pathological nail-biting by performing motor sequences that decouple and rearrange the behavioral elements involved in the habit. A total of 72 participants with excessive nail-biting were recruited via specialized self-help forums and were

Incompetent lip seal and nail biting as risk factors for malocclusion in Japanese preschool children aged 3-6 years.

BACKGROUND: Malocclusion is a multifactorial condition associated with genetic and environmental factors. The purpose of this study was to investigate the prevalence of occlusal traits, oral habits, and nose and throat conditions by age and to assess the association between malocclusion and its environmental factors in Japanese preschool children. METHODS: A total of 503 Japanese children (258 boys and 245 girls aged 3-6 years) were recruited. Occlusal traits were assessed visually to record sagittal, vertical, and transverse malocclusion, and space discrepancies. Lip seal was recorded by an

Association of nail biting and psychiatric disorders in children and their parents in a psychiatrically referred sample of children.

BACKGROUND: Nail biting (NB) is a very common unwanted behavior. The majority of children are motivated to stop NB and have already tried to stop it, but are generally unsuccessful in doing so. It is a difficult behavior to modify or treat. The objective of this study was to investigate the prevalence of co-morbid psychiatric disorders in a clinical sample of children with NB who present at a child and adolescent mental healthcare outpatient clinic and the prevalence of psychiatric disorders in their parents. METHOD: A consecutive sample of 450 referred children was examined for NB and 63 (14

Risk factors for dental anxiety and cooperativeness in pediatric patients with or without oral habits (bruxism, nail biting, and thumb sucking).

INTRODUCTION: Anxiety and fear can deter patients from visiting dentists and can deteriorate their oral health. In children, this can also affect their cooperativeness. No studies exist on potential links between oral habits and dental fear and children's cooperation. Moreover, predictors of dental anxiety and patient cooperativeness are controversial. Therefore, this study was conducted with the objectives of (1) evaluating associations between oral habits and children's dental fear and cooperativeness as well as (2) potential risk factors of dental anxiety, cooperation, and oral habits. Thumb sucking or nail biting in childhood and adolescence is associated with an increased risk of Crohn's disease: results from a large case-control study.

BACKGROUND: The hygiene hypothesis suggests that a reduction in microbial exposure contributes to an impaired immune response later in life and increases the incidence of immune-mediated diseases such as inflammatory bowel diseases (IBD). Thumb sucking and nail biting are two early habits that modulate the oral microbiota composition and antigen load. OBJECTIVE: We hypothesized a lower risk of Crohn's disease (CD) and ulcerative colitis (UC) in adults with prior thumb sucking and nail biting. METHODS: 918 IBD cases and their 918 siblings without IBD were asked to fill out a survey containing

Nail Regeneration of an Allen III Fingertip Amputation After a Dog Bite Using the Semi-occlusive Dressing Technique: A Case Report and Literature Review.

There is ongoing controversy regarding the best treatment of fingertip amputations, conservative treatment with secondary healing or surgical intervention. Healing by secondary intention has been proven to offer satisfactory recovery and function. More extensive wounds are treated surgically. However, even in Allen III and IV amputations, there is a lack of evidence to support enhanced healing and function of fingertips after surgical treatment compared to conservative management. Regarding fingertip amputations after animal bites, thorough debridement is the preferred treatment due to various

"You Pinch It With the Nail Clipper, and You Kill It": A Reflexive Thematic Analysis of How Maya Families Prevent and Manage Tick Bites in Yucatan, Mexico.

In peasant livelihoods, families are regularly exposed to ectoparasites such as ticks, and thus to the diseases they carry. Little is known about how rural families' knowledge influences health in domestic and community environments. We applied reflexive thematic analysis (RTA) to family preventative and curative strategies for tick exposure and bites in a Maya community in Yucatan, Mexico. Data were collected over six months via participant observation and 26 semi-structured interviews with fifteen women homemakers, five peasant farmers, three traditional healers, and three biomedical profess

Onychophagia as a clinical symptom: A pilot study of physicians and literature review.

Although onychophagia is a medical condition and is associated with poorer health, there are no guidelines for assessment or treatment. The purpose of this study was to investigate the clinical aspects of nail biting from doctors' points of view, to estimate the prevalence of onychophagia among physicians, and to review the literature on and treatment methods for onychophagia. Twenty-four percent of doctors reported nail-biting periods during their lifetimes, and 2% of them remained active nail biters. A total of 64.4% of doctors see nail biting in their practices, and 60.6% never or only on r

Onychophagia and onychotillomania: prevalence, clinical picture and comorbidities.

Onychophagia is defined as chronic nail biting behaviour, which usually starts during childhood. Onychotillomania results from recurrent picking and manicuring of the fingernails and/or toenails, leading to visual shortening and/or estraction of nails. The aim of this study was to assess the prevalence of onychophagia and onychotillomania in young adults, and the comorbidity of these conditions with anxiety disorders and obsessive compulsive disorders (OCD), as well as to determine factors related to these behaviours. A total of 339 individuals were interviewed with a structured questionnaire.
