Axillary Management after Neoadjuvant Endocrine Therapy (NET).

BACKGROUND: In patients with hormone receptor-positive (HR+), HER2-negative breast cancer, there is a lack of data available to define optimal axillary management strategies after neoadjuvant endocrine therapy (NET). PATIENTS AND METHODS: We performed a retrospective review of patients with clinical stage I-III, HR+ breast cancer who received > 90 days of NET at a single comprehensive cancer center between 2004 and 2024. RESULTS: A total of 230 patients were included; 120 (52.2%) were clinically node-negative (cN0), while 110 (47.8%) were clinically node-positive (cN+). In the cN+ group, 7.3

Imaging Neoadjuvant Therapy Response in Breast Cancer.

The use of neoadjuvant systemic therapy in the treatment of breast cancer patients is increasing beyond the scope of locally advanced disease. Imaging provides important information in assessing response to therapy as a complement to conventional tumor measurements via physical examination. The purpose of this article is to discuss the advantages and limitations of current assessment methods, as well as review functional and molecular imaging approaches being investigated as emerging techniques for evaluating neoadjuvant therapy response for patients with primary breast cancer. © RSNA, 2017.

Neoadjuvant therapy for NSCLC: From monotherapy to precision combination therapy.

Neoadjuvant therapy aims to reduce tumor size, increase resectability, eliminate micrometastases, and improve outcomes for patients with advanced non‑small cell lung cancer (NSCLC). Platinum‑based chemotherapy was the standard neoadjuvant treatment strategy in the past. In recent years, the emergence of targeted therapy and immunotherapy has significantly promoted neoadjuvant strategies. Immune checkpoint inhibitors (ICIs) such as programmed cell death 1 (PD-1) inhibitors, programmed cell death ligand 1 (PD-L1) inhibitors, and cytotoxic T lymphocyte associated protein 4 (CTLA-4) inhibitors rep

[Therapy monitoring of neoadjuvant therapy with MRI. RECIST and functional imaging].

CLINICAL/METHODICAL ISSUE: Neoadjuvant chemotherapy is increasingly being applied in patients with operable breast cancer. Thus, an early prediction of response to neoadjuvant chemotherapy is of high relevance. STANDARD RADIOLOGICAL METHODS: The interobserver variability of clinical examination, mammography and ultrasonography in the assessment of response to neoadjuvant chemotherapy is high. METHODICAL INNOVATIONS: Magnetic resonance imaging (MRI) allows the assessment of functional parameters in addition to changes in tumor size and morphology. PERFORMANCE: A reliable therapy response mon

A study about different findings of PET-CT between neoadjuvant and non-neoadjuvant therapy: SUVmax is not a reliable predictor of lymphatic involvement after neoadjuvant therapy for esophageal cancer.

BACKGROUND: No definitive findings or established guidelines have been published for the evaluation of esophageal tumors (tumor) and regional lymph nodes (LN) using positron emission tomography computed tomography (PET-CT) in patients with esophageal cancer. In addition, it remains unclear whether PET-CT findings vary between neoadjuvant (NT) and non-neoadjuvant (non-NT) therapy cases. Therefore, preoperative evaluation using PET-CT might provide unreliable information and influence the management plan for esophageal cancer. The purpose of the present study is to clarify the different findings

De-escalation of Axillary Surgery After Neoadjuvant Therapy.

Surgical de-escalation of the axilla has evolved over the past 28 years since the emergence of sentinel lymph node surgery. Well-documented complications of the once standard of care axillary lymph node dissection (ALND), including lymphedema, led physician scientists towards a progressive push to study and incorporate less invasive techniques in the axilla. Many trials have justified oncologic safety of axillary de-escalation in patients who are spared neoadjuvant treatment. The applicability in the neoadjuvant setting, however, is less clear and axillary surgical approaches in this patient p

Radiation therapy on the axilla after neoadjuvant chemotherapy.

PURPOSE OF REVIEW: The introduction of primary systemic therapy has established a new treatment paradigm for breast cancer patients. However, recommendations for regional node irradiation after neoadjuvant chemotherapy are not supported by level I evidence, yet. RECENT FINDINGS: In addition to strategies optimising systemic treatments and surgery, current discussions focus on tailoring radiation therapy for breast cancer. Especially in view of the increasingly pivotal role of neoadjuvant chemotherapy, gauging the extent of radiation therapy in the breast and nodal volumes. SUMMARY: The curre

Neoadjuvant imatinib therapy in rectal gastrointestinal stromal tumors.

Rectal gastrointestinal stromal tumor (GIST) is a rare entity. Thus, its clinical features have not been well documented, and optimal treatment strategies have not been established. Surgery for rectal GISTs may be difficult because they are often large in size. In addition, rectal GISTs were found to be associated with high rates of local recurrence, regardless of the surgical procedure, before imatinib was introduced in the early 2000s. Since the introduction of imatinib therapy, accumulating evidence suggests that neoadjuvant imatinib therapy may improve the outcomes of rectal GIST treatment

Neoadjuvant therapy in thyroid cancer: an emerging approach.

PURPOSE: Neoadjuvant therapy in thyroid cancer has emerged as a promising strategy for selected patients with locally advanced disease, but its rationale and clinical impact vary substantially across histologies. METHODS: This comment examines the current evidence and clinical rationale for neoadjuvant therapy across anaplastic, differentiated and medullary thyroid carcinomas. RESULTS: For BRAF V600E-mutated anaplastic thyroid carcinoma, targeted therapy has created a meaningful window for surgical resection in patients previously considered unresectable, representing the clearest proof of c

Adrenocortical Carcinoma: Role of Adjuvant and Neoadjuvant Therapy.

Adjuvant and neoadjuvant chemotherapy in the treatment of adrenocortical carcinoma (ACC) is limited by few existing trials, most of which are retrospective. The drug mitotane has been used for the treatment of ACC, although existing guidelines only support its use in high risk of recurrence. The first phase 3 trial involving systemic chemotherapy for ACC supports the use of etoposide, doxorubicin, cisplatin, and mitotane for combination therapy. No significant breakthrough has been discovered thus far in of targeted and immunotherapies. Neoadjuvant chemotherapy is only used to allow for comple

Neoadjuvant therapy for cholangiocarcinoma: A comprehensive literature review.

Biliary tract cancers (BTCs) comprise a heterogenous group of aggressive and rare malignancies arising in the bile duct outside or within the liver. BTCs include cholangiocarcinoma (CCA), gallbladder cancer (GBC) and ampulla of Vater cancer (AVC); according to the "historical" anatomical classification, CCAs are further subdivided into extrahepatic cholangiocarcinomas (eCCAs) - including distal (dCCA) and perihilar (pCCA) - and intrahepatic cholangiocarcinomas (iCCA). Notably enough, these subtypes reflect distinct features in terms of biology, epidemiology, prognosis and therapeutic strategie

Management of the Axilla after Neoadjuvant Systemic Therapy.

As the use of neoadjuvant systemic therapy (NAST) increases, the optimal management of the axilla has become increasingly complex. Consensus among professional organizations is that those patients with clinically negative axillary nodes who are being considered for NAST should undergo a sentinel lymph node (SLN) biopsy following NAST. If a positive SLN is subsequently identified, an axillary lymph node dissection (ALND) is the current standard of care. For patients with clinically node-positive disease, SLN biopsy is a reasonable option for those with a good response to NAST. Patients should u

Surgical treatment of breast carcinoma after neoadjuvant therapy.

INTRODUCTION: Neoadjuvant therapy (NT) applied before breast cancer surgery can lead in favourable cases to regression of the tumor or its total disappearance - pathological complete response (pCR). Due to downstaging after NT, the extent of the surgery can be reduced. pCR represents an important prognostic factor for survival. The aim of this study was to evaluate the effectiveness of NT resulting in pCR on our sample of patients and to assess the frequency of locoregional recurrence (LRR) depending on the extent of the surgery in postoperative care. METHODS: This retrospective study was per

Neoadjuvant Checkpoint Inhibitor Therapy for Merkel Cell Carcinoma.

A 78-year-old man presented with rapidly enlarging lymph nodes in the right preauricular region and neck. Needle biopsy revealed a cytokeratin 20-positive, high-grade neuroendocrine neoplasm consistent with Merkel cell carcinoma (MCC). Cross-sectional imaging disclosed a 5.2-cm intraparotid mass and extensive adenopathy in the ipsilateral cervical and submental chains (Figs 1A and 1C), without distant metastatic disease. A skin examination did not reveal a primary lesion (hence, stage IIIA, T0N1bM0). The patient's history was notable for hypertension, diet-controlled diabetes type II, high cho

Neorectal hyposensitivity after neoadjuvant therapy for rectal cancer.

BACKGROUND AND PURPOSE: Preoperative radiotherapy for rectal cancer has a detrimental effect on long-term anorectal function and quality of life, additional to that observed after rectal resection. The exact physiological mechanisms for the excess impairment remain unknown. We aimed to investigate neorectal and anal sphincter properties in patients treated with neoadjuvant therapy (NT) prior to total mesorectal excision (TME). MATERIAL AND METHODS: Sixteen patients (NT+ patients) were examined by multimodal neorectal stimulation and standard anorectal physiological testing. Data were compared
