The role of the pediatric nurse practitioner in a neonatal unit.

A pediatric nurse practitioner was given responsibility for a nursery. Her major responsibilities included: (1) daily chart and infant rounds, (2) initial physical examinations, (3) rounds with mothers, and (4) daily report to the physician on the condition and problems in her nursery. Over a one-year period of time, the PNP detected 20 serious problems, such as significant jaundice, sepsis, tachypnea, hypoglycemia, and two major congenital anomalies. The accuracy of the physical examination by the PNP was compared with that of the physician. In 72 percent of cases, the assessment of the PNP w

Transient Myeloproliferative Disorder: An Update for Neonatal Nurses.

Down syndrome (DS) is a well-known genetic disorder that affects 700-1,000 infants per year. One particular comorbidity of DS is transient myeloproliferative disorder (TMD), a disease characterized by leukocytosis with elevated blast counts. Approximately 10 percent of DS infants develop TMD, which usually manifests during the first week of life and can lead to an extended hospitalization in a NICU. In addition to hallmark hematologic findings, other manifestations include jaundice, conjugated hyperbilirubinemia, hepatomegaly, and pericardial or pleural effusions. TMD generally resolves sponta

The Role of Neonatal Nurses in the Prevention of Retinopathy of Prematurity.

The main risk factors for preterm babies developing retinopathy of prematurity (ROP) are poorly administered supplemental oxygen, infections, poor weight gain and transfusion of blood products, meaning ROP is sensitive to the quality of neonatal inpatient care provided, especially the nursing care. Nurses are the primary caregivers in neonatal units and play a critical role in preventing ROP related blindness. We discuss the role of the neonatal nurse in prevention of ROP based on a framework of five dimensions of care: specialist knowledge, clinical advocacy, leadership and mentorship, servic

Neonatal apnea: diagnosis by nurse versus computer.

In an effort to characterize significant neonatal apnea and evaluate the nursing diagnosis of apnea, apnea type and frequency were determined in 27 infants by continuous computer recording of heart rate, respiratory impedance, end-tidal CO2, and either or both transcutaneous oxygen and pulse oximetry. Of the 1,266 recorded apneas, 46% were central, 44% were mixed, and 10% were obstructive. Mixed apnea was associated with a longer mean duration and greater mean decrease in heart rate than central apnea. Apnea duration was positively correlated with both a decrease in heart rate and oxygen satur

Analysis of Obstetric Nursing Interventions for Home Monitoring of Neonatal Jaundice.

BACKGROUND: Neonatal jaundice is one of the most common conditions in the early postnatal period, and timely identification is essential to prevent complications such as bilirubin encephalopathy and long-term neurodevelopmental impairment. With early hospital discharge becoming more common, effective home-based monitoring strategies are urgently needed. This study evaluated the effectiveness of a structured obstetric nursing intervention in improving early detection of jaundice, reducing the incidence of severe hyperbilirubinemia and hospital readmission, and enhancing parental confidence and

Advanced practice: a model for neonatal/perinatal and women/child nursing.

Collaboration between service and academia has often been disparate. A new model of cooperation was designed in the Fall of 1992 by faculty at the University of Connecticut School of Nursing and nurse administrators at John Dempsey Hospital of the University of Connecticut Health Center. A combined effort was undertaken to expeditiously prepare, at the graduate level, acute care nurse practitioners in Critical Care and Neonatal/Perinatal Nursing. The impetus for the collaborative effort was due in part to: reduced medical residency hours for patient coverage; limited access by patients to care

Tiny Infants, Positional Head Deformity, Developmental Positioning and Neonatal Nursing Practice.

BACKGROUND: Low-birth weight infants are at risk for neurodevelopmental complications. Prolonged mechanical ventilation and endotracheal tube stability can compromise developmentally supportive positioning and result in positional head deformity (PHD). Nationally, PHD prevalence is 22%-66%, yet an internal audit of 93 neonatal intensive care unit (NICU) discharge summaries found normal head assessments. PURPOSE: To evaluate PHD prevalence among NICU preterm infants and neurodevelopmental positioning practice. METHODS: A convenience sample of 50 NICU preterm infants >72 hours of life discharg

Postpartum and neonatal nursing care during the 2009 H1N1 influenza pandemic.

We describe select influenza infection control policies and practices related to postpartum and newborn care during the 2009 H1N1 pandemic. In an online survey of obstetric and neonatal nurses, significantly more nurses indicated a written hospital policy supporting each of the practices during versus before the pandemic. The two practices least often implemented were temporary separation of healthy newborns from ill mothers (37.7 percent) and testing newborns for influenza virus infection if signs of influenza were observed (31.4 percent). Presence of written hospital policies increased imple

[Nursing care in the use of neonatal continuous positive airway pressure].

Noninvasive neonatal ventilation, and principally the method of Continuous Positive Airway Pressure (CPAP), has become an essential tool in the management of respiratory pathology and a vital support for the newborn. Its use has become a common procedure in the majority of our hospitals. Actually, it is the treatment of choice for neonatal respiratory distress syndrome, as much for full-term newborns as for premature babies. This is an alternative technique to invasive ventilation and one that aims to diminish the occurrence of bronchopulmonary dysplasia and other associated complications. The

Nursing Care of Surgical Neonates in the Neonatal Intensive Care Unit: A Narrative Review.

Surgical neonates represent one of the most vulnerable populations in the NICU due to physiologic immaturity, limited compensatory reserves, and exposure to perioperative stress. Although advances in neonatal surgery, anesthesia, and intensive care have improved survival, outcomes remain closely linked to the quality and consistency of nursing care. Nurses play a critical role in physiologic stabilization, prevention of complications, early recognition of deterioration, and delivery of family-centered care throughout the perioperative course. This narrative review synthesizes current evidence
