Germ cell tumors (GCTs) classically occur in gonads. However, they are the most frequent neoplasms in the pineal region. The pineal location of GCTs may be caused by the neoplastic transformation of a primordial germ cell that has mismigrated. The World Health Organization (WHO) recognizes 5 histological types of intracranial GCTs: germinoma and non-germinomatous tumors including embryonal carcinoma, yolk sac tumor, choriocarcinoma and mature or immature teratoma. Germinomas and teratomas are frequently encountered as pure tumors whereas the other types are mostly part of mixed GCTs. In this s

During the last 11 years, 144 nonlymphoreticular neoplasms were diagnosed in exfoliative cytology specimens obtained from patients younger than 17 years of age. Neuroblastoma was the single most common neoplasm (30 cases). Other categories of malignant neoplasms were primary bone tumors (30 cases), soft-tissue sarcomas (25 cases), brain tumors (25 cases) and epithelial neoplasms (7 cases). Of the 780 cytologic specimens, 335 were positive for malignant cells. Serous effusions provided most of the positive specimens from patients with neuroblastoma, germ-cell tumors and bone sarcomas. Germ cell tumors (GCT) are neoplasms that originate predominately in the ovary and testis. Tumors of germ cell origin only very uncommonly arise in extragonadal sites. We have diagnosed ten primary malignant extragonadal GCT arising in the mediastinum, retroperitoneum, liver, and sacrococcygeal region by fine-needle aspiration biopsy (FNAB). Patient ages ranged from 1 to 54 years; the majority were males. Our series included three seminomas, three yolk sac tumors (YST), one choriocarcinoma, one embryonal carcinoma, and two mixed, poorly differentiated GCT. In aspirates, seminomatous elements a

Intracranial germ cell tumors (iGCTs) are rare neoplasms that predominantly affect children and adolescents, accounting for up to 20% of pediatric germ cell tumors and showing higher incidence in East Asian populations. These tumors commonly arise in the pineal and suprasellar regions, with a subset presenting as bifocal lesions. iGCTs are broadly classified into germinomas and non-germinomatous germ cell tumors (NGGCTs), with germinomas representing two-thirds of cases. Tumor classification reflects their developmental origin from primordial germ cells and is confirmed through histopathology

Most tumors are sporadic and originated from somatic mutations. Some rare germline mutations cause familial tumors, often involving multiple tissues or organs. Tumors from somatic mosaicism during embryonic development are extremely rare. We describe here a pediatric patient who developed both an ovarian germ cell tumor and systemic mastocytosis. Targeted DNA next-generation sequencing analysis revealed similar genomic changes including the same KIT D816V mutation in both tissues, suggesting a common progenitor cancer cell. The KIT mutated cells are likely from early embryonic development duri

Mediastinal germ cell tumors are among the most common tumors in the anterior mediastinum. This tumor should be considered strongly in the differential diagnosis of anterior mediastinal masses. Benign teratomas are common in this site, and complete surgical resection results in cure of nearly all patients. Malignant mediastinal seminomas are highly curable and either radiotherapy (mediastinal disease) or cisplatin-based chemotherapy (metastatic disease) will result in long-term survival in 80% or more of patients. Mediastinal nonseminomatous germ cell malignancies represent a clinically and bi

Teratomas of the ovary and testis are confusing because, despite histologic similarities, they exhibit different biologic behaviors, depending mostly on the site of occurrence and the age of the patient. Thus, most ovarian teratomas are benign, and most testicular teratomas are malignant, with the exception of those occurring in children. These general statements, however, do not hold true for ovarian teratomas that are "immature" or exhibit "malignant transformation" and for dermoid and epidermoid cysts of the testis, categories of ovarian and testicular teratomas that are malignant and benig

BACKGROUND: Brain tumors in infants represent a rare but clinically challenging subset of pediatric central nervous system (CNS) neoplasms. These tumors are biologically and clinically distinct from those in older children and adults, with a predilection for supratentorial locations, frequent presentation with macrocephaly and signs of increased intracranial pressure, and unique molecular profiles. There have been many advances in molecular profiling, now influencing treatment choice and outcomes. Non-embryonal tumor types-including glial tumors, choroid plexus tumors, craniopharyngioma, epend

Most pineal region masses are malignant germ cell neoplasms that occur in young male patients. The most common is a germinoma, which is a homogeneous mass with signal intensity and attenuation similar to those of gray matter; the mass engulfs a densely calcified pineal gland. Teratomas are multilocular heterogeneous masses containing lipid areas. Other types of pineal region masses include choriocarcinoma, endodermal sinus tumor, and embryonal carcinoma. Pineal parenchymal neoplasms are usually either pineocytomas or pineoblastomas, which may "explode" preexisting pineal calcifications. Unlike

Extracranial germ cell tumors (EGCTs) are rare pediatric neoplasms characterized by significant histological heterogeneity and variability in clinical behavior. This highlights the necessity for precise preoperative differential diagnosis. While computed tomography (CT) provides essential imaging information for differentiation, conventional visual assessments are often limited due to overlapping radiological features. Although artificial intelligence (AI) has demonstrated potential in enhancing diagnostic accuracy in medical imaging, its application to EGCTs has been constrained by the scarci

Ovarian malignant germ cell tumors (OMGCTs) are heterogeneous tumors that are derived from the primitive germ cells of the embryonic gonad. OMGCTs are rare, accounting for about 2.6% of all ovarian malignancies, and typically manifest in adolescence, usually with abdominal pain, a palpable mass, and elevated serum tumor marker levels, which may serve as an adjunct in the initial diagnosis, monitoring during therapy, and posttreatment surveillance. Dysgerminoma, the most common malignant germ cell tumor, usually manifests as a solid mass. Immature teratomas manifest as a solid mass with scatter

DICER1 tumor predisposition syndrome is a rare autosomal-dominant disorder resulting from heterozygous germline pathogenic variants in DICER1, a critical regulator of microRNA biogenesis and RNA interference. The syndrome predisposes individuals to a broad spectrum of benign and malignant neoplasms, primarily affecting pediatric and young adult populations. Gynecologic manifestations are notably heterogeneous and include the well-established associations with embryonal rhabdomyosarcoma and Sertoli-Leydig cell tumor. DICER1 alterations have also been identified in gynandroblastoma, a subset of

Primary central nervous system (CNS) germ cell tumors (GCTs) are a rare heterogeneous group of lesions, which the clinicopathological features have a marked degree of heterogeneity comparing with that of gonadal GCTs. Accurately diagnosing CNS GCTs might be extremely difficult and requires immunohistochemical verification. This study was to investigate the biological feature of CNS GCTs and diagnostic value of immunohistochemical markers OCT3/4, C-kit, PLAP, and CD30 in CNS GCTs. A retrospective study was performed on 34 patients with CNS germ cell tumors between 1990 and 2014. 34 CNS GCTs acc

Immunohistochemical evaluation of CD30 expression is commonly performed in the assessment of hematopoietic disorders and germ cell tumors. A new monoclonal antibody, CON6D/B5, directed against the CD30 antigen has become commercially available for use in formalin-fixed paraffin-embedded tissue samples. The performance characteristics of CON6D/B5 were tested and compared with the well-characterized monoclonal CD30 antibody Ber-H2. Among the various neoplasms analyzed, there was complete concordance between the results of CON6D/B5 and Ber-H2 staining. Similar to Ber-H2, CON6D/B5 immunoreactivity

DICER1 syndrome is a tumor predisposition syndrome in which patients are at an increased risk of developing a wide variety of benign and malignant neoplasms with a hallmark constellation of pediatric pleuropulmonary blastoma, cystic nephroma, and thyroid lesions. DICER1 encodes an RNA endoribonuclease that is crucial to the processing of microRNA and may play a role in the maturation of Müllerian tissue. Within the gynecologic tract, germline mutations in DICER1 are associated with an array of rare tumors, including Sertoli-Leydig cell tumor, embryonal rhabdomyosarcoma of the cervix, gynandrob

PURPOSE: Pediatric germ cell tumors are rare, representing about 3% of childhood malignancies in children less than 15 years of age, presenting in neonates or adolescents with a greater incidence noted in older adolescents. Aberrations in primordial germ cell proliferation/differentiation can lead to a variety of neoplasms, including teratomas, embryonal carcinoma, choriocarcinoma, and yolk sac tumors.
PATIENTS AND METHODS: Three Finnish families with varying familial germ cell tumors were identified, and whole-genome sequencing was performed using an Illumina sequencing platform. In total, 2

Cystic trophoblastic tumor (CTT) is a rare non-aggressive germinative neoplasm from the group of non-choriocarcinomatous trophoblastic tumors, which is presented by cystic spaces lined with mononuclear degenerative-looking trophoblastic cells. CTT has been most often described as a residual disease in dissected retroperitoneal lymph nodes of patients with metastatic germ cell testicular tumours after chemotherapy. There were published only sporadic cases of primary testicular mixed germ cell tumour with CTT component. Hereby, the authors present a case of a 22-year-old man with a mixed germ ce

BACKGROUND: Testicular tumors are rare, particularly in aged men. Indeed, germ cell tumors occur mostly in the young population. Importantly, few studies have systematically characterized testicular tumors, other than lymphomas and spermatocytic tumor, diagnosed in the elderly. We herein aim to assess the clinicopathologic features of such tumors, particularly germ cell tumors, and their oncologic outcomes in old men.
METHODS: We analyzed 46 consecutive patients undergoing orchiectomy for testicular tumors at the age of ≥60 years at the University of Rochester Medical Center, together with 34

BACKGROUND: Brain tumors in patients with Down syndrome (DS) rarely are reported, and their behavior is not well known.
METHODS: The authors report on a male patient age 19 years who had DS with diffuse astrocytoma (World Health Organization Grade 2) that recurred twice despite treatment, leading to a glioblastoma and, finally, to death in just over 2 years. The literature on brain tumors in patients with DS is reviewed.
RESULTS: Although brain neoplasms were suspected to be in excess in patients with DS, the authors found only 36 patients with brain neoplasms and 2 spinal tumors. An unusual

BACKGROUND: Testicular cancer constitutes 1.0% of male cancer and typically carries a good prognosis. As far as we are aware, the role for hydrogen sulfide in testicular cancer and the level of hydrogen sulfide-synthesizing enzyme have never been addressed. Here we examined cystathionine gamma-lyase (CSE) expression in several germ-cell testicular tumors.
MATERIALS AND METHODS: Tissue microarrays were employed to examine CSE expression in 32 benign testicular samples, 88 testicular seminomas, 34 embryonal carcinomas, 4 mature teratomas, and 16 yolk sac tumors, and CSE expression was compared
