[Percutaneous nephrostomy in neoplasm patients: when?].

The management of cancer patients with a lesion in its end stage is a clinical problem a satisfactory solution to which has not been found yet, because of the ethical and legal problems involved. The authors analyzed the survival rates of 218 patients with advanced neoplastic disease and obstructive renal failure who had undergone external/internal urinary diversion. One month after the procedure 20% of the patients were dead. The figure rose to 75% at 6 months and reached 98% at one year. Urinary diversion in cancer patients with renal failure is aimed not only at avoiding death and assuring

[Retrograde percutaneous nephrostomy: a case report].

We performed retrograde percutaneous nephrostomy on a 79-year-old woman admitted to our hospital with recurrent right acute pyelonephritis. Computed tomographic scan showed mild hydronephrosis and some renal stones in the right kidney. Antegrade percutaneous nephrostomy was thought to be impossible because the patient could not be placed in a prone position. Urgent drainage by ureteral catheter placement in the right renal pelvis was performed in the lithotomy position. After pyelonephritis improved, retrograde percutaneous nephrostomy was performed, which provided precise percutaneous access

Percutaneous nephrostomy in gynecologic oncology patients.

Percutaneous nephrostomies were used in 34 patients with ureteral obstruction caused by gynecologic diseases. Group 1 consisted of 12 patients with untreated cervical carcinoma; group 2 consisted of six patients with recurrent cervical carcinoma; group 3 consisted of seven patients with complications of urinary conduits, and group 4 consisted of eight patients with a variety of malignant and benign gynecologic diseases causing ureteral obstruction. One perinephric hematoma and one perinephric abscess (4%) occurred during primary percutaneous nephrostomy diversion of 53 renal units. One (3%) pe

Tamponade nephrostomy catheter for percutaneous nephrostolithotomy.

Hemorrhage from the nephrostomy tract is the most common major complication associated with percutaneous nephrostolithotomy. The tamponade nephrostomy catheter is a new addition to the endourologist's instruments; it is expressly designed to achieve immediate tamponade of the nephrostomy tract. The large-diameter, occlusive balloon (36F) is carried on a 14-F nephrostomy tube which is passed over a 5-F ureteral stent. As such, the catheter not only tamponades the nephrostomy tract but effectively drains the renal pelvis, while maintaining ureteral access.

Percutaneous nephrostomy: experience in 107 kidneys.

We were successful using percutaneous nephrostomy to relieve upper urinary obstruction in 98 of 107 kidneys. In two thirds of our patients obstruction was due to neoplasm, with prostatic cancer the leading cause. Percutaneous nephrostomy proved to be ideally suited for drainage of pyonephrosis and in azotemic patients. Its use can obviate surgical nephrostomy and prolonged ureteral catheter drainage. We also are encouraged by our early experience with extensions of percutaneous nephrostomy, including percutaneous ureteral stenting, renal stone dissolution, and percutaneous pyelolithotomy.

Outcome of percutaneous nephrostomy for the management of pyonephrosis.

OBJECTIVE: The aim of this study was to evaluate the efficacy of percutaneous nephrostomy (PCN) drainage for the interim management of pyonephrosis. METHODS: Ninety-two consecutive patients [29 men, 63 women; mean age, 57 years; range, 23 to 88] who underwent PCN for the treatment of pyonephrosis from 1996 to 1999 were evaluated retrospectively. The clinical presentation, bacteriology and patient outcomes were analyzed. RESULTS: The majority [77%] of patients had underlying obstructing urinary calculi. Other causes of obstruction included strictures [9%], papillary necrosis [7%], pelvi-urete

Emergency percutaneous nephrostomy in the septic kidney.

From 250 upper tract obstructive uropathy cases we have studied 64 patients hospitalized with toxico-septic shock. The constant symptom was arterial hypotension. Other 3 patients with long-standing urinary infection due to lithiasis developed this dreaded complication after PNL (staghorn stones-2, pyelic stone-1). In complicated obstructive uropathy cases associated with toxico-septic shock, percutaneous nephrostomy for high urinary derivation in emergency is usually made under local anaesthesia. Its aim is rapid and efficient clearance of kidney obstruction, with minimal damage for the patien

The role of percutaneous nephrostomy in gynecologic oncology.

Percutaneous nephrostomy can provide rapid relief of renal failure due to ureteral obstruction by contiguous spread of cervical malignancy. A series of 26 percutaneous nephrostomies placed in 14 patients with cervical cancer, using only local anesthesia and ultrasound or fluoroscopic guidance is presented. Twelve patients experienced no complications, one developed pyelonephritis which cleared rapidly with antibiotics, and one suffered a hematoma managed by surgical nephrostomy. Three of six previously untreated patients and one of eight patients with recurrent disease survived over a year. A

Percutaneous nephrostomy. Aspects on applications and technique.

Percutaneous nephrostomy was performed in 32 patients: in 8 patients for permanent and in 24 for temporary drainage. The most common indication was obstruction of the distal ureter due to malignancy. Puncture of the renal pelvis was performed with the aid of fluoroscopy and/or ultrasound. Change to larger catheters was achieved using the Seldinger technique. Complications in the form of bleeding occurred in 5 patients and were in conjunction with repuncture after the initial catheter had slipped out, or occured in connection with change to larger catheters. With the routine use of ultrasound a

Management of the retained percutaneous nephrostomy catheter.

Percutaneous nephrolithotomy is a widely accepted treatment for urinary calculi, but it is not without complications. We present the case of a 76-year-old male with a retained council tip catheter after percutaneous nephrolithotomy. Fluoroscopic guidance was used to perform percutaneous puncture of the catheter balloon, and the catheter was removed without complication. Advantages of various nephrostomy tube designs and additional measures to prevent this type of complication are discussed.

A 'split-away technique' for percutaneous nephrostomy.

The currently used techniques for nephrostomy sometimes fail to advance the drainage catheter through the nephrostomy track into the renal pelvis. To solve this problem a new technique has been developed. The improvement with this technique is the dilatation of the track with polypropylene dilators and a split-away sheath. The split-away sheath is threaded on the largest dilator. When the dilator is removed the drainage catheter is coaxially passed through the split-away sheath into the renal pelvis. The sheath is then removed. The technique has been used successfully in 32 patients without co

[Percutaneous nephrostomy--the method and technical problems].

Ultrasound guided percutaneous nephrostomy (PNS) was performed on 72 patients (80 kidneys) including pretreatment for percutaneous nephro-uretero lithotomy (PNL). PNS was performed for post-renal anuria or hydronephrosis in 23 cases (28 kidneys), for urinary leakage in 4 cases (5 kidneys), for vesical bleeding in 1 case (2 kidneys) and as pretreatment of PNL in 44 cases (45 kidneys). Ultrasound guided renal puncture was done percutaneously and a 0.038 inch J-tipped wire guide was inserted into the suitable calyx. Then the nephrostomy tract was dilated with fascia dilators of Malecot nephrostom

Percutaneous nephrostomy in neonates, infants, and children.

In a 20-month period, 13 percutaneous nephrostomies were placed in 10 pediatric patients ranging in age from one day to six and one-half years. The procedure was successful in all cases with no significant complications. Final diagnoses included ureteropelvic junction obstruction, ureterovesical junction obstruction, posterior urethral valves, and pyonephrosis. Localization of the obstruction as detected on initial antegrade nephrostography was found to be incomplete or incorrect in three patients. In one patient, both ureteropelvic and ureterovesical junction obstruction were present. In the

Percutaneous nephrostomy: a series and review of the literature.

A series of 53 percutaneous nephrostomies and a literature review of 516 such procedures are presented. The selection of patients with supravesical urinary obstruction, urinary fistulas, and renal calculi is discussed, and the concept is advanced that percutaneous nephrostomy is more than a temporizing preoperative procedure. The available techniques are compared; modified instrumentation for balloon catheter insertion, a combination of fluoroscopic with real-time ultrasound guidance, and some additional principles of subsequent management are introduced. Successful results can be expected in
