A thirty-nine-year-old paraplegic with an ileal conduit urinary diversion and long ureteral strictures bilaterally required urinary tract reconstruction to establish effective internal drainage. Reported techniques proved infeasible. Therefore, the right kidney with its small intrarenal pelvis was opened by anatrophic nephrotomy and anastomosed to a segment of transverse colon in two layers. Between 1971 and 1981, 120 patients or 127 kidneys underwent surgery for staghorn calculi in the Department of Urology at the University of Berne. We report a 71-year-old woman who presented with unilateral flank pain and sepsis. A computed tomographic (CT) scan demonstrated left-sided hydronephrosis. Subsequent percutaneous nephrotomy drainage showed pus-like material, confirming the diagnosis of pyonephrosis. Xanthogranulomatous pyelonephritis is a rare variant of pyelonephritis that often requires radical surgery. We report a case of a 51-year-old female patient who was managed with prolonged placement of nephrostomy tubes prior to surgery. This case illustrates the importance of surgical optimization of a poor operative candidate and the use of nephrostomy tubes as a temporizing management strategy.

BACKGROUND: Robotic-assisted surgeries have gradually become the standard of care for many procedures, especially in the field of urology. Despite the widespread use of robotic assistance in surgeries, data on its postoperative complications are extremely limited. We detail a rare presentation of fulminant Clostridium difficile colitis requiring surgical intervention in a patient with a solitary ectopic pelvic kidney who underwent a robotic-assisted pyelolithotomy. Highlights of the most recent management recommendations for C. difficile infection are also presented.
