Are nerve conduction studies altered in functional neurological disorders?

BACKGROUND: Functional neurological disorders represent conditions without a readily identifiable origin or laboratory-supported diagnostic. We report a case of functional neurological disorder, presenting with muscle weakness with alterations in F-waves on the affected side. CASE REPORT: A retrospective case review of a patient seen in clinic. Electrophysiological evaluation included nerve conduction studies, including recording of F-waves in lower limbs, and needle EMG. A patchy sensory loss and unilateral muscle weakness of the left lower limb persisted nine days after a 40-year-old female

Radial motor nerve conduction studies in the upper arm.

INTRODUCTION: The main goal of this study was to determine the contribution of the anterior forearm muscles to the compound muscle action potential (CMAP) recorded from the extensor digitorum (ED) after proximal stimulation. METHODS: Twenty-one healthy volunteers and 114 patients with compressive and traumatic radial neuropathies were examined. Stimulation was carried out at six different points: distal third of the upper arm; Erb's point; axilla; medial upper arm; antecubital fossa; and ulnar groove. RESULTS: In the control group, Erb's CMAP area was significantly greater than the distal CM

Tibial and peroneal nerve conduction studies in ankle sprain.

BACKGROUND: Lower limb injuries and ankle sprain occur frequently among football players, due to the high incidence of physical contact. As a result, lower limb nerves are subject to injury. This project aims to evaluate the lower limb nerve conduction among students playing football to investigate the probable relationship between ankle sprain in football and nerve conductivity. MATERIALS AND METHODS: Fifty volunteer students aged between 19 and 25 were studied. They fell into three groups; one comprising of 20 healthy football players whose football experience exceeded three years, the seco

Relation of wrist angles to median nerve conduction studies.

OBJECTIVES: To measure the changes in median nerve conduction parameters at various angles of wrist flexion and extension. DESIGN: We studied 8 patients with carpal tunnel syndrome (CTS) and 4 subjects (5 hands) without the condition (controls). Conduction parameters were measured with the wrist in neutral position and at different wrist angles. RESULTS: Onset motor latency increased in both groups with wrist extension and decreased with wrist flexion. Sensory onset latency decreased in both groups with wrist flexion. Compound Muscle Action Potential (CMAP) amplitude increased with wrist fle

Ulnar palmar cutaneous nerve and hypothenar sensory conduction studies.

The ulnar palmar cutaneous nerve (UPCN) is potentially useful to the electrodiagnostician. However, no definitive nerve conduction study techniques for the UPCN have been reported. The UPCN supplies sensory innervation to the hypothenar palm. This study describes an orthodromic sensory conduction technique for the hypothenar palm. Data were collected from 20 normal limbed patients (ages 22-58). Potentials were recorded over the ulnar nerve 10cm proximally and at the elbow. Distal latency was 2.19 +/- 0.17msec, and distal amplitude was 12.7 +/- 6.9uv. Additionally, a modified collision techniqu

Nerve Conduction Studies in Surgical Cubital Tunnel Syndrome Patients.

Background: Electrodiagnostic studies (EDX) serve a prominent role in the diagnostic workup of cubital tunnel syndrome (CBTS), but their reported sensitivity varies widely. The goals of our study were to determine the sensitivity of EDX in a cohort of patients who responded well to surgical cubital tunnel release (CBTR), and whether the implementation of the Association of Neuromuscular and Electrodiagnostic Medicine (AANEM) criteria improves the sensitivity. Methods: We identified 118 elbows with clinical CBTS who had preoperative EDX and underwent CBTR. The EDX diagnoses were CBTS, ulnar neu

Sensory nerve conduction studies of the superficial peroneal nerve in L5 radiculopathy.

BACKGROUND: Theoretically, sensory nerve action potential (SNAP) of the superficial peroneal nerve (SPN) should be preserved when L5 roots are damaged. Previous study indicated that SNAP of SPN was lost or reduced in amplitude in patiens with L5 radiculopathy. To address this issue, this study compared results of SPN sensory conduction studies between healthy subjects and patients with L5 radiculopathy. METHODS: Ninety-four healthy subjects were enrolled and assigned to two age groups: group I (< or = 60 years, n=61) and group II (> 60 years, n=33). Forty-one patients with unilateral L5 radic

Hypoglossal Nerve Conduction Studies and Repetitive Nerve Stimulation: Technical Aspects and Normative Data.

INTRODUCTION/AIMS: Traditional methods for assessing the neurophysiological function of the hypoglossal nerve involve placing electrodes directly on the tongue, but these are limited by technical challenges. We propose an alternative method for hypoglossal nerve conduction studies. METHODS: This study included 34 healthy adults, in whom compound muscle action potentials (CMAP) were recorded using an alternative electrode placement method, with the superficial active electrode positioned on the submental region and the reference electrode placed contralaterally. Repetitive nerve stimulation (R

Double peak sensory nerve action potentials to single stimuli in nerve conduction studies.

INTRODUCTION: In humans, sensory nerve action potentials (SNAPs) can show 2 separate deflections, i.e., double peak potentials (DPp), which necessarily means that 1 peak is delayed with respect to the other. DPps may have various origins and be due to either physical or physiological properties. METHODS: We review the nature of commonly encountered DPps in clinical practice, provide the most likely interpretations for their physiological origin, and assess their reproducibility and clinical utility. RESULTS: We classified the DPps into 3 categories: (1) simultaneous anodal and cathodal stimu

Segmental near nerve sensory conduction studies of the medical and lateral plantar nerve.

Tarsal Tunnel Syndrome (TTS) can be difficult to diagnose: electrophysiologic corroboration is important and has therapeutic implications. Conventional electrodiagnostic techniques are insensitive: motor latency abnormalities exist in only 52%; sensory responses are frequently absent (a nonlocalizing finding). Additionally, previously described near nerve techniques do not isolate conduction velocity (CV) measurement to the short segment across the flexor retinaculum (FR), which would theoretically improve sensitivity. We describe a technique which allows for the determination of segmental sen

Nerve conduction studies in experimental non-freezing cold injury: I. Local nerve cooling.

In rabbits, the tibial nerve was exposed in the lower thigh under general anesthesia and cooled in a metal trough at 1 to 2 degrees C or 5 degrees C for 2, 3, or 4 hours. Nerve conduction studies showed local failure of conduction at the site of cooling which persisted after rewarming, and which was followed by distal degeneration of affected fibers. No persistent conduction block was seen. Changes in maximal velocity indicated that the fastest-conducting motor and afferent axons had been preferentially affected. Histological findings in nerves examined at different intervals after cooling con

Carpal tunnel decompression in patients with normal nerve conduction studies.

Some patients present with typical clinical features of carpal tunnel syndrome despite normal nerve conduction studies. This study compared the preoperative and 1-year postoperative QuickDASH scores in patients with normal and abnormal nerve conduction studies, who underwent carpal tunnel decompression. Of the 637 patients included in the study, 19 had clinical features of carpal tunnel syndrome but normal nerve conduction studies, and underwent decompression after failure of conservative management. Preoperative QuickDASH scores were comparable in both groups (58 vs 54.8). However, there were

Sonography in carpal tunnel syndrome with normal nerve conduction studies.

INTRODUCTION: We assessed the yield of high-resolution ultrasonography (HRUS) in patients with clinically definite carpal tunnel syndrome (CTS) and normal nerve conduction studies (NCS). METHODS: This blinded, prospective, cross-sectional study involved 35 patients (60 hands) with clinically definite CTS and normal NCS, and 20 controls (40 hands). Cross-sectional area (CSAs) of the median nerve at the level of the pisiform bone and flexor retinaculum thickness (FRT) were measured. RESULTS: CSA was abnormal in 48.6% of patients (confidence interval 32.0-65.2%, P = 0.95). FRT was increased in
