[Usefulness of neuroendoscopy and a neuronavigator for removal of clival chordoma].

We report a case of large clival chordoma. The patient was a 56-year-old male who was admitted to our hospital with left eye ptosis and diplopia of 2 months duration. On admission, neurological examinations revealed oculomotor nerve palsy of the left eye. Skull radiographs with polytomographs demonstrated marked destruction of the clivus. A plain computed tomography (CT) scan revealed a large iso-attenuated mass in the clivus, extending anteriorly into the sphenoidal sinus, superiorly into the suprasellar cistern, bilaterally into the petrous apex, posteriorly into the prepontine cistern and c

[Identification of the central sulcus using magnetoencephalography and neuronavigator].

The brain-generated currents that produce potentials measured by the electroencephalogram also produce magnetic fields which can be measured by the magnetoencephalogram (MEG), N 20 compatible evoked field after median nerve stimulation is known to be generated in primary sensory cortex. Using MEG with 37 channel SQUIDs, a current dipole is back traced which corresponds to the sensory cortex. When the dipole is projected onto the MRI of the same patient, the primary sensory cortex is precisely identified in the MRI images. These data were used as the key images for navigator enabling a surgeon

Real-Time Tractography-Assisted Neuronavigation for Transcranial Magnetic Stimulation.

State-of-the-art navigated transcranial magnetic stimulation (nTMS) systems can display the TMS coil position relative to the structural magnetic resonance image (MRI) of the subject's brain and calculate the induced electric field. However, the local effect of TMS propagates via the white-matter network to different areas of the brain, and currently there is no commercial or research neuronavigation system that can highlight in real time the brain's structural connections during TMS. This lack of real-time visualization may overlook critical inter-individual differences in brain connectivity

Endoscopic Management of Quadrigeminal Arachnoid Cyst with Neuronavigation Guidance.

Quadrigeminal cistern arachnoid cysts (ACs) are usually asymptomatic, may be accidental findings during radiological evaluation, and are rare (5%-10% of all intracranial ACs). We report a case of type I quadrigeminal cistern AC managed via navigation-guided cystoventriculostomy followed by endoscopic third ventriculostomy (Video 1). A 0° rod-lens endoscope was used. Different types of cysts may require different endoscopic approaches, and our procedure was facilitated by the presence of significant ventriculomegaly.1 The endoscopic procedure was completed uneventfully; navigation was used to l

Frameless image-guided neuronavigation in orbital surgery: practical applications.

Frameless neuronavigation has been increasingly used in intracranial and spinal neurosurgery. However, the application of neuronavigation in orbital surgery has not yet been reported. The purpose of this study was to evaluate whether frameless navigation in the orbit provides sufficient clinical application accuracy and thus a useful tool in orbital surgery. A frameless infrared-based neuronavigation system (VectorVision, BrainLAB, Heimstetten, Germany) was used in the microsurgical removal of 11 orbital tumors (five cavernomas, three lymphomas, two dermoid tumors, and one rhabdomyosarcoma). Neuronavigation-guided endoscopic surgery for pineal tumors with hydrocephalus.

We have applied the neuronavigation system to endoscopic biopsy and third ventriculostomy in the management of patients with a pineal tumor with hydrocephalus. With the guidance of neuronavigation, the two optimal sites of burr hole and trajectories were planned preoperatively, and the advancing endoscopic device was monitored in real time during the procedure. In our five patients, the diameters of the tumors were 2-3 cm, and the mean systemic accuracy of registration with neuronavigation was 1.2 mm. The biopsy and third ventriculostomy were performed successfully via the respective optimal b

Optical Neuronavigation without Rigid Head Fixation During Awake Surgery.

OBJECTIVE: Optical neuronavigation without rigid pin fixation of the head may lead to inaccurate results because of the patient's movements during awake surgery. In this study, we report our results using a skull-mounted reference array for optical tracking in patients undergoing awake craniotomy for eloquent gliomas. METHODS: Between March 2013 and December 2014, 18 consecutive patients (10 men, 8 women) with frontotemporal (n = 16) or frontoparietal (perirolandic; n = 2) lesions underwent awake craniotomy without rigid pin fixation. All patients had a skull-mounted reference array for optic

Neuronavigation using three-dimensional proton magnetic resonance spectroscopy data.

BACKGROUND: Applications in clinical medicine can benefit from fusion of spectroscopy data with anatomical imagery. For example, new 3-dimensional (3D) spectroscopy techniques allow for improved correlation of metabolite profiles with specific regions of interest in anatomical tumor images, which can be useful in characterizing and treating heterogeneous tumors that appear structurally homogeneous. OBJECTIVES: We sought to develop a clinical workflow and uniquely capable custom software tool to integrate advanced 3-tesla 3D proton magnetic resonance spectroscopic imaging ((1)H-MRSI) into indu

Neuronavigation guided surgery for parenchymal neurocysticercosis in two patients.

Neurocysticercosis is a rare disease in the Baltic area while it is common in the endemic regions. Two patients with serologically negative parenchymal neurocysticercosis from our neurosurgical department who underwent extirpation of the cystic lesions with neuronavigation guided surgery are reported in this paper. Though most publications propose medical treatment with albendazole and praziquantel for parenchymal neurocysticercosis, surgery can be an option for diagnosis and treatment in conjunction with cysticidal medication if the diagnosis is unclear particularly in non-endemic areas.

Neuronavigation in the minimally invasive presacral approach for lumbosacral fusion.

INTRODUCTION: Intraoperative 3D navigation (3D NAV) is gaining importance in spinal surgery, especially with the advancement of minimally invasive techniques in this field. We hypothesized that 3D NAV may be of benefit in the recently described minimally invasive presacral approach for L4-S1 fusion (AxiaLIF). METHODS: Five patients with refractory low back pain and L5-S1 degenerative disease underwent 3D NAV-assisted placement of a presacral L5-S1 screw. Additionally, a patient recently underwent a two-level AxiaLIF procedure (L4-S1) which was also performed with 3D NAV. Navigation was perfor

Neuronavigational neuroendoscopy--to be or not to be? An international pilot questionnaire-based study.

OBJECTIVE: The almost age-old neuroendoscopy (NE) and neuronavigation (NN) in its twenties independently and indisputably have proved their high value as neurosurgical armamentarium and became even indispensable in some pathologies. However, nowadays the effectiveness of their simultaneous and combined application still is a matter of debate. The purpose of our pilot international, questionnaire-based survey was to assess the position of the opinion leaders in the field of neuroendoscopy worldwide toward the neuronavigational neuroendoscopy (NNNE). METHODS: Within 3 months, a questionnaire wi
