Methods and Principles of the Intraoperative Neurophysiologic Monitoring in Neurosurgery.

Intraoperative neurophysiologic monitoring (IONM) is an innovation introduced in neurosurgery in the past decades. It aims to support and guide the neurosurgeon to obtain the best surgical result possible, preventing the occurrence of neurological deficits. The somatosensory evoked potentials (SSEP) assess the integrity of the sensory pathways monitoring the dorsal column-medial lemniscus pathway during spine and cerebral surgery. Motor evoked potentials (MEPs) provide information on the integrity of the motor pathway monitoring the efferent motor pathways from the motor cortex to the muscle t

Intraoperative Neurophysiologic Monitoring for Degenerative Cervical Myelopathy.

Multimodal intraoperative neurophysiologic monitoring is a reliable tool for detecting intraoperative spine injury and is recommended during surgery for degenerative cervical myopathy (DCM). Somatosensory evoked potential (SEP) can be used to monitor spine and peripheral nerve injury during positioning in surgery for DCM. Compensation technique for transcranial evoked muscle action potentials (tcMEPs) should be adopted in intraoperative monitoring during surgery for DCM. Free-running electromyography is a useful real-time monitoring add-on modality in addition to SEP and tcMEP.

Neurophysiologic intraoperative monitoring of the glossopharyngeal and vagus nerves.

Neurophysiologic intraoperative monitoring (NIOM) of the glossopharyngeal and vagus nerves (CN IX and X) is often used during surgeries involving the lower brain stem. Although both of these nerves contain sensory, autonomic, and motor fibers, it is the motor fibers that are most amenable to NIOM. CN IX supplies the stylopharyngeus muscle, and CN X supplies striated muscles in the soft palate, pharynx, and larynx. Monitoring of these CN can be performed by monitoring free running and stimulated electromyography (EMG) from the stylopharyngeus muscle (CN IX) and the vocal cords (CN X). Various s

Multi-modality neurophysiologic monitoring for cardiac surgery.

BACKGROUND: A high percentage of patients who undergo cardiac surgery experience persistent cognitive decline. The costs to insurers from brain injury associated with cardiac surgery is enormous. Furthermore, the same processes that injure the brain also appear to cause dysfunction of other vital organs. Therefore, there are great clinical and economic incentives to improve brain protection during cardiac surgery. This article discusses the methods of monitoring neurophysiologic function during heart surgery, including electroencephalography (EEG), near-infrared spectroscopy (NIRS), transcrani

[Intraoperative neurophysiological monitoring in surgery for spinal intramedullary tumors].

Despite modern advances in neurosurgery, treatment of intramedullary spinal cord tumors is still associated with postoperative neurological impairment and high incidence of adverse outcomes. Intraoperative neurophysiological monitoring allows real-time functional assessment of spinal cord conduction pathways and accurate tumor resection. Currently, concomitant monitoring of somatosensory and motor evoked potentials, as well as D-wave is the most common. This excludes damage to both sensory and motor pathways. In modern literature, there are some differences in methods of intraoperative neuroph

Scoliosis Corrective Surgery With Continuous Intraoperative Neurophysiological Monitoring (IONM).

Scoliosis is a spine deformity that presents as Cobb's angle greater than 10 degrees. Pedicle screw placement can be employed in scoliosis corrective procedures but poses a danger of disrupting the motor and sensory pathways by injuries to the nerves, spinal cord, and vasculature. Occasionally traction weight is applied before the instrumentation for correction. This correction weight may cause spinal cord functional compromise and may result in postoperative paresis or paralysis. A 10-year-old female patient with Cobb's angle of 120 degrees was scheduled for scoliosis correction surgery. A mu

Neurophysiological intraoperative monitoring during an optic nerve schwannoma removal.

This paper reports the case of a patient with optic nerve schwannoma and the first use of neurophysiological intraoperative monitoring of visual evoked potentials during the removal of such tumor with no postoperative visual damage. Schwannomas are benign neoplasms of the peripheral nervous system arising from the neural crest-derived Schwann cells, these tumors are rarely located in the optic nerve and the treatment consists on surgical removal leading to high risk of damage to the visual pathway. Case report of a thirty-year-old woman with an optic nerve schwannoma. The patient underwent sur

Neurophysiological monitoring of cranial nerves during posterior fossa surgery.

Intraoperative neurophysiological monitoring of cranial nerve functions in surgery for microvascular decompression and tumors of the posterior fossa is important for minimizing risk of permanent damage to the nerves. In particular, intraoperative BAEP and the EMG function of muscle innervated by trigeminal and facial muscle have been found useful. We report here our experiences with intraoperative monitoring of brainstem auditory evoked potentials (BAEP) and EMG recorded from muscles supplied by the trigeminal and facial nerves.

Intraoperative neurophysiologic monitoring: are we really that bad?

STUDY DESIGN: Retrospective review of a prospectively collected, multicenter database. OBJECTIVE: To assess the rates of new neurologic deficit (NND) associated with spine surgery. SUMMARY OF BACKGROUND DATA: New neurologic deficit is a potential complication of spine surgery but previously reported rates are often limited by small sample size and single-surgeon experiences. METHODS: The Scoliosis Research Society morbidity and mortality database was queried for spinal surgery cases complicated by NND from 2004 to 2007, including nerve root deficit, cauda equina deficit, and spinal cord def

Neurophysiologic intraoperative monitoring in children with Down syndrome.

OBJECTIVE: Neurophysiological monitoring during complex spine procedures may reduce risk of injury by providing feedback to the operating surgeon. This tool is a well-established and important surgical adjunct in adults, but clinical data in children are not well described. Moreover, to the best of our knowledge, neurophysiologic intraoperative monitoring data have not been reported in children with neurodevelopmental disorders, such as Down syndrome, who commonly present with craniocervical instability requiring internal fixation. The purpose of this study is to determine the reliability and

Neurophysiologic intraoperative monitoring of olfactory and optic nerves.

Olfaction and vision are two sensory functions that can be lost during surgery. Intraoperative neurophysiologic monitoring can be used to monitor the integrity of the olfactory and the visual pathways during surgery. No objective methods have been established to test olfactory evoked potentials (OEPs) in the operating room. It is a challenge to obtain reliable visual-evoked potentials (VEPs) in the operating room. However, it would be very valuable to reliably monitor these pathways during surgery to predict and prevent iatrogenic neurologic injury. The aim of the review is to describe the tec

Neurophysiologic intraoperative monitoring of pediatric cerebrovascular surgery.

The surgical and endovascular treatment of cerebrovascular disorders (CVDs) in children, such as cerebral arteriovenous malformations, cavernous malformations, and moyamoya disease, have become commonplace and routine in many centers. As in the adult population, these procedures carry the risk of intraoperative cerebral ischemia. Therefore, similar strategies used to reduce the risk of cerebral ischemia in adults should be used in children. Unfortunately, there are no published studies on the intraoperative use of available techniques to identify, prevent, or potentially reverse cerebral ische

Intraoperative neurophysiologic monitoring during spinal surgery.

Intraoperative neurophysiologic monitoring (IONM) is a battery of neurophysiologic tests used to assess the functional integrity of the spinal cord, nerve roots, and other peripheral nervous system structures (eg, brachial plexus) during spinal surgery. The underlying principle of IONM is to identify emerging insult to nervous system structures, pathways, and/or related vascular supply and to provide feedback regarding correlative changes in neural function before development of irreversible neural injury. IONM data provide an opportunity for intervention to prevent or minimize postoperative n

Neurophysiologic monitoring in the neuroscience intensive care unit.

Neurologically critically ill patients, more often than others, are unable to communicate and, for a crucial period of time, have the vital functions of their brains hidden in the "black box" of the cranial vault behind a curtain of ambiguity and immobility. Customarily--and naively--we have relied upon beside clinical observations to pierce these barriers. Recent insights lead us to conclude that these "neurochecks" no longer suffice. This article has surveyed four major monitoring systems relied upon by neurointensivists to evaluate the pathophysiology of their patients. Of these, ICPM has t

Neurophysiologic monitoring during cranial base surgery.

The last decade has seen a rapid growth in surgical techniques directed at resection of skull base tumors. Lesions that were previously inoperable can now be resected either partially or in some cases completely. The morbidity in these procedures has diminished with increased surgical experience. Additional aids to successful surgical resection include intraoperative neurophysiologic monitoring techniques. The following text outlines methods of monitoring cranial nerve and brain function in a manner that is directly relevant to cranial base surgery in which so many structures are potentially a
