Hearing Loss as a Neurological Manifestation of Diabetes.

Diabetes mellitus affects millions of people and is a significant public health concern. Neurologically, hearing occurs with intact outer, middle, and inner ear functioning, with brain recognition and decoding. Hearing impairments are underrecognized and contribute to safety issues and poor quality of life. CONTENT: This article identifies how diabetes contributes to neurological changes that manifest as impaired hearing and balance. A pathophysiological review of the auditory pathway and ongoing theories of diabetes-related changes in the auditory nerves and blood vessels results in impaired

Valproic Acid: Special Considerations and Targeted Monitoring.

Valproic acid (VPA) is a medication used to treat multiple neuroscience conditions. It is an inexpensive and useful medication, with a low incidence of adverse drug events. Nonetheless, optimal clinical outcomes require that a series of screening and laboratory steps be followed before the initiation of VPA therapy. An additional aspect of pharmacovigilance is to recognize clinical patterns signaling genetic traits that preclude VPA, background of the black box warnings, targeted assessments, and laboratory monitoring indicated while on VPA. The intention of this article is to provide a focuse

Neurodevelopmental treatment: application to nursing and effects on the hemiplegic stroke patient.

Nursing care of the hemiplegic stroke patient is based on a traditional approach, using compensatory rehabilitative principles. However, during recent years, the neurodevelopmental treatment (NDT) approach, based on the noncompensatory rehabilitative principles and focused on activating the affected side of the hemiplegic stroke patient, is increasingly begin applied to neuroscience nursing. Until recently, little evidence existed to support the superiority of the NDT approach. The NDT approach requires considerable investment in education and training. Therefore, benefit of the NDT approach i

The false localizing signs of increased intracranial pressure.

False localizing signs, although rare, indicate significant increased intracranial pressure with tissue shift, has occurred. A review of the historical perspective highlights the discovery of these false localizing signs, and exploration of the anatomic and physiologic causes provides important insight for neuroscience nurses. Critical thinking skills, and a world view paradigm for clinical practice are developed upon an in-depth understanding of neurologic dysfunction. Collaborative relationships are enhanced by an interdisciplinary approach to assessment of the critically ill neuroscience pa

Sustained delirium reduction in an inpatient neuroscience unit.

BACKGROUND: Delirium is described as acute brain dysfunction that often fluctuates throughout the day and is highly prevalent in hospitalized adults. Delirium negatively affects patient outcomes with consequences including decreased cognitive functioning and increased mortality and healthcare costs. Furthermore, neurological patients are at increased risk for developing delirium due to their underlying diagnoses. PURPOSE: The purpose of this evidence-based practice project was to evaluate the long-term impact of nursing education and use of trained volunteers to prevent the development of del

Recalibrating Nursing Education: A Call to Action for Moving from Pedagogy to Andragogy.

AIM: The time has come for nursing education to fully abandon historic pedagogical models and embrace andragogical principles that honor the adult learner's capacity for self-direction and ownership of their professional development. BACKGROUND: Evidence demonstrates that andragogical approaches that emphasize self-direction, experiential learning, and problem-centered education better serve adult learners. Yet formal deep embrace remains uneven across nursing programs globally. METHOD: This analysis examines evidence distinguishing pedagogical from andragogical approaches in nursing educati

It's not your "run of the mill" meningioma: characteristics differentiating low-grade from high-grade meningeal tumors.

Approximately 30% of primary brain tumors are meningiomas; 90% of these are benign. The remaining 10% have aggressive pathological features and significantly higher recurrence rates. Treatments include surgery, radiation therapy, and other medical therapies. Management of these patients involves vigilant neuroradiological imaging, follow-up visits, symptom management, and ongoing patient and family teaching. Even with aggressive treatment modalities, morbidity and mortality rates remain high.

Management of the Pregnant Patient With a Spinal Cord Injury.

INTRODUCTION: Spinal cord injuries (SCIs) and pregnancy can present a challenging scenario for healthcare professionals. Information regarding the management of patients who become pregnant and have SCIs is limited in the nursing literature, and therefore, more case studies must be presented and research must be conducted to build evidence-based care. Physiologic changes in pregnancy can increase a patient's risk for serious complications during pregnancy and, consequently, may put the fetus at a greater risk. Neuroscience healthcare providers must take into consideration the level of the SCI

Blepharospasm, hemifacial spasm and the nurse's role.

Blepharospasm and hemifacial spasm are involuntary movement disorders that affect the facial muscles. They are classified as cranial dystonias. Their cause is unknown and the underlying pathophysiology is poorly understood. Both dystonias are more common in women than in men. It is the middle-aged group that is most frequently affected. Because of their high visibility, these disorders may cause considerable distress and embarrassment. Affected persons are often mistakenly considered to have psychiatric problems. In addition, both dystonias may result in severe disability. For example, the per

Transient Ischemic Attack and Cognitive Impairment: A Review.

Transient ischemic attack (TIA) is a neurologic deficit resulting from focal ischemia in the brain, spinal cord, or retina. Historically, the definition included symptom resolution within 24 hours. However, recent studies investigating cognition after TIA suggest that deficits in executive function persist at 7 days post-TIA, although few studies have examined these effects long term. Recent advances in neuroimaging techniques provide emerging evidence of permanent microvascular tissue damage in the brain, suggesting that the effects of TIA may persist beyond resolution of focal symptoms. A fu

Meningioma, astrocytoma and germinoma: case presentations of three intracranial tumors.

In this article three intracranial tumors are described including signs, symptoms and radiologic findings. Two of the more common tumor types are discussed--meningioma and astrocytoma, along with one less common type, the germinoma. The germinoma with its less common features provides an interesting comparison. Each tumor is exemplified using case presentations. The neuroscience nurse, in understanding the mechanisms by which these tumors manifest themselves, will be better able to predict treatment, outcome and correlate nursing care as necessary.

Assessment and management of carotid artery trauma associated with mild head injury.

Management of patients with trauma to the central nervous system is a challenge for the neuroscience nurse. When trauma to the carotid artery is present, the management of closed head injury becomes even more complex. Although the incidence of carotid artery dissection is small, the risks of morbidity and mortality are high. This article discusses the mechanism of injury, associated symptoms, and theories of management for victims of carotid artery injury associated with blunt craniocervical trauma. New assessment considerations for nursing diagnosis and intervention are provided.

Rabbit Maternal Behavior: A Perspective from Behavioral Neuroendocrinology, Animal Production, and Psychobiology.

Rabbit maternal behavior (MB) impacts meat and fur production on the farm, survival of the species in the wild, and pet welfare. Specific characteristics of rabbit MB (i.e., three-step nest building process; single, brief, daily nursing bout) have been used as models for exploring particular themes in neuroscience, like obsessive-compulsive actions, circadian rhythms, and cognition. Particular hormonal combinations regulate nest building by acting on brain regions controlling MB in other mammals. Nonhormonal factors like type of lodging and the doe's social rank influence nursing and milk prod

The Silence Between Infusions: How Quiet Presence Shapes Healing in Oncology Nursing.

In ambulatory oncology settings, some of the most meaningful healing occurs not during active treatment but within the quiet spaces surrounding it. This reflective essay explores therapeutic silence and attuned presence as holistic nursing interventions that support emotional, relational, and spiritual well-being for individuals receiving cancer treatment. Drawing on mindfulness-based nursing literature, communication theory, and co-regulation neuroscience, it examines how nurses create conditions of safety and connection even in time-pressured environments. De-identified clinical narratives i
