Neurotologic manifestations of the osteopetroses.

Facial nerve paralysis or symptoms of audiovestibular nerve dysfunction may be the first indication of one of the group of bone diseases known as the osteopetroses. We describe three such patients. The pathophysiology of these diseases and the treatment of the resulting symptoms of facial and audiovestibular nerve dysfunction are discussed.

Neurotology symptoms at referral to vestibular evaluation.

BACKGROUND: Dizziness-vertigo is common in adults, but clinical providers may rarely diagnose vestibular impairment and referral could be delayed. To assess neurotology symptoms (including triggers) reported by patients with peripheral vestibular disease, during the year just before their referral to vestibular evaluation. METHODS: 282 patients with peripheral vestibular disease and 282 control subjects accepted to participate. They had no middle ear, retinal, neurological, psychiatric, autoimmune or autonomic disorders. They reported their symptoms by a standardized questionnaire along with

Four-handed, two-surgeon microsurgery in neurotology.

INTRODUCTION: Since the early 1960s, microsurgical removal has been the standard for curative treatment of vestibular schwannomas and neurotologic skull base lesions. The narrow operative corridor with only one surgeon actively operating sometimes interferes with simultaneous application of standard surgical principles such as suction, counter tension, and dissection and makes attempts at "co-surgery" into "sequential" surgery. Surgical efficiency is often delayed by the need to change surgical instruments and reorient the surgical field. METHODS: In this four-handed technique, the microscope

Hemostasis in Otologic and Neurotologic Surgery.

Hemostasis is a critical component of otologic and neurotologic surgery. In these surgeries the surgical field is small; thus, even a small amount of bleeding can obstruct the view of critical and extremely small structures. Additionally, relatively large vascular structures traverse the area; if they are encroached on by trauma or disease, bleeding must be controlled within a very small space in a meticulous fashion that does not encroach on structures of the middle ear and mastoid. The authors discuss several hemostatic agents in the middle ear, mastoid, and lateral skull base, highlighting

Use of Alloderm in the neurotologic setting.

PURPOSE: To determine whether Alloderm (Life Cell Corporation, The Woodlands, TX) can be used to cover canal wall down mastoid defects, as well as repair dural defects from translabyrinthine and transpetrosal approaches to the skull base. METHODS: A retrospective analysis was performed at an academic tertiary care medical center. Eighteen patients were operated on with canal wall down mastoidectomies, and Alloderm was used to reconstruct the tympanic membrane and line the mastoid cavity. Eleven patients had dural defects reconstructed after skull base approach surgery. We wanted to determine

Neurotological evaluation of Harada's disease.

We have had 30 patients with Harada's disease at our clinic from 1980 to 1993 and their neurotological findings were retrospectively reviewed. There were 12 males and 18 females, whose ages ranged from 15-years to 67-years. Preceding symptoms such as headache or common cold were observed in 50% of the patients. Pleocytosis of the cerebrospinal fluid was found in 89% of the patients. Subjective cochlear and/or vestibular symptoms were noted in 60% of the patients. In 43 of the 50 ears, sensorineural hearing loss (< 40 dB) was observed. In 77% of the patients, vestibular function tests gave abno

Promontory testing in neurotologic diagnosis.

The ability of promontory testing (PT) to differentiate a retrocochlear from a cochlear lesion in a group of 88 patients having unilateral sensorineural hearing loss was assessed. Promontory stimulation was performed, using the Nucleus Promontory Stimulator (model Z10012, Cochlear Corporation, Melbourne, Australia), by placing a needle electrode transtympanically on the basal turn of the cochlea. Patients with measurable hearing gave inconsistent results. Patients who had undergone eighth nerve section or translabyrinthine removal of acoustic neuroma were used as controls. They had no percepti

Neurotologic manifestations of the fibromyalgia syndrome.

The purpose was to assess otologic symptoms, and audiologic and vestibular findings in fibromyalgia (FM) syndrome. Twenty-four female patients with FM syndrome (FMS) were included in the study. The assessments were based on history, physical examination, audiometry, bithermal caloric testing and auditory brainstem response (ABR) testing. Dizziness was the most common complaint of the patients, and was followed by tinnitus, hearing loss and vertigo. Almost 50% of the patients had some sort of otologic symptoms.Dix-Halpike maneuver proved positional rotary vertigo in 5 (20.8%) patients. The audi

Neurotologic findings in basilar migraine.

Treatment of a patient with otologic symptoms and associated migraine-like headache presents the otolaryngologist with formidable problems. Although clinical practice and scientific publications recognize their frequent association, relationships have yet to be well defined. This study seeks to add order to disarray by delineating symptoms and signs of a clearly identified group of migraine patients. Fifty patients with well-defined basilar migraine underwent a thorough neurotologic examination, as well as comprehensive auditory and vestibular testing. Patients were selected from 5880 patients

Use of bacitracin for neurotologic surgery.

Wound infection, cerebrospinal fluid leak, and meningitis are serious potential complications of neurotologic procedures that transgress the posterior cranial fossa dura. A study of 236 patients was made to determine the effect of perioperative intravenous antibiotics and topical bacitracin irrigation on the incidence of these complications. Of the 236 patients, 170 (72%) underwent translabyrinthine resection of acoustic tumors, while 66 (28%) underwent retrolabyrinthine vestibular nerve section. Patients were divided into four groups: those who received no antibiotics, those who received peri

Charles Skinner Hallpike and the beginnings of neurotology.

Although the symptom of vertigo had been well-recognized for several thousand years, it was not until the pioneering work of Prosper Ménière in the mid-19th century that it was appreciated that vertigo could originate from damage to the inner ear. Before that time, patients with vertigo (regardless of the cause) were said to have "cerebral congestion," a condition resulting from excessive blood filling the brain. Bloodletting and leeches to relieve the congestion were the treatment of choice. The discovery of endolymphatic hydrops in temporal bone specimens from patients with Ménière's disease

Vestibular evoked myogenic potentials (VEMPs): usefulness in clinical neurotology.

Testing vestibular evoked myogenic potentials (VEMPs) may be the most important new clinical test for evaluation of vestibular function developed during the past 100 years since the introduction of the caloric test. VEMPs are easily recordable and therefore suitable for everyday testing in clinical neurotology. VEMPs in response to air-conducted sound stimulation using surface electrodes over the sternocleidomastoid muscles reveal saccular function, inferior vestibular nerve function, and vestibulocollic connections. At present, VEMPs are of clinical importance for estimating the severity of p

Sir Charles Alfred Ballance: contributions to otology and neurotology.

STUDY DESIGN: A review of primary and secondary historical and scientific literature concerning the life and writings of Sir Charles Alfred Ballance (1856-1936). RESULTS: Sir Charles Alfred Ballance was a pioneer in otology and neurotology, responsible for many "firsts" in the field, including the complete removal of a cerebellopontine angle tumor with significant patient survival and grafting the mastoid cavity with epithelium to speed healing. He was a strict abdicator of the complete mastoid operation with ligature of the jugular vein and drainage of the lateral sinus, and he advanced many

Subclavian steal syndrome: neurotological manifestations.

The subclavian steal syndrome is characterized by a subclavian artery stenosis located proximal to the origin of the vertebral artery. In this case, the subclavian artery steals reverse-flow blood from the vertebrobasilar artery circulation to supply the arm during exertion, resulting in vertebrobasilar insufficiency. As the vertebrobasilar arterial system feeds both the peripheral and central auditory and vestibular systems, in subclavian steal syndrome, neurotological symptoms are expected because of the vertebrobasilar insufficiency. In this report, we describe three patients suffering from

Neurotologic management of intracranial epidermoid tumors.

OBJECTIVES: Epidermoid cysts are the most common intracranial embryonal tumor, although they account for only 1% of all intracranial tumors. Epidermoids often spread into several intracranial compartments. Thorough preoperative surgical planning is imperative for safe epidermoid removal. This paper discusses the neurotologic management of intracranial epidermoid cysts STUDY DESIGN: Retrospective chart review. METHODS: A database search revealed 10 patients with diagnosis of intracranial epidermoid cysts between January 1, 1971 and December 31, 2003 at our institution. RESULTS: Six males and

Management of pseudomeningocele following neurotologic procedures.

OBJECTIVES: A pseudomeningocele results from the escape of cerebrospinal fluid through a dural defect with trapping of subarachnoid fluid in surrounding soft tissue. The present study evaluates the incidence of pseudomeningocele following neurotologic procedures and delineates an algorithm for management. METHODS: A retrospective review of 375 consecutive patients undergoing neurotologic procedures at a single institution identified 17 patients with the postoperative complication of pseudomeningocele. RESULTS: The incidence of pseudomeningocele formation was 4.5% in the present study. Fourte

Pediatric neurotologic skull base surgery.

The objectives of neurotologic skull base surgery are complete resection of the lesion and high-grade function following surgery. There is a perception that these goals are more difficult to achieve in children than in adults. Skull base disease in children and adolescents is rare. Of the 292 skull base tumors treated from 1970 to 1995 by The Otology Group in Nashville, 15 were in patients 21 years of age or younger, with only 5 patients under 10 years old. In this retrospective study, the authors review these 15 cases and compare them to their adult series. The pathology encountered in the 15
