[Nociception and chronic oral and cervicofacial pain].

This short review focuses on the main properties of the nervous system responsible for pain originating from oral, nasal and pharyngeal cavities. First, the major roles of the somatic sensations coming from the orofacial and pharyngeal areas are emphasized. Then, the description of the respective peripheral fields of the different cranial nerves involved in these somatic sensations is followed by the description of the primary afferent neurons and the central nuclei and pathways. The review emphasizes that pain does not result from a simple transmission of nervous impulses that would follow a

Qualitatively altered nociception in chronic myofascial pain.

The aim of the present study was to investigate the stimulus-response function for pressure versus pain in patients with myofascial pain. Forty patients with chronic tension-type headache and 40 healthy controls were examined. Tenderness in 8 pericranial muscles and tendon insertions was evaluated by manual palpation with a standardized evaluated methodology. Thereafter, a highly tender muscle and a largely normal muscle were palpated with 7 different pressure intensities using a palpometer, and the induced pain was recorded by the subjects on a visual analogue scale blinded for the observer.

Schwann cells modulate nociception in neurofibromatosis 1.

Pain of unknown etiology is frequent in individuals with the tumor predisposition syndrome neurofibromatosis 1 (NF1), even when tumors are absent. Nerve Schwann cells (SCs) were recently shown to play roles in nociceptive processing, and we find that chemogenetic activation of SCs is sufficient to induce afferent and behavioral mechanical hypersensitivity in wild-type mice. In mouse models, animals showed afferent and behavioral hypersensitivity when SCs, but not neurons, lacked Nf1. Importantly, hypersensitivity corresponded with SC-specific upregulation of mRNA encoding glial cell line-deriv

Acid-sensing ion channels modulate bladder nociception.

Sensitization of neuronal pathways and persistent afferent drive are major contributors to somatic and visceral pain. However, the underlying mechanisms that govern whether afferent signaling will give rise to sensitization and pain are not fully understood. In the present report, we investigated the contribution of acid-sensing ion channels (ASICs) to bladder nociception in a model of chemical cystitis induced by cyclophosphamide (CYP). We found that the administration of CYP to mice lacking ASIC3, a subunit primarily expressed in sensory neurons, generates pelvic allodynia at a time point at

Toxins in anti-nociception and anti-inflammation.

The use of toxins as novel molecular probes to study the structure-function relationship of ion-channels and receptors as well as potential therapeutics in the treatment of wide variety of diseases is well documented. The high specificity and selectivity of these toxins have attracted a great deal of interest as candidates for drug development. This review highlights the involvement of the proteins and peptide toxins as well as non-proteinaceous compounds derived from both venomous and non-venomous animals, in anti-nociception and anti-inflammation. The possible mechanisms of these potential t

Effects of diazepam on nociception in rats.

Acute i.p. injection of diazepam (1 mg/kg) resulted in a moderate increase in the tail-flick latency in rats. Tolerance to this diazepam effect developed after 10 days of diazepam treatment (1 mg kg-1 day-1). The benzodiazepine antagonist Ro 15-3505 only partially reversed the effect of diazepam on nociception. Naloxone (5 mg/kg i.p.) failed to affect the effect of diazepam on nociception, while the kappa antagonist MR 2266 fully antagonized the diazepam-induced increase of the tail-flick latency. Diazepam injected intracerebroventricularly (1, 5, 20 micrograms/rat) did not alter basal nocicep

Cortical nociceptive systems.

There is evidence that the cerebral cortex is involved in the perception of pain but no specific area appears to be the 'pain centre'. Limited knowledge exists on the cortical processing of the noxious input. The nociceptors are most likely to activate at least two different systems with different characteristics. One has a bilateral cortical projection, no apparent topographical pattern, low synaptic security and excites cells in large areas. This system may give rise to the widespread increase in blood flow and the widely distributed surface potentials recorded in man following a painful sti

Astrocytogenic bidirectional plasticity at spinal nociceptive synapses regulates acute nociceptive processing.

Astrocytes are key players in chronic pain, driving maladaptive changes in neuronal circuits. Yet, their influence on acute nociception-the body's first line of defense against harmful stimuli-remains poorly understood. Using chemogenetic tools to mimic endogenous astrocytic G-protein-coupled receptor-mediated signaling, we reveal that astrocytes induce bidirectional plasticity at nociceptive synapses in the dorsal horn. Depending on the state of these synapses and on concurrent afferent input, astrocytes can either suppress nociceptive transmission by enhancing glycinergic tone, or amplify sy

Effects of substance P on nociceptive and non-nociceptive trigeminal brain stem neurons.

As little information is available on the chemistry of synaptic transmission in trigeminal brain stem nuclei, an iontophoretic study was done on the effects of glutamate and substance P on single neurons in trigeminal nuclei oralis and caudalis in cats anesthetized with chloralose and paralyzed. The neurons were additionally studied for their responses to natural noxious and innocuous cutaneous and intraoral stimuli as well as to bipolar stimulation of the ipsilateral and contralateral canine tooth pulps, the exposed infraorbital and superior laryngeal nerves and forepaw. Abnormal head nociceptive withdrawal reaction to facial nociceptive stimuli in Parkinson's disease.

INTRODUCTION: Trigemino-cervical-spinal reflexes (TCSRs) are complex brainstem stereotyped nociceptive responses involved in a defensive withdrawal reaction of the head from facial nociceptive stimuli. OBJECTIVE: The present study was undertaken to collect data on possible TCSR abnormalities in idiopathic Parkinson's disease (PD) and investigate any correlation with motor signs and L-DOPA administration. METHODS: TCSRs were registered from the semispinalis capitis and biceps brachii muscles after electrical stimulation of the supraorbital nerve in 18 patients with PD and 24 controls. The lat

Agreement between analgesia nociception index (ANI) and surgical pleth index (SPI) in assessing nociception under varying systemic vascular resistance conditions.

BACKGROUND: Analgesia nociception index (ANI) is derived from heart rate variability, while surgical pleth index (SPI) is a combined measurement of pulse wave amplitude and heartbeat interval. Given that systemic vascular resistance (SVR) may affect the pulse wave amplitude, the reliability of SPI may be affected during dynamic changes in SVR. We aimed to measure the agreement between ANI and SPI in assessing nociception across varying SVR. METHODS: In this prospective clinical study, we collected ANI and SPI data simultaneously at 5-minute intervals in each patient undergoing living donor liv

Effect of phobic visual stimulation on spinal nociception.

To explore the role of strong negative emotions in spinal nociception, we evaluated the effect of fear-relevant videos of small animals on the nociceptive withdrawal reflex (NWR) and reflex-related pain perception in healthy subjects with a specific phobia of small animals. Twenty healthy subjects with a specific phobia of small animals diagnosed according to DSM-V criteria were included in this study. The NWR was evoked in the lower limb by stimulating the sural nerve and recording EMG activity in the biceps femoris. NWR pain-related perception was quantified on an 11-point numerical rating s

Nociception at the diabetic foot, an uncharted territory.

The diabetic foot is characterised by painless foot ulceration and/or arthropathy; it is a typical complication of painless diabetic neuropathy. Neuropathy depletes the foot skin of intraepidermal nerve fibre endings of the afferent A-delta and C-fibres, which are mostly nociceptors and excitable by noxious stimuli only. However, some of them are cold or warm receptors whose functions in diabetic neuropathy have frequently been reported. Hence, it is well established by quantitative sensory testing that thermal detection thresholds at the foot skin increase during the course of painless diabet

Metabotropic glutamate receptor 5 (mGluR5) regulates bladder nociception.

BACKGROUND: Interstitial cystitis/painful bladder syndrome (IC/PBS), is a severely debilitating chronic condition that is frequently unresponsive to conventional pain medications. The etiology is unknown, however evidence suggests that nervous system sensitization contributes to enhanced pain in IC/PBS. In particular, central nervous system plasticity of glutamatergic signaling involving NMDA and metabotropic glutamate receptors (mGluRs) has been implicated in a variety of chronic pain conditions. Here, we test the hypothesis that mGluR5 mediates both non-inflammatory and inflammatory bladder

Sertraline inhibits formalin-induced nociception and cardiovascular responses.

The objective of the present study was to determine the antihyperalgesic effect of sertraline, measured indirectly by the changes of sciatic afferent nerve activity, and its effects on cardiorespiratory parameters, using the model of formalin-induced inflammatory nociception in anesthetized rats. Serum serotonin (5-HT) levels were measured in order to test their correlation with the analgesic effect. Male Wistar rats (250-300 g) were divided into 4 groups (N = 8/per group): sertraline-treated group (Sert + Saline (Sal) and Sert + Formalin (Form); 3 mg·kg-1·day-1, ip, for 7 days) and saline-tre

Neuropeptides and their role in nociception and analgesia.

The authors highlight the potential importance of neuropeptides as clinical tools in the study and treatment of pain. Although many questions remain unanswered, it is encouraging that several clinical and experimental advances have shed new light on the neuropharmacology of nociception and have prompted new hope for more effective treatments of such diverse problems as chronic intractable pain, migraine, and drug dependency. Within the next few years, it is anticipated that further work in this area will lead to better basic and clinical understanding of these important clinical problems.

Photoplethysmography parameters in monitoring nociception during general anesthesia.

BACKGROUND: Validated monitoring methods for evaluating the balance of nociception and anti-nociception (BNAN) are needed in general anesthesia. This study assessed six photoplethysmography (PPG) parameters, computed from finger photoplethysmographic waveforms in patients undergoing gynecological surgery under general anesthesia. METHODS: A total of 20 participants were included, each undergoing general anesthesia with propofol and remifentanil. The same concentration of remifentanil was maintained throughout the experiment, four different intensities of electrical stimulation were administer

Contribution of Sympathetic Sensory Coupling to Craniofacial Nociception.

Stress and anxiety are associated with increased pain intensity in temporomandibular disorders (TMDs) patients. It is possible that this association is due to a direct interaction between the sympathetic and sensory nervous systems. This narrative review examines evidence for a potential sympathetic sensory interaction in deep craniofacial tissues and the trigeminal ganglion. Research articles were identified using PubMed with the mesh terms adrenergic, ganglion neuron, masseter, sensory, signaling, temporomandibular and trigeminal and, subsequently, from the reference lists of those articles

Different perspectives for monitoring nociception during general anesthesia.

Safe anesthesia is achieved using objective methods that estimate the patient's state during different phases of surgery. A patient's state under anesthesia is characterized by three major aspects, which are linked to the main effects produced by each of the families of anesthetic agents administered: hypnosis, analgesia, and muscular relaxation. While quantification techniques designed to assess muscular relaxation under neuromuscular blocking agents have a relatively long history with a high degree of standardization and understanding (e.g., the train-of-four), the knowledge and techniques u
