Nocturnal enuresis: systems for assessment.

Childhood nocturnal enuresis, or bedwetting, is a common childhood condition that causes children and their families stress and embarrassment and which can radically affect the lives of these children. Penny Dobson and Anne Weaver outline how nurses can inform and support these families, and describe a research-based approach, the three systems, which provides a useful framework for assessing the problem before choosing the most appropriate treatment strategy. Specific help that could be offered teenagers is also outlined.

Psychosocial implications of nocturnal enuresis.

Primary nocturnal enuresis (PNE), a biobehavioral problem affecting approximately 5,000,000 children in the United States, is among the most bothersome and frustrating disorders of childhood. Negative psychosocial consequences are common, secondary to the impact of enuresis on family members and others. The enuretic child may be at increased risk for emotional or even physical abuse from family members and may experience stress related to fear of detection by peers. These factors contribute to the loss of self-esteem that the enuretic child often experiences. Fortunately, a number of treatment

Development of a novel scale for primary monosymptomatic nocturnal enuresis: Nocturnal Enuresis Symptom Score (NESS).

INTRODUCTION: Monosymptomatic nocturnal enuresis (MNE) is defined as urinary incontinence occurring during sleep in absence of daytime lower urinary tract symptoms. International Children Continence Society (ICCS) classifies treatment success as complete response (100 % resolution of symptoms), partial response (50-99 % reduction in symptoms) and no response (<50 % resolution of symptoms). There are no symptom scores for management or follow-up for MNE in the existing literature. OBJECTIVE: In this study, we aimed to develop a symptom score for monitorization of MNE treatment. STUDY DESIGN:

Pathophysiology and impact of nocturnal enuresis.

Nocturnal enuresis in children is not a psychogenic disorder. It is caused by a hereditary delay in maturation of the somatic mechanisms (reduction of nocturnal urine production and a normal arousal to a full bladder) which prevent the child from wetting the bed. Traditionally, doctors treating bedwetting children have used an expectant attitude, because nocturnal enuresis has been looked upon as self-limiting and harmless. According to recent research this is not true. More than 5% of children and 0.5% of the adult population report nocturnal enuresis, meaning that 10% of enuretic children wi

Role of Urodynamic Study in Nocturnal Enuresis: Urodynamic Findings and Treatment Outcome Correlation in Children with Pharmacotherapy-resistant Monosymptomatic Nocturnal Enuresis or Severe Non-monosymptomatic Nocturnal Enuresis.

OBJECTIVES: This study aimed to determine whether or not a urodynamic study (UDS) is beneficial for management of pediatric nocturnal enuresis (NE), especially in pharmacoresistant monosymptomatic nocturnal enuresis (PRMNE) or severe non-monosymptomatic nocturnal enuresis (NMNE) patients. METHODS: Children with PRMNE or severe NMNE who underwent a UDS for the process of NE treatment were retrospectively reviewed. The UDS findings of patients and treatment outcomes of subsequent tailored therapies according to the UDS findings were analyzed. RESULTS: A total of 80 children (50 boys and 30 gir

Evaluating nocturnal polyuria in Japanese children with nocturnal enuresis.

BACKGROUND: The purpose of this study was to assess whether enuretic Japanese patients with nocturnal polyuria (NP) who met Hoashi's criteria (6-9 years: ≥200 mL; 10 years and older: ≥250 mL) met the International Children's Continence Society (ICCS; expected bladder capacity × 130%) and Rittig's criteria for nocturnal polyuria (>[age+9] × 20 mL). We also compared the effectiveness of 1-desamino-8-d-arginine vasopressin (DDAVP) with the three criteria. METHODS: Fifty-four patients who had NP with normal bladder capacity were enrolled: 36 boys (67%); median age, 8 (interquartile range: 7-9). W

Nocturnal enuresis among primary school children.

To determine the prevalence and personal and family risk factors for nocturnal enuresis (NE) among primary school children in Al-Mukalla City, Yemen, we conducted a cross-sectional survey using a self-administered, three-part structured questionnaire involving 832 school children aged 6 - 15 years between 2007 and 2008. We assessed participants' socio-demographic factors, family characteristics and factors related to the presence of NE. The mean age of the children was 11.5 (±2.7) years. The overall prevalence of NE was 28.6%, with a predominance of girls, and the prevalence decreased with inc

Evaluation of nocturnal bladder capacity and nocturnal urine volume in nocturnal enuresis.

OBJECTIVE: To investigate the diagnostic accuracies of maximum voided volume (MVV) and first morning urine osmolality and compare these with accuracies of nocturnal bladder capacity (NBC) and nocturnal urine volume, respectively, in children with nocturnal enuresis. MATERIALS AND METHODS: A total of 70 children with nocturnal enuresis were assessed (mean age 7.1 ± 2.2 years). Baseline parameters were obtained by measuring MVV using a voiding diary, first morning urine osmolality on a wetting day, diaper weight by checking twice per night and first morning urine volume. RESULTS: The proportio

Monosymptomatic nocturnal enuresis is associated with abnormal nocturnal bladder emptying.

PURPOSE: We investigate the nature of enuresis episodes in monosymptomatic nocturnal enuresis using a fluid provocation model. MATERIALS AND METHODS: The study included 18 children 7 to 13 years old with monosymptomatic nocturnal enuresis. Based on basal home recordings patients were subgrouped into those with a normal nocturnal urine output and those with nocturnal polyuria (mean nocturnal urine production on wet nights exceeding 130% of functional bladder capacity, normal functional bladder capacity for age provided). Children were admitted to the hospital for 4 consecutive nights. After an

Management of treatment-resistant nocturnal enuresis.

Nocturnal enuresis is defined as intermittent urinary incontinence during sleep in children 5 years of age and older, occurring at least once a month for at least 3 months. In Japan, pediatricians who do not specialize in nocturnal enuresis have become more proactive in treating the condition since 2016, when the guidelines for treating it were revised for the first time in 12 years. For monosymptomatic nocturnal enuresis, the first step is lifestyle guidance, with a focus on the restriction of fluid intake at night; however, if lifestyle guidance does not decrease the frequency of nocturnal e

Nocturnal enuresis in sickle cell disease.

Nocturnal enuresis is a prevalent and challenging problem in children and young adults with sickle cell disease (SCD). Limited progress has been made in elucidating etiology and pathophysiology of nocturnal enuresis in individuals with SCD. Among adults with SCD ages 18-20 years, approximately 9% report nocturnal enuresis. Nocturnal enuresis contributes to decreased health related quality of life in people with SCD, resulting in low self-esteem and sometimes social isolation. Postulated non-mutually exclusive causes of nocturnal enuresis in individuals with SCD include hyposthenuria leading to

Therapeutic options in childhood nocturnal enuresis.

Monosymptomatic nocturnal enuresis, a heterogeneous condition, is frequently treated in children aged >5 years. Of the various treatment options, enuresis alarm has been widely advocated as being effective for treating nocturnal enuresis, while extracorporeal pelvic floor magnetic stimulation for overactive bladder, urge incontinence and urgency-frequency syndrome has not yet been confirmed by controlled studies as primary treatment for monosymptomatic nocturnal enuresis. Desmopressin, an antidiuretic hormone (ADH) analog, or arginine vasopressin (AVP), can resolve primary nocturnal enuresis b

[Endocrine theory of idiopathic nocturnal enuresis].

Body fluid homeostasis is maintained by the kidney. Such an accurate control in achieved via the secretion of antidiuretic hormone (ADH), the secretion of which is regulated by hypothalamic osmoreceptors. Both urine flow rate and the excretion of most electrolytes have a diurnal rhythm; they increase during daytime and decrease during nighttime. Such a rhythm seems to be absent in some subjects who suffer from bedwetting because of relative polyuria. In these cases, the polyuria is associated with a decreased nocturnal secretion of ADH and the subsequent excretion of dilated urine. A deficit i

[Dry bed training in nocturnal enuresis].

In order to determine the prevalence of bedwetting, in the period September 1987-June 1989 an investigation was conducted among 1882 pupils of 15 primary schools in a rural area of the Netherlands. The investigation coincided with the periodical medical examination for which all pupils of these groups were invited and for which over 99% reported. In addition to this epidemiological study, a study was made of the effect of dry bed training, as a part of youth health care, in 36 children with nocturnal enuresis. Of the children examined, 8% wet their beds at least once a week, boys twice as ofte

Predicting treatment outcome in nocturnal enuresis.

A presenting sample of 127 consecutive referrals to a community based enuresis clinic were evaluated after treatment with baseline behavioural recording and the enuresis alarm. Almost one in five became dry after baseline recording only while 81 of 96 (84%) enuretics who used the alarm achieved the initial dryness criterion. Successful outcome was associated with the absence of adverse environmental factors and psychiatric disorders in the child. A logistic regression procedure enabled a risk score to be created so that successful outcome could be predicted. Psychiatric disorder in the child,

The role of sleep in the pathophysiology of nocturnal enuresis.

Nocturnal enuresis (NE) is a common condition affecting 5-10% of all 7-year-old children. NE pathophysiology relies on three main factors, abnormal bladder function, excess urine production during sleep and the inability to awaken to the signals of a full bladder. The aim of this review is to evaluate the connection between sleep and its structure and the pathophysiology of NE. NE often occurs early at night and primarily in sleep stage 2 and "deep sleep". Although sleep stage distribution seems similar between NE and healthy children recent studies indicate differences in sleep microstructure

Nocturnal enuresis: an approach to assessment and treatment.

On the basis of strong evidence, although primary monosymptomatic nocturnal enuresis (PMNE) is common and most children will outgrow the condition spontaneously, the psychological effect to the child can be significant and represents the main reason for treatment of these children. On the basis of international consensus panels, treatment of PMNE should be targeted toward the specific type of bedwetting patterns the child has, using bladder diary, sleep history, and daytime elimination concerns as a guide (Table 3). On the basis of international consensus panels, it is important for the primar

[Effectiveness of behavioral intervention in nocturnal enuresis].

BACKGROUND: Nocturnal enuresis is a common, often misunderstood condition that affects millions of children. Although the rate of spontaneous resolution is 15 % per year, treatment is often required for social or psychological reasons. OBJECTIVE: To determine the prevalence of nocturnal enuresis in a primary health care area, how was it diagnosed, which treatments were employed and treatment response. MATERIAL AND METHODS: A retrospective study was performed that included all the medical records of children aged between 6 and 14 years old in a basic health area. RESULTS: Twenty-seven childr
