Trends in Prescribing of Non-steroidal Anti-inflammatory Medications in the US Ambulatory Care Setting From 2006 to 2016.

While opioid prescribing has significantly decreased from a peak in 2012, less is known about the national utilization of non-opioid analgesics such as non-steroidal anti-inflammatory drugs (NSAIDs) and acetaminophen (APAP) in the context of the opioid crisis. The objective of this study is to characterize the prescribing trends of NSAIDs and APAP in the US ambulatory care setting. We conducted repeated cross-sectional analyses using the 2006-2016 National Ambulatory Medical Care Survey. NSAID-involved visits were defined as patient visits among adults in which NSAIDs were ordered, supplied, a

Non-Prescribed Use of Opioid Agonist Medications and Associations with Non-Fatal Overdoses: A Repeated Cross-Sectional Study across a Decade of Reduced Monitoring.

UNLABELLED: <p>Introduction: Non-prescribed use of opioid agonist treatment (OAT) medications is a public health concern. This study analyzes the prevalence of non-prescribed use and non-fatal overdoses in Norway in 2013 and 2023, a period marked by an increasingly flexible OAT regimen, and examines associations between non-prescribed use and non-fatal overdoses. METHODS: Cross-sectional surveys with two convenience samples (n1 = 611 in 2013, n2 = 523 in 2023) of street-recruited individuals, who reported recent use of opioids and/or stimulants but were not currently enrolled in OAT, were emp

Pharmacist and Homeless Outreach Engagement and Non-medical Independent prescribing Rx (PHOENIx): a study protocol for a pilot randomised controlled trial.

INTRODUCTION: The number of people experiencing homelessness (PEH) is increasing worldwide. Systematic reviews show high levels of multimorbidity and mortality. Integrated health and social care outreach interventions may improve outcomes. No previous studies have targeted PEH with recent drug overdose despite high levels of drug-related deaths and few data describe their health/social care problems. Feasibility work suggests a collaborative health and social care intervention (Pharmacist and Homeless Outreach Engagement and Non-medical Independent prescribing Rx, PHOENIx) is potentially benef

Non-adherence to medications prescribed to patients with heart failure in general practice: prevalence, risk factors and association with mortality and hospitalisation.

AIM: In the UK, pharmacological management of patients with heart failure (HF) occurs predominantly in general practice. Using data from the Clinical Practice Research Datalink, we examined the prevalence and risk factors for medication non-adherence and its association with hospitalisation and mortality over a 9-year period. METHODS: A retrospective cohort study of 127 927 patients, ≥18 years old in England with incident HF diagnosed during 1 January 2009 to 31 December 2018. We evaluated non-adherence to any ACE inhibitor, angiotensin receptor blocker, β-blocker or mineralocorticoid recepto

Self-medication with non-prescribed pharmaceutical agents in an area of low malaria transmission in northern Tanzania: a community-based survey.

BACKGROUND: Self-treatment with antimicrobials is common in sub-Saharan Africa. Little is known about the prevalence of this practice where malaria transmission intensity is low, and little is known about the prevalence of self-treatment with other medications such as antihypertensives and antihyperglycemics. METHODS: A two-stage randomized population-based cluster survey with selection proportional to population size was performed in northern Tanzania. Self-identified healthcare decision-makers from randomly selected households were asked to report instances of self-medication without a pres

Perspectives of people experiencing homelessness with recent non-fatal street drug overdose on the Pharmacist and Homeless Outreach Engagement and Non-medical Independent prescribing Rx (PHOENIx) intervention.

INTRODUCTION: In Scotland, a third of all deaths of people experiencing homelessness (PExH) are street-drug-related, and less than half of their multiple physical- and mental health conditions are treated. New, holistic interventions are required to address these health inequalities. PHOENIx (Pharmacist Homeless Outreach Engagement and Non-medical Independent prescribing Rx) is delivered on outreach by National Health Service (NHS) pharmacist independent prescribers in partnership with third sector homelessness charity workers. We describe participant's perspectives of PHOENIx. METHODS: This

Antibiotic prescribing by non-medical healthcare professionals in primary care: a systematic review of effects on antibiotic use, patient experience, interprofessional relationships and job satisfaction.

BACKGROUND: Demand for primary care services has increased significantly in the last decade, with substantial pressure placed on general practitioners (GPs), such as in the English Pharmacy First programme. Developing a more diverse skill mix to include other healthcare professionals (HCPs), such as nurses and pharmacists in the management of common, acute, self-limiting infections is considered part of the solution to this issue in many countries. AIM/OBJECTIVES: This study aims to review the evidence available on the effects of expanding antibiotic prescribing to include HCPs other than doc

Impact of Age on Prescribing Patterns of Cardiovascular Medications in Older Japanese Patients with Non-Dialysis-Dependent Chronic Kidney Disease: A Cross-Sectional Study.

AIM: Older patients with chronic kidney disease (CKD) are more likely to be excluded from clinical trials. This exclusion affects the quality of cardiovascular disease (CVD) prevention in this population. METHODS: Baseline data from the Fukuoka Kidney Disease Registry (FKR) cohort, which included 4476 adult patients with CKD stages G1-G5, were cross-sectionally analyzed to compare the use of recommended drugs for preventing CVD in each age group. RESULTS: Different prescribing patterns were observed according to age for the cardiovascular drug classes. Older patients with CKD were less likel

A longitudinal observational study with ecological momentary assessment and deep learning to predict non-prescribed opioid use, treatment retention, and medication nonadherence among persons receiving medication treatment for opioid use disorder.

BACKGROUND: Despite effective treatments for opioid use disorder (OUD), relapse and treatment drop-out diminish their efficacy, increasing the risks of adverse outcomes, including death. Predicting important outcomes, including non-prescribed opioid use (NPOU) and treatment discontinuation among persons receiving medications for OUD (MOUD) can provide a proactive approach to these challenges. Our study uses ecological momentary assessment (EMA) and deep learning to predict momentary NPOU, medication nonadherence, and treatment retention in MOUD patients. METHODS: Study participants included a

Non-fatal overdose risk associated with prescribing opioid agonists concurrently with other medication: Cohort study conducted using linked primary care, secondary care and mortality records.

BACKGROUND AND AIMS: An apparently protective effect of opioid agonist treatment (OAT) on all-cause and cause-specific mortality risk has been widely reported. Non-fatal overdose (NFO) often precedes subsequent drug-poisoning deaths. We hypothesized that benzodiazepines, gabapentinoids, antipsychotics, antidepressants, Z-drugs or opioids increase the NFO risk when co-prescribed with OAT. DESIGN: We conducted a cohort study using the Clinical Practice Research Datalink GOLD and Aurum databases. The cohort was linked to Hospital Episode Statistics admitted patient care data (HES-APC), neighbour

Implementation evaluation of a pharmacist prescribing service for the management of dermatological conditions: a study protocol.

BACKGROUND: The scope of practice for community pharmacists is evolving globally. Policy drivers are to increase access to primary care, due to the lack of accessibility to general medical practitioners or inappropriate demand on emergency departments. Most implementation studies in this space have reported on determinants for pharmacist prescribing services. There is a lack of comprehensive evaluations including investigation of the implementation process and outcomes. METHODS: A cohort design hybrid type 2 effectiveness-implementation study, applying mixed methods will be carried out between
