Differences between radiographic and non-radiographic axial spondyloarthritis patients in a Mexican cohort.

To compare the demographic, clinical, and laboratory characteristics, disease onset, and clinical features of radiographic axial Spondyloarthritis (r-axSpA) and non-radiographic axial Spondyloarthritis (nr-axSpA) patients. All patients who attended outpatient spondylarthritis (SpA) clinics at Hospital General de Mexico and the Instituto Nacional de la Nutrición from 1998 to 2005 and met the rheumatologist diagnostic criteria for SpA were selected. Then the SpA patients were classified by European Spondyloarthropathy Study Group criteria (ESSG). We selected SpA patients with axial presentation

Evidence for inflammation-induced bone loss in non-radiographic axial spondyloarthritis.

OBJECTIVES: Non-radiographic axial SpA (nr-axSpA) comprises those patients who may have clinical and laboratory features of SpA but do not have definite radiographic sacroiliitis. Bone loss (osteopenia or osteoporosis) is well recognized in AS, however, bone status in nr-axSpA is unclear. The aims of this study were to investigate BMD in patients with nr-axSpA and compare them with age- and sex-matched patients with mechanical low back pain (mLBP). The relationship between inflammation on MRI of the lumbar spine and BMD was also assessed in nr-axSpA. METHODS: Patients with chronic LBP were co

Chinese recommendations for the diagnosis and management of non-radiographic axial spondyloarthritis.

BACKGROUND: The term of non-radiographic axial spondyloarthritis (nr-axSpA) has stimulated a number of practice-changing developments, but the meaning of nr-axSpA has not been well understood by clinicians, including rheumatologists. The diagnosis of nr-axSpA remains clinically problematic, with substantial risks of overdiagnosis. OBJECTIVE: To generate a set of evidence-based recommendations for the diagnosis and management of patients with nr-axSpA for physicians, health professionals, and rheumatologists. METHODS: The recommendations were developed through a Delphi method prioritizing 37

The term 'non-radiographic axial spondyloarthritis' is much more important to classify than to diagnose patients with axial spondyloarthritis.

The term axial spondyloarthritis (axSpA) now is used frequently to describe patients with predominantly axial symptoms who fit into the spectrum of a well-recognised rheumatic disease that continues to be known as ankylosing spondylitis (AS). The 2009 Assessment of SpondyloArthritis international Society (ASAS) classification criteria, developed to identify patients with early or atypical disease which could not be classified by the 1984 modified New York (mNY) criteria for AS, have led to a differentiation between non-radiographic axial spondyloarthritis (nr-axSpA) and radiographic axSpA, whi

Challenges in diagnosing and monitoring radiographic and non-radiographic axial spondyloarthritis in daily clinical practice.

INTRODUCTION/OBJECTIVES: Axial spondyloarthritis (axSpA) is a heterogeneous disease with variable clinical features changing over the disease course, making diagnosis and monitoring challenging. Large progress has been made in classification development, imaging, disease-activity assessment. However, there is limited insight in how these developments are addressed in daily clinical practice. Therefore, our study aim was to gain better insight into diagnosis, monitoring and treatment and to explore if non-radiographic (nr-)axSpA is addressed differently from radiographic (r-)axSpA. METHODS: A

Is ultrasonographic enthesitis evaluation helpful for diagnosis of non-radiographic axial spondyloarthritis?

The aim of this study is to evaluate the diagnostic utility of ultrasonographic enthesitis assessment in patients with non-radiographic axial spondyloarthritis (nr-axSpA) and to compare different sonographic scoring methods. Patients with nr-axSpA (n = 30) and mechanical back pain (MBP) (n = 30) were enrolled in the study with standardized clinical criteria. For both of the groups, a total of 18 entheses were evaluated in each patient with B mode ultrasound and power Doppler by a sonographer who is blinded to initial clinical and radiological assessments. Glasgow Ultrasound Enthesitis Scoring

Extra-articular manifestations and burden of disease in patients with radiographic and non-radiographic axial spondyloarthritis.

OBJECTIVE: Although the prevalence of peripheral and extra-articular disease in ankylosing spondylitis (AS) has been assessed in many studies, data on non-radiographic axial spondyloarthritis (nr-axSpA) is scanty. The aim of this study was first, to compare radiographic-axSpA/AS (r-axSpA/AS) and nr-axSpA concerning peripheral arthritis and extra-articular manifestations (EAMs), and second, to assess potential differences between patient subgroups with or without EAMs regarding disease burden. METHODS: Data was extracted from our single center axSpA database. Patients having at least one of th

Non-Radiographic Axial Spondyloarthritis (nr-axSpA): Advances in Classification, Imaging and Therapy.

Non-radiographic axial spondyloarthritis (nr-axSpA) is a recently described form of axial inflammatory arthritis that has not caused substantial erosive damage to the sacroiliac joints. Nr-axSpA is associated with significant impairment in quality of life and, in a proportion of patients, it can evolve into ankylosing spondylitis (AS, also termed radiographic axSpA). The identification in the clinic of nr-axSpA has been made possible by advances in magnetic resonance imaging (MRI). Classification criteria for nr-axSpA have been proposed but there remains discussion in the international communi

[Guideline for the diagnosis and treatment of non-radiographic axial spondyloarthritis (2024 edition)].

Non-radiographic axial spondyloarthritis (nr-axSpA) is a new term of disease entity, which was introduced in 2009. Nearly ten targeted drugs have been approved for the indication of nr-axSpA abroad, and one drug has also been approved in China. However, in both China and other countries, many doctors, including rheumatologists, are not familiar with the accurate meaning of nr-axSpA. Overdiagnosis or misdiagnosis of nr-axSpA occurs occasionally in both clinical practice and clinical research. Therefore, Chinese Rheumatology Association and Committee for Rheumatologists and Immunologists of Chin

Australian Consensus Statements for the Assessment and Management of Non-radiographic Axial Spondyloarthritis.

BACKGROUND: The understanding of non-radiographic axial spondyloarthritis (nr-axSpA) has accelerated over the last decade, producing a number of practice-changing developments. Diagnosis is challenging. No diagnostic criteria exist, no single finding is diagnostic, and other causes of back pain may act as confounders. AIM: To update and expand the 2014 consensus statement on the investigation and management of non-radiographic axial spondyloarthritis (nr-axSpA). METHODS: We created search questions based on our previous statements and four new topics then searched the MEDLINE and Cochrane da

Non-radiographic axial spondyloarthritis and ankylosing spondylitis: what are the similarities and differences?

The development of the axial spondyloarthritis and ankylosing spondylitis (ASAS) classification criteria has had several implications for our understanding of the entire spectrum of spondyloarthritides (SpA). Going beyond the modified New York criteria, which concentrate on conventional radiographs of the sacroiliac joints (SIJ) for the classification of ankylosing spondylitis, the ASAS criteria add active inflammation of the SIJ as obtained by MRI and human leucocyte antigen (HLA) B27 to classify patients with chronic back pain starting at a young age as axial SpA (axSpA). AxSpA should be con

Characteristics and burden of disease in patients with radiographic versus non-radiographic axial spondyloarthritis in the ASRI cohort.

BACKGROUND: Axial spondyloarthritis (axSpA) comprises patients with both radiographic and non-radiographic features. Previous studies have shown similar burden of disease between these two groups. AIMS: The Ankylosing Spondylitis Registry of Ireland (ASRI) was formed with the objective to measure the burden of axial spondyloarthritis in the population and identify early predictors of a poor outcome. For this analysis, the ASRI database was used to compare the characteristics and burden of disease in patients with radiographic versus non-radiographic axial spondyloarthritis. METHODS: Patients
