The effect of nuchal cord on amniotic fluid and cord blood erythropoietin at delivery.

OBJECTIVE: We investigated the effect of a nuchal cord on fetal hypoxia by using amniotic fluid and cord blood erythropoietin as markers of chronic and acute hypoxia, respectively. METHODS: A total of 167 full-term pregnancies without maternal complications or fetal prelabor complications except fetal growth restriction of unknown cause were studied prospectively. Of these, 47 had a nuchal cord at delivery, and 62 had one or more complications during labor and delivery (nonreassuring fetal heart rate pattern, birth weight less than 2500 g, Apgar score at 1 minute less than 7, presence of meco

Perinatal features and umbilical cord blood gases in newborns complicated with nuchal cord.

Presence of nuchal cord (NC) is associated with transient decrease of umbilical cord blood flow. However, the exact perinatal effect of presence of NC in a newborn is still under debate. The aim of this study was to evaluate the perinatal complications and umbilical cord blood gases of deliveries complicated with NC and summarize the associated literature. Gestational age-matched term singleton pregnancies complicated with NC (n=160) were compared with neonates without NC (n=160). Patients' files and Labor and Delivery Unit database were used to extract maternal age, gestational age, presence

Effect of the Presence of Nuchal Cords on Vaginal Breech Labor.

OBJECTIVE: Breech delivery has been reported to be associated with a high incidence of abnormal fetal heart rate pattern leading to neonatal asphyxia. In this study, we examined the effect of the presence of nuchal cords on perinatal outcomes of vaginal breech labor. METHODS: We reviewed the obstetric records of all singleton breech labor cases at the Japanese Red Cross Katsushika Maternity Hospital between 1999 and 2011. Of the 266 breech labor cases in singleton pregnancies, the presence of nuchal cords was recognized in 50 cases (18.8%) on neonatal findings at birth. We compared the clinic

Lipid peroxidation in nuchal cord cases: implication for fetal distress.

OBJECTIVE: To examine whether nuchal cord affects fetal lipid peroxidation and cord arterial blood gases, and thus to determine whether lipid peroxidation can show intrapartum distress. METHODS: Pregnant women giving birth to a baby with nuchal cord (n = 32) formed the study group and others without this condition made up the control group (n = 36). The maternal malondialdehyde levels (MDA) before and after delivery as well as fetal umbilical cord MDA and arterial blood gases were measured in both the groups. RESULTS: Mean cord MDA level was higher in the study group (p < 0.02) and was signi

[Color Doppler flowmetry values in fetuses with nuchal cord encirclement].

In order to evaluate the presence of nuchal cord entanglement and to measure resistance index with Doppler color ultrasonography, a prospective, observational, cross-sectional study was carried out, 132 low risk term pregnant women were included, 50 of them had umbilical cord encirclement (37%), the sensitivity, specificity, positive predictive and negative predictive values were 92%, 87%, 81% and 95%, respectively. McNemar'test for discordance had a p = 0.121 in comparing ultrasonography result with the gold standard. Patients with nuchal cord entanglement had higher frequency of cesarean sec

A Rare Case of Double True Umbilical Cord Knots With a Nuchal Cord: A Case Report.

A double true umbilical cord knot (TUCK) is a rare complication of pregnancy that is often missed on ultrasonography. The stricture caused by TUCK can lead to occlusion of fetal circulation, fetal asphyxia, and subsequent death. Despite these risks, there is a lack of evidence and no specific consensus on both antepartum and intrapartum management of TUCK. We report a rare case of a healthy male neonate with a double TUCK and nuchal cord delivered vaginally with a vacuum assist. A 26-year-old female, G2P1001, at 39 weeks of gestation, presented to the labor and delivery unit for induction of l

Sextuple Nuchal Cord Entanglement in a Preterm Neonate: A Case Report.

BACKGROUND: Nuchal cord, or wrapping of the umbilical cord around the fetal neck, is a common obstetric finding; however, multiple loops are uncommon, and entanglement exceeding four loops is rare. High-order nuchal cords (≥ 5 loops) may predispose to cord compression, abnormal fetal heart rate tracings and intrauterine compromise. Advances in prenatal ultrasonography, particularly colour Doppler imaging, have improved detection, but the optimal management of these cases remains uncertain. KEY FINDINGS: We present a case of a 42-year-old gravida 6, para 3124 woman (three term deliveries, one

Does nuchal cord at birth increase the risk for cerebral palsy?

BACKGROUND: Nuchal cord is a common finding in pregnancy. It is unclear whether a nuchal cord at birth causes birth asphyxia and raises the risk for developing cerebral palsy of the offspring. AIM: To evaluate the incidence of cerebral palsy in children born with and without nuchal cord. STUDY DESIGN: A population-based cohort analysis including all singleton deliveries over >20 years at a single tertiary medical center was conducted. The incidence of cerebral palsy in children up to 18 years of age was evaluated. Kaplan-Meier survival curve was used to compare cumulative incidence between t

Significance of intrapartum amniotic fluid volume in the presence of nuchal cords.

Among 70 women delivering infants with nuchal cords, there was a significantly higher incidence of meconium passage and severe variable fetal heart rate decelerations/fetal bradycardia in those who had intrapartum oligohydramnios, as defined by an amniotic fluid index less than or equal to 5.0 cm. Oligohydramnios in the presence of nuchal cord entanglement might represent an increased risk of ominous intrapartum fetal heart rate patterns.

Cardiac Adaptation and Preferential Streaming in Fetuses with Multiple Nuchal Cords.

The echocardiographic monitoring of a fetus with multiple nuchal cords around the fetal neck is important as it may result in cardiac remodeling and preferential streaming, thus affecting the condition of the fetus. The main aim of our study was to assess whether the collision of the umbilical cord around the fetal neck can lead to discrepancies in the size of the pulmonary artery and the aorta in the three-vessel view and to an increase in the size of the heart, which may result from the compression of the carotid arteries caused by the umbilical cord wrapping around the fetal neck. A total n

Nuchal cord evident at birth impacts on fetal size relative to that of the placenta.

We sought to determine whether umbilical cord complications, as evidenced by a nuchal cord at the time of birth, affects birth weight, placental weight and, therefore, the birth to placental weight ratio as a mechanism whereby fetal and placental growth may be differentially affected. The computerized perinatal database of St. Joseph's Health Centre, London, Ontario, was used to obtain the birth weight, placental weight, umbilical cord gases, and nuchal cord status, for all term singleton liveborn infants between January, 1991 and December, 1994. The effect of no nuchal cord versus nuchal cord

Color ultrasonography: a useful technique in the identification of nuchal cord during labor.

OBJECTIVE: To compare the accuracy of intrapartum ultrasound with and without color Doppler for identification of nuchal cord displacement during labor. METHODS: 180 normal pregnant women, admitted in labor, or for induction to labor, were examined independently by two researchers, using either conventional real-time gray-scale imaging or color Doppler imaging. A repeat examination was performed by the other researcher using the other ultrasound modality. Nuchal cord displacement was classified as either negative, definite or suspicious. Nuchal cord at birth was classified as either tight or
