BACKGROUND: Nursing homes (NH) are less well studied than hospices or hospitals as a setting for terminal care. The aim of this study is to identify the direct causes and underlying diseases of the terminal phase in Dutch nursing homes.
METHODS: A prospective study of terminally ill patients with a maximum life-expectancy of (less than) 6 weeks in 16 NHs in the Netherlands. 544 long-term care patients were enrolled in the study.
RESULTS: The terminal phase was marked with symptoms of low fluid and food intake, general weakness and respiratory problems/dyspnea. Direct causes of these conditio

For some persons, being dependent on a ventilator is the only alternative to death. Most residents residing in New York state's approved ventilator-dependent beds have needed the support of a ventilator for more than a year. The quality of their lives since admission to specialized programs has been significantly different than it had been in hospitals. These residents are now actively participating in the life of the facility. Many who had not spoken during the entire time they were hospitalized are now speaking with the aid of special tracheostomy tubes. Many have been assisted out of bed fo

Two psychiatrists, using standardized clinical examinations, found that 21% of 125 patients newly admitted to nursing homes had delusions. The psychiatric disorders associated with these delusions are described. Delusional patients were more behaviorally disturbed before admission than nondelusional patients. These behavior problems were often the reason for admission, and they persisted after admission. Nursing home staff members infrequently identified and often inappropriately treated delusional patients. Since delusions are an important potentially treatable cause of behavior problems in n

OBJECTIVES: Our aim was to clarify if persons living with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) have adequate economic access to antiretroviral therapy (ART) when admitted to nursing homes (NHs). Medicare Part A pays NHs a bundled skilled nursing rate that includes prescription drugs for up to 100 days, after which individuals are responsible for the costs.
DESIGN: A cross-sectional study.
SETTING: NHs.
PARTICIPANTS: A total of 694 newly admitted long-stay (>100 d) NH residents with HIV.
MEASUREMENTS: We used Minimum Dataset v.3.0, pharmacy dispensing d

PURPOSE: To estimate the proportion of residents newly initiating long-acting opioids in comparison to residents initiating short-acting opioids and examine variation in long-acting opioid initiation by region and resident characteristics.
METHODS: This cross-sectional study included 182 735 long-stay nursing home residents in 13 881 US nursing homes who were Medicare beneficiaries during 2011 to 2013 and initiated a short-acting or long-acting opioid (excluding residents <50 years old, those with cancer, or receiving hospice care). Previous studies have demonstrated that sleep-related problems are common in the aging process. Such problems are greater in nursing home residents than other elderly people. This study had the following objectives: (1) establish the clinical profile of nursing home residents according to their quality of sleep and intake of hypnotic medication, (2) assess participants' functionality in relation with their quality of sleep and (3) correlate participants' functional status with their quality of sleep. The study sample was composed of 334 nursing home residents. Results showed a high percentage

BACKGROUND: Given the potential adverse effects of antipsychotics, high use in nursing homes creates concern. Our study goal was to explore the use of antipsychotics in relation to resident characteristics, and to assess the appropriateness of antipsychotic prescribing in Belgian nursing homes.
METHODS: Data were used from a cross-sectional study (Prescribing in Homes for the Elderly; PHEBE) conducted in 76 nursing homes in Belgium. Antipsychotics were classified into typical and atypical, using the anatomical therapeutic and chemical classification. Ten inappropriate antipsychotic prescribin

Although research on domestic elder abuse and neglect has grown over the past 20 years, there is limited research on elder neglect in nursing homes. The purpose of this study is to estimate the incidence of elder neglect in nursing homes and identify the individual and contextual risks associated with elder neglect. Data came from a 2005 random digit dial survey of individuals in Michigan who had relatives in long term care. Our analytic sample included 414 family members who had a relative aged 65 or older in nursing homes. Results showed that about 21% of nursing home residents were neglecte

A postal questionnaire survey was undertaken in registered nursing homes in three different health districts in England: Gloucestershire, North Staffordshire and Leeds. Nursing homes may be registered as general nursing or mental health homes. If homes also have provision for residential beds these are defined as dual registered homes. Overall, 9% (438/4900) of residents, with an equal male:female split, had urinary catheters. There was no significant difference in the overall urinary catheterization rate in the three districts (P=0.9). There was a wide range of urinary catheterization prevale

BACKGROUND: Use of atypical antipsychotics for "off-label" indications, such as behavioral and psychological symptoms of dementia, depression, and bipolar disorder, have been frequently reported, although not systematically studied. We describe the pattern of atypical antipsychotic use among nursing home residents and identify demographic and clinical correlates.
METHOD: We conducted a cross-sectional study on 139,714 nursing home residents living in 1732 nursing homes in 5 U.S. states from Jan. 1, 1999, to Jan. 31, 2000. Data were obtained from the computerized Minimum Data Set (MDS) assessm

1. Many people, including health care professionals, believe that hearing loss is a normal part of aging and that little can be done to improve older adults' hearing. If the staff and physicians believe that hearing loss among nursing home residents is expected due to advanced age, then the likelihood of further evaluation of hearing impairments is minimal. 2. Hearing loss is a serious health problem among the elderly. Social isolation, depression, acting out inappropriately, and paranoia have been related to impaired hearing. Cerumen impaction is a reversible, frequently overlooked cause of a

The prescribing patterns for hypnotic medications were surveyed in 765 patients of three skilled-nursing facilities and two intermediate-care facilities. Seven per cent of the patients received a hypnotic routinely; an additional 3 per cent had as-needed orders for a hypnotic medication. Temazepam, flurazepam, and triazolam were, in descending order, the three most commonly prescribed hypnotics and accounted for 79 per cent of the hypnotic prescriptions. The average duration of use for triazolam, temazepam, and flurazepam was 11 weeks, 24 weeks, and 82 weeks, respectively. Seventy-six per cent

Two defining features of the nursing home industry are the tremendous churn in chain ownership and the perception of low-quality care at many facilities. We examined whether nursing homes that underwent chain-related transactions, such as mergers and acquisitions, experienced a larger number of health deficiency citations than nursing homes that maintained common ownership over the same period. Using facility-level data for the period 1993-2010, we found that those nursing homes that underwent chain-related transactions had more deficiency citations in the years preceding and following a trans

This study estimates the effect of nursing home closure on occupancy, net profit margin, and operating margin of nearby nursing homes. We use national nursing home data from 2009 to 2019 from Medicare cost reports, Medicare Provider of Services (POS), and LTCfocus.org data. Using the Callaway and Sant'Anna difference-in-differences model, we compare the changes in occupancy, net profit margin, and operating margin between incumbent nursing homes in markets with any closure and nursing homes in markets without a closure, overall, and across rurality. Our findings suggest that nursing home closu

In recent times the need for physical care of residents of Dutch old people's homes has clearly increased. This is strongly related to the ageing of the population of these homes as well as to the introduction of legal measures concerning the admission of new residents. Staff members have also noted an increase in the mental and social problems of residents. In this article we describe a rating scale devised for the assessment of psychosocial problems in a home for the aged. The staff completed this scale and an ADL scale four times at three-month intervals. The prevailing problems appear to b

Quality of care has been a long-standing issue in US nursing homes. The culture change movement attempts to transition nursing homes from health care institutions to person-centered homes. While the adoption of culture change has been spreading across nursing homes, barriers to adoption persist. Nursing homes that disproportionately serve minority residents may have additional challenges implementing culture change compared with other facilities due to limited financial and staffing resources. The objective of this study was to examine how nursing home characteristics are associated with cultu

BACKGROUND: Polypharmacy and the use of potentially inappropriate medications are common among nursing home residents and are associated with negative outcomes. Although deprescribing has been proposed as a way to curtail these problems, the best way to implement multidisciplinary comprehensive medication review and deprescribing and its real impact in specific high-risk populations, such as nursing home residents, is still unclear. This multicenter randomized controlled clinical trial aims to assess the effects of a multidisciplinary mediation management program on medication use and health p

BACKGROUND AND OBJECTIVE: The persistent shortage of direct care workers across the long-term services and supports system has worsened since the onset of the COVID-19 pandemic. This shortage resulted in nursing homes (NHs) relying more heavily on agency staff to maintain a stable workforce. This study examines patterns of agency staff utilization and associated concerns reported by NH administrators.
RESEARCH DESIGN AND METHODS: Data from the 2023 Ohio Biennial Survey of Long-Term Care Facilities were used to report utilization patterns and concerns related to the use of agency-provided cert

OBJECTIVES: To evaluate current practices regarding palliative sedation in nursing homes. Despite being an essential component of palliative care, empirical data on its use are currently lacking. Understanding these practices can help improve care quality and support evidence-based policy development for end-of-life care.
