Nutrition as Therapy in Liver Disease.

PURPOSE: The importance of nutrition is often underrecognized in the routine clinical care of patients with chronic liver disease. Nutrition therapy plays a significant role in the management of alcohol-related liver disease and nonalcoholic fatty liver disease. In patients with cirrhosis from any etiology, malnutrition and sarcopenia are directly related to mortality, and nutritional interventions play an important role in the management of these patients. This review explores the role of nutritional intervention as adjuvant therapy across all chronic liver disease. METHODS: A narrative, qua

[Chronic pancreatitis: nutrition and pain therapy].

Therapy of chronic pancreatitis rests on five arms: Avoidance of alcohol, treatment of pain, replacement therapy for exocrine and endocrine insufficiency and adequate nutrition. Alcohol withdrawal improves pain and the patient's compliance. It also seems to retard the chronic inflammatory process. Therapy of pain depends on the pathomechanism of pain. There is a lack of prospective, controlled studies comparing various treatment regimens. Thus, treatment options are partly dependent on the experience of the physician taking care of the patient and include i.e. for pseudocysts: surgical vs perc

Nutrition therapy for the cancer patient.

Malnutrition commonly occurs in cancer and adversely affects the quality of life and survival of patients. It is caused by a variety of factors, including decreased food intake, adverse effects from anticancer treatment, and wasteful metabolic processes. Over the past 2 decades, there have been major advances in methods and techniques of feeding patients with cancer and other disease. Special diets can be helpful in the treatment of a variety of conditions, some of which are outlined in Table 1. Enteral feeding is developing rapidly because endoscopic techniques have made it simpler to place f

Infection control during parenteral nutrition therapy.

Parenteral nutrition therapy can achieve an anabolic state in patients who are unable to maintain normal nitrogen balance; however, it may be associated with infectious complications. Infections may be related to contamination of the cannula and the cannula wound, of the infusate, or of other parts of the parenteral nutrition system. A variety of microorganisms has been associated with these infections. The exact mechanisms that initiate cannula-related infection are poorly understood. Susceptibility of the host, the method and site of cannula insertion, colonization of parenteral nutrition ca

Aggressive nutrition therapy in malnutrition and sarcopenia.

Aggressive nutrition therapy is essential to improve nutrition and function in patients with malnutrition and sarcopenia. Malnutrition and sarcopenia negatively affect functional recovery and activities of daily living. Nutrition improvement is associated with better functional recovery. Target energy intake in aggressive nutrition therapy is defined as total energy expenditure (TEE) plus the amount of energy accumulated. The amount of energy accumulation per 1 kg of body weight is generally 7500 kcal. If the goal is to gain 1 kg of weight over 30 d, TEE + 250 kcal is the target daily energy i

The future of critical care nutrition therapy.

At present, we are in a "revival" period in clinical nutrition in critical care, especially in the area of "pharmaconutrition." Adequate nutrition may hinge not only on how many calories are provided but also on the ability to provide key pharmacologically acting nutrients. Traditionally, nutrition has been viewed as vital for metabolism, growth, and repair. But, it is now known that some nutrients, when given in therapeutic doses, appear to serve as pharmacologic agents to improve clinically relevant outcomes. Thus, larger therapeutic doses of specific nutrients may be required to replace acu

[Dysphagia - epidemiology, diagnostics, therapy and nutrition-management].

BACKGROUND: Swallowing is a complex neuro-muscular process at which around 50 muscle couples need to be coordinated. The swallowing process is controlled by the swallowing centres in cortex and brainstem as well as by five brain nerves. EPIDEMIOLOGY: Severe dysphagia appears most frequently in the context of neurological illnesses. Particularly in patients with acute stroke, swallowing disorders are reported in up to 50% of cases. DIAGNOSTICS: The main emphases must be put on the case history, a swallowing specific physical examination, a "Swallowing Screening Test" and the use of technical

Nutrition therapy for hepatic glycogen storage diseases.

Hepatic glycogen storage diseases (GSD) are a group of rare genetic disorders in which glycogen cannot be metabolized to glucose in the liver because of one of a number of possible enzyme deficiencies along the glycogenolytic pathway. Patients with GSD are usually diagnosed in infancy or early childhood with hypoglycemia, hepatomegaly, poor physical growth, and a deranged biochemical profile. Dietary therapies have been devised to use the available alternative metabolic pathways to compensate for disturbed glycogenolysis in GSD I (glucose-6-phosphatase deficiency), GSD III (debrancher enzyme d

[Palliative therapy in cancer 2. Nutrition control].

Deterioration in nutritional status occurs late in the progress of cancers at certain sites, but at all stages in patients with gastrointestinal cancer. Weight loss with decrease in body fat and muscle wastage, occurs to a varying degree. Superficially, the clinical condition resembles simple food deprivation. However, the derangements in metabolism are often and some patients show an elevated resting energy expenditure, disturbances of carbohydrate, fat and protein metabolism and generally, a failure to adapt to reduced food intake, which is characteristic of cachexia. Cancer cachexia then be

Tailoring nutrition therapy to illness and recovery.

Without doubt, in medicine as in life, one size does not fit all. We do not administer the same drug or dose to every patient at all times, so why then would we live under the illusion that we should give the same nutrition at all times in the continuum of critical illness? We have long lived under the assumption that critical illness and trauma lead to a consistent early increase in metabolic/caloric need, the so-called "hypermetabolism" of critical illness. What if this is incorrect? Recent data indicate that early underfeeding of calories (trophic feeding) may have benefits and may require

Acne: the role of medical nutrition therapy.

Acne is a common disease in Westernized nations, particularly among adolescents and young adults. Acne has substantial effects on quality of life, making treatment essential. Medical nutrition therapy as a potential treatment for acne is not new, although the literature examining diet and acne during the past 100 years is mixed. During the late 1800s and early 1900s, diet was commonly used as an adjunct treatment for acne. During the 1960s, however, the diet-acne connection fell out of favor. In recent years, dermatologists and registered dietitians have revisited the idea and become increasin

[Review of our home nutrition therapy cases].

The provision of home care service, 20 years since it was established, is becoming more important. The aging population is now at its highest ever level, and the number of patients in need of nutrition therapy is increasing. We have provided a home care service since 1996, mainly for the provision of home palliative care. Home care service has been provided to 168 patients, about 90% of which suffered from malignant disease, and about 80% of the malignant disease patients were in the terminal stage. The patients included 151 on home parenteral nutrition(HPN)and 7 on home enteral nutrition(HEN)

Nutritional therapy for hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is the most frequent primary liver cancer and presents together with cirrhosis in most cases. In addition to commonly recognized risk factors for HCC development, such as hepatitis B virus/hepatitis C virus infection, age and alcohol/tobacco consumption, there are nutritional risk factors also related to HCC development including high intake of saturated fats derived from red meat, type of cooking (generation of heterocyclic amines) and contamination of foods with aflatoxins. On the contrary, protective nutritional factors include diets rich in fiber, fruits and

[Nutritional therapy in chronic hepatopathy].

Severe protein-energy malnutrition, a common complication in patients with advanced liver disease, negatively influences clinical evolution and survival, while appropriate nutritional intervention has been found to improve liver function and prognosis. Although the importance of nutritional factors has been greatly emphasized in the last decade, nutritional therapy remains a controversial issue in clinical hepatology. Because a considerable heterogeneity exists among patients regarding both nutritional needs and the pathophysiology of malnutrition, adequate assessment of nutritional status is

Nutritional Therapy in Gastrointestinal Cancers.

Malnutrition is the most frequent nutritional disorder in patients with gastrointestinal cancer and is associated with cachexia syndrome, worsening of prognosis, and shortened survival rate. Early nutrition screening, assessment, and intervention are able to favorably modify the clinical evolution of affected patients. The adequate provision of nutritional requirements has been associated with improvement of immunologic status, and avoidance of further complications related to poor nutritional status, surgical treatment, and anticancer therapy. In malnourished patients, the supplementation of

Nutritional therapies for mental disorders.

According to the Diagnostic and Statistical Manual of Mental Disorders, 4 out of the 10 leading causes of disability in the US and other developed countries are mental disorders. Major depression, bipolar disorder, schizophrenia, and obsessive compulsive disorder (OCD) are among the most common mental disorders that currently plague numerous countries and have varying incidence rates from 26 percent in America to 4 percent in China. Though some of this difference may be attributable to the manner in which individual healthcare providers diagnose mental disorders, this noticeable distribution c
