Patients of the Department of Psychiatry of the Municipal Hospital Centre in Duisburg-Rheinhausen, Germany, are offered work therapy mainly by the nearby Sociopsychiatric Centre "Cuckoo's Nest". The possibilities of working and establishing contacts are described in detail after a brief review of the development and the advantage of shifting work therapy to an external, non-clinical site. In Lithuania, the stroke is not only medical, but social issue as well, since only 20% of patients suffering from stroke remain active at work. Yearly stroke incidence in Lithuania is 7000-8000 cases. The most common outcome of stroke is unilateral paralysis (hemiplegia) followed by disorders of coordination, balance, and movements. The number of persons suffering from chronic mental disease but living outside the psychiatric hospital in the community, is rising, resulting in an increasing demand for facilities for occupation, occupational therapy and rehabilitation and for paid employment. The past few years have seen a distinct increase in the number of sheltered workplaces for the mentally ill. This development has also led to qualitative differentiation in the offers. In the region of Westphalia there are currently some 940 work places in special departments for the mentally handicapped at workshops for the disabled.
