BACKGROUND: Esophageal carcinoma is predominantly a disease of the elderly, a group often only considered for palliative therapies.
METHODS: A case note review identified 31 octogenarians undergoing resection for carcinoma of the esophagus or gastric cardia over a 12-year period ending December 1994.
RESULTS: Nineteen patients made either an uncomplicated postoperative recovery (n = 12) or suffered minor complications (n = 7). Of the 12 patients who suffered moderate or severe complications, 5 died (in-hospital mortality, 16%). The deaths included 2 of 3 patients who underwent emergency oper

The records of 52 patients aged 80 years or older who underwent 56 carotid artery reconstructions were analyzed retrospectively. Four patients had amaurosis fugax, 27 patients had experienced one or more transient ischemic attacks, eight had a completely or partially reversible stroke, and 10 had vertebrobasilar insufficiency. Three patients were asymptomatic. Arteriograms documented stenosis > 80% on the operated side in 48 cases, whereas the contralateral carotid artery was occluded or had > 80% stenosis in 10 cases each. Two or more cerebral arteries were involved in 37 patients. CT scans w

Cholecystectomy is the standard treatment for symptomatic gallstone or acute cholecystitis, and a growing number of elderly patients are undergoing resection. The aim of this study is to evaluate the clinical outcome of cholecystectomy in elderly patients. We retrospectively reviewed the medical records of 337 patients with symptomatic gallstone or acute cholecystitis who underwent cholecystectomies between January 2011 and June 2013. Perioperative data were compared between octogenarians and younger patients. A subgroup undergoing cholecystectomy for acute cholecystitis (n = 146, 43.3 %) was

BACKGROUND AND AIMS: As our population ages and life expectancy increases the number of people aged over 80 and more referred for cardiac surgery is growing. This study sought to identify the outcome of aortic valve replacement (AVR) in octogenarians.
METHODS: 68 patients aged 80 years or more underwent AVR at the Freeman Hospital, between April 2001 and April 2004. A retrospective review of the notes and outcomes from the patients' GP and the NHS strategic tracking service was performed. 54% (37) underwent isolated AVR whilst 46% (31) underwent combined AVR and CABG.
RESULTS: Follow up was

A significant percentage of patients in need of a permanent pacemaker are older than 80 years. The implantation policy may be determined either by the patient's physical activity or by chronologic age. The trend in pacemaker implantation in patients over 80 during the last 10 years in our institution was evaluated and compared with the trend in the patients younger than 80 at the time of implantation. Of 519 patients who had primary pacemaker implantation, 152 (29%) were older than 80 at the time of the procedure. Another 189 patients had second implantation procedures, and 80% of them were ol

During a 10-year period from January 1983 to December 1992, 79 carotid endarterectomies were performed in patients aged 80 years or older. This represented 7.4% of the total patient population undergoing carotid endarterectomy at Emory University Hospital. The indications for surgery in this elderly population were transient ischemic attacks in 24 (30.3%), cerebrovascular accident in 12 (15.2%), amaurosis fugax in seven (8.9%), vascular tinnitus in one (1.3%), and asymptomatic stenosis in 35 (44.3%). The average degree of ipsilateral stenosis was 76.8%. Concomitant risk factors included corona

Seventy-six consecutive patients, aged 80 to 89 (mean 82), underwent cardiac operations with cardiopulmonary bypass. Hypothermia (22 degrees C) and hyperkalemic cardioplegia were used in each. There were 35 men and 41 women. Thirteen patients (17%) were in New York Heart Association Functional Class III and 62 patients (81%) were in Class IV preoperatively. Coronary bypass procedures (Group I) were performed in 38 patients, of whom five had combined carotid endarterectomy. The average number of grafts was 3.7 per patient. There were two early deaths (5.2%). Single or double valve replacement,

BackgroundMajor lower extremity amputations, defined as above-ankle amputations, are morbid and significantly life-changing. Older patients may be more susceptible to poor outcomes. Our goal was to assess the short- and long-term outcomes of major amputations in octogenarians.MethodsA retrospective single-center chart review was performed on all octogenarian patients who underwent a major amputation at a safety-net hospital between 2014-2023. Demographics, operative characteristics, postoperative outcomes, and 30-day and 1-year outcomes were collected and analyzed. Multivariable binary logisti

BACKGROUND: Implantable cardiac defibrillators (ICDs) are essential for preventing sudden cardiac death. Despite inclusion in national guidelines, older adults are often underrepresented in trials. Evaluating ICD use in the aging population, particularly with advancements in heart failure treatment, is crucial.
OBJECTIVE: This study examines outcomes in octogenarians undergoing ICD implantation for primary and secondary prevention, focusing on ICD therapies and mortality timing.
METHODS: A retrospective observational study at a single Canadian academic center included patients ≥80 years old

OBJECTIVE: Abdominal aortic aneurysm (AAA) repair is recommended for aneurysms greater than 5.5 cm in men and 5 cm in women. Because AAA is more common among the elderly, we sought to evaluate contemporary practices of elective AAA repair and 2-year postoperative outcomes in octogenarians.
METHODS: We identified octogenarians undergoing elective AAA repair in the Vascular Quality Initiative from 2012 to 2019. We included patients undergoing endovascular (EVAR) and open (OAR) aortic repair. Demographics and comorbid conditions were compared between patient groups. Frailty was calculated using

BACKGROUND: Limited data are available on the outcome of primary percutaneous coronary intervention (PCI) in octogenarian patients, as the elderly are under-represented in randomized trials. This study aims to provide insights on clinical characteristics, management and outcome of the elderly and very elderly presenting with STEMI.
METHODS: 2225 STEMI patients ≥70years old (mean age 76.8±5.1years and 53.8% men) were admitted into the network of the ISACS-TC registry. Of these patients, 72.8% were ≥70 to 79years old (elderly) and 27.2% were ≥80years old (very-elderly). The primary end-point wa

BACKGROUND AND OBJECTIVES: Extremely elderly patients usually present with complicated gallstone disease and are less likely to undergo definitive treatment. The purpose of this study was to evaluate the results of laparoscopic cholecystectomy in octogenarians, with an interest in patients presenting initially with complicated gallstone disease and pancreatitis who underwent laparoscopic cholecystectomy during the same hospitalization.
METHODS: Data for 42 patients > or = 80 years who underwent an elective laparoscopic cholecystectomy between January 2007 and August 2011 were retrospectively

Stroke is the third leading cause of death in the United States, after coronary heart disease and cancer. Many survivors are left with mental and physical impairment and require assistance with activities of daily living. Twenty-eight percent of patients with stroke are under 65 years of age. We analyzed retrospectively 213 elderly patients with the diagnosis of ischemic cerebrovascular event that were consecutively admitted to the stroke unit of our institution between January 2002 and December 2004. Patient age varied from 65 to 100 years (mean = 79.8 years), and there were 66.19% females an

Preoperative profiles, postoperative complications, and the early and late results in 32 patients 80 yrs. of age and older (elderly group) who underwent coronary artery bypass grafting were compared with those in patients under 80 yrs. of age (control group). In the elderly group, the prevalence of patients with preoperative creatinine clearance (Ccr.) <50 l/day (34.4%), unstable angina pectoris (78.1%) and left main trunk disease (40.1%) was significantly higher than those in the control group. The incidences of arrhythmia and intensive care unit(ICU) syndrome were also significantly higher i

BACKGROUND: Aortic valve replacement (AVR) is considered one of the most potent disease-modifying procedures among patients with severe aortic stenosis (sAS). Accordingly, we have witnessed a consistent increase in the procedure rates in recent years. Nevertheless, the elderly population, particularly octogenarians, remains relatively undertreated. The current study aims to document the disparities in AVR rates among octogenarians and its prognostic significance.
METHODS: A vast database of Maccabi Health Services, the second largest health maintenance organisation in Israel, counting nearly

OBJECTIVES: Robotic-enhanced minimally invasive direct coronary artery bypass grafting surgery (RE-MIDCAB) is based on the use of a robotic console and instrumentation for the dissection of the left internal thoracic artery (LITA). The LITA to left anterior descending (LAD) artery anastomosis is subsequently constructed through a mini thoracotomy. The purpose of this study is to present our experience of RE-MIDCAB outcomes in elderly patients.
METHODS: From 2002 until 2015, 44 octogenarians (the mean age of 82.9 years) underwent RE-MIDCAB. The mean logistic EuroSCORE was 9.2. The majority of

BACKGROUND AND PURPOSE: Radical cystectomy is the standard of care for muscle-invasive bladder cancer; however, few patients over age 75 undergo cystectomy. Morbidity and mortality rates in this age group approach 60% and 10%, respectively. We sought to determine if minimally invasive surgery, in the form of robot-assisted radical cystectomy (RARC), may reduce morbidity and mortality associated with this operation in elderly patients.
PATIENTS AND METHODS: After Institutional Review Board approval, all RARC performed between 2009 and 2012 from one institution were reviewed, and 23 cases in pa

OBJECTIVE: The utilization of endovascular abdominal aortic aneurysm repair (EVAR) in suitable patients has resulted in decreased perioperative morbidity and mortality. Octogenarians as a subgroup have been more readily offered EVAR, as it is less invasive, and therefore presumably better tolerated than conventional open aortic repair. The purpose of this study is to investigate periprocedural and late EVAR outcomes in octogenarians compared with patients less than 80 years of age.
METHODS: From January 2003 to May 2008, 322 patients underwent EVAR. A total of 117 octogenarians were compared
