There is substantial evidence that prostaglandins (PG's) mediate acute elevations of intraocular pressure (IOP) due to laser irradiation of the iris. In the present study, we determined the optimal laser parameters needed to produce the maximal rise in IOP in anesthetized rabbits. Application of argon laser (0.75 watts, 0.5 sec. duration, and 8 spots of 500 micron size) to the iris of pigmented rabbits elicited an acute rise in IOP and miosis accompanied by an increase in aqueous protein concentration. This response was followed by a delayed ocular hypotension that lasted for more than 3 days

The exact function of the sympathetic nervous system in the regulation of intraocular pressure remains unclear. Many observers have noted reduced intraocular pressure in eyes whose sympathetic supply has been interrupted. A case of ocular sympathetic denervation associated with ocular hypertension is reported. Reports on the relationship between intraocular pressure and sympathetic denervation are reviewed and their relevance to this case discussed.

PURPOSE: To estimate the risk of blindness in patients with ocular hypertension (OHT) using an appropriate model and current empirical data.
DESIGN: A Markov model with data from a systematic literature review.
METHODS: A Markov model with 3 health states was built: OHT, primary open-angle glaucoma (POAG), and unilateral blindness. Literature was searched for reports on conversion from OHT to POAG and progression from POAG to blindness, to estimate a range of annual conversion and progression probabilities. The model had a cycle length of 1 year.
RESULTS: The 15-year risk estimates ranged f

Should patients with ocular hypertension be treated to prevent glaucomatous visual field loss? Three considerations suggest that for most patients the answer is no. First, the average risk of visual field loss in untreated ocular hypertension is small. Population surveys and prospective studies indicate that no more than about one of every nine persons with intraocular pressures higher than 20 mm Hg will develop a visual field defect. Secondly, treatment of ocular hypertension to lower intraocular pressure is unproven efficacy in preventing visual field defects. Thirdly, adverse reactions to g

Steroid induced ocular hypertension and glaucoma represent iatrogenic changes of pharmacogenic nature. They are mainly due to exogenous steroids following ocular periocular, intravitreal and systemic administration. Elevated ocular pressure is brought about by structural trabecular changes as well as obstruction of the outflow ways of the aqueous humor localized within the trabecular juxtacanalicular area. Although mostly raised ocular pressure spontaneously descends to basal values after ceasing the steroid therapy, progressive optic nerve damages and glaucomatous visual field defects may occ

The authors review the risk factors that influence conversion of ocular hypertension into primary open-angle glaucoma. They are represented mainly by advanced age and higher intraocular pressure, central corneal thickness as well as greater values of vertical and horizontal cup/disc ratios. Therapy must be immediately performed if there is a moderate or high conversion risk (e.g. elderly age, greater intraocular pressure, central corneal thinnness, greater cup/disc ratio) (10% of cases). Should these factors be missing (more than 90% of the patients) then the treatment is not necessary, the on

PURPOSE: To study the optic disc size in eyes with ocular hypertension (OHT) in comparison to primary open-angle glaucoma (POAG) and normals.
METHODS: Optic disc photographs obtained with the Nidek 3dx NM camera were digitized (Nikon coolscan) and disc area calculated using Littmann correction in a randomly chosen eye of 28 OHT, 42 POAG and 30 normal subjects. OHT was defined as increased intraocular pressure with no disc or field changes suggestive of glaucoma with open angles.
RESULTS: The optic disc area in OHT was 9.47 +/- 1.09 mm2; 12.27 +/- 2.87 mm2 in POAG; and 12.11 +/- 2.83 mm2 in n

BACKGROUND: The relation between Goldmann applanation tonometry and central corneal thickness (CCT) was evaluated in several studies during the last thirty years. Patients with ocular hypertension were found to have a significantly higher CCT compared with normals and glaucomas.
PATIENTS: To report on two sisters with elevated CCT and ocular hypertension diagnosed by raised intraocular-pressure (IOP) readings on Goldmann applanation tonometry.
RESULTS: In both patients, there were no signs for early functional or morphological glaucomatous changes. Maximum IOP was 30 mm Hg and 26 mm Hg. The

The intraocular pressure (IOP) course during the first postoperative month was measured with Goldmann applanation tonometry in 93 eyes. The patients include 30 eyes of intracapsular cataract extraction (ICCE), 32 eyes of extracapsular cataract extraction (ECCE) and 31 eyes of ECCE combined with posterior chamber intraocular lens (PC-IOL) implantation. The preoperative average IOP was 13.30 +/- 3.24 mmHg in 93 eyes. The first day after cataract extraction was 25.33 +/- 8.18 mmHg and 59 eyes (63.44%) had an IOP higher than 23 mmHg. On the second day it was 16.03 +/- 7.54 mmHg. On the third day i

PURPOSE: To assess the cost-effectiveness of treating ocular hypertension (OHT) in the United States.
DESIGN: A Markov model was constructed to perform a cost-effectiveness analysis.
PARTICIPANTS: Patients with OHT.
METHODS: The health states considered were stable OHT and glaucoma. Practice patterns for the model were derived from the Ocular Hypertension Treatment Study (OHTS), and transition probabilities were derived from previous literature. Specific unit costs used for medications, patient visits, and diagnostic and therapeutic procedures were obtained from Blue Cross/Blue Shield. The

Reductions in flicker sensitivity in ocular hypertension are thought to precede manifest glaucomatous damage, but the proportion of patients with ocular hypertension reported to have losses in flicker sensitivity (50-90%) is far out of step with the proportion of ocular hypertensive patients in whom clinically defined glaucoma will develop (5-30%). The authors examined the possibility that the flicker losses in some of these patients represent not early glaucomatous damage, but instead a transient influence of raised intraocular pressure (IOP) on an otherwise normal eye. Temporal contrast sens

A retrospective case-control study compared 100 ocular hypertensive patients with 100 ocular normotensive patients matched for sex and age. Interviews were conducted and charts were reviewed to explore demographic, personal, and environmental factors, as well as medical history and refractive error. Multiple logistic regression analyses assessed which factors were independently related to the presence or absence of ocular hypertension. The variables significantly associated with the presence of ocular hypertension were family history of glaucoma, myopic refractive error, absence of liquor inta

Measurements were made of the percent areas of optic disc cupping and pallor in ocular hypertensive eyes and normal eyes. The ocular hypertensive eyes showed significant increases in pallor but not in cupping compared to normal eyes. There was also a significant positive correlation of cupping with age in normal eyes but not in ocular hypertensive eyes. Fluorescein angiography of the optic disc showed an increase in the number and area of filling defects in ocular hypertensive compared to normal eyes. There were significant positive correlations in ocular hypertensives between percent area of

PURPOSE: Spaceflight-associated neuro-ocular syndrome (SANS) shares several clinical features with idiopathic intracranial-hypertension (IIH), namely disc edema, globe-flattening, hyperopia, and choroidal folds. Globe-flattening is caused by increased intracranial pressure (ICP) in IIH, but the cause in SANS is uncertain. If increased ICP alone causes SANS, then the ocular deformations should be similar to IIH; if not, alternative mechanisms would be implicated.
METHODS: Using optical coherence tomography (OCT) axial images of the optic nerve head, we compared "pre to post" ocular deformation

Measurements of pulsatile ocular blood flow (POBF) have been recorded in a group of healthy, ocular normotensive volunteers and ocular hypertensive patients recruited from outpatients. Use of a pneumotonometric probe linked to a Langham ocular blood flow system enabled readings of intraocular pressure and its variation with heart rate (ocular pulse) to be taken in erect and supine positions. Pulsatile ocular blood flow was calculated from these values by means of the pressure-volume relationship previously described for living human eyes. Assumption of the supine posture was accompanied by a s

Forty-eight patients aged from 60 to 69 years (58 eyes) with ocular hypertension (OHT) or primary open-angle glaucoma (POAG) and a control group of 16 persons of the same ages (26 eyes) were studied with a clinical contrast sensitivity test, the Vistech 6500 test. No significant differences could be found between the normal and OHT or POAG eyes in any of the spatial frequencies. The group of newly found POAG eyes showed the greatest difference from the normals, but this difference did not reach the level of significance in any of the spatial frequencies.
