Neurobrucellosis developing unilateral oculomotor nerve paralysis.

Brucellosis is a zoonotic infectious disease that is common around the world. Its clinical course demonstrates great diversity as it can affect all organs and systems. However, the central nervous system is rarely affected in the pediatric population. Neurobrucellosis is most frequently observed with meningitis and has numerous complications, including meningocephalitis, myelitis, cranial nerve paralyses, radiculopathy, and neuropathy. Neurobrucellosis affects the second, third, sixth, seventh, and eighth cranial nerves. Involvement of the oculomotor nerves is a very rare complication in neuro

[Oculomotor, trochlear, and abducens nerve palsies].

Patients with oculomotor, trochlear, or abducens nerve palsies mainly complain of binocular double vision, but sometimes merely of blurred vision or vertigo. The clinical signs comprise strabismus, pathologic head posture, and disturbed saccades. The characteristic motility deficits are picked up by measuring the strabismic angles at different directions of gaze. Documentation of all three spatial strabismic components is advantageous. Nonparetic strabismus and orbital diseases are important differential diagnoses. Conclusions about the cause of a palsy can be drawn from the time course of dou

Sporadic malignant nerve sheath tumour of the oculomotor nerve.

Malignant peripheral nerve sheath tumours (MPNST) are exceedingly rare in an intracranial location. In this report clinical and pathological evidence for the diagnosis of a MPNST arising from of the oclumotor nerve is presented. To our knowledge this is the first such case reported in the medical literature.

Malignant peripheral nerve sheath tumor of the oculomotor nerve.

We present the short-term follow-up magnetic resonance (MR) studies and 1H-MR spectroscopy in a child with malignant peripheral nerve sheath tumor of the oculomotor nerve associated with other less aggressive cranial nerve schwannomas. The tumor revealed perineural extension and diffuse nerve involvement besides rapid growth. 1H-MR spectroscopy was helpful in excluding an intra-axial neoplasm with exophytic growth, mainly due to the absence of creatine and N-acetyl aspartate peaks, and markedly elevated choline peak.

Posttraumatic abducens to oculomotor nerve misdirection.

INTRODUCTION: Paradoxical patterns of extraocular muscle, eyelid, or pupillary movements can occur following injury between divisions of the oculomotor nerve, trigeminal and abducens nerves, and trigeminal and oculomotor nerves. We report three cases of unusual ocular motility and eyelid movements that are a result of aberrant connections between the abducens and oculomotor nerves. METHODS: Three patients with unusual eye movement abnormalities after trauma were studied. A complete ophthalmic examination plus neuroradiologic evaluation were performed. RESULTS: Each patient manifested an aber

Solitary oculomotor nerve palsy in childhood.

In most cases of isolated oculomotor nerve palsy in 30 patients under age 20 years the palsy was congenital, and aberrant regeneration was present. The most common causes of acquired oculomotor nerve palsy were blunt trauma and infectious processes, both local and systemic. Neoplasms, aneurysms, and ophthalmoplegic migraine caused the other cases. The causes of isolated oculomotor nerve palsy in childhood differ from those in adults both in nature and in frequency of occurrence.

En bloc resection of an intracavernous oculomotor nerve schwannoma and grafting of the oculomotor nerve with sural nerve. Case report and review of the literature.

A case in which a left oculomotor nerve schwannoma treated by en bloc resection of the lesion and grafting of the oculomotor nerve with sural nerve is presented. Recovery of nerve function was partial, but useful and cosmetically good. The last follow-up examination performed 2 years after surgery revealed recovery of function in the elevator muscle of the upper eyelid, together with slight vertical movement of the eye.

[Use of nerve division in oculomotor paralysis].

Sectors have a double interest in an extraocular muscle palsy: to avoid the diplopia and to aid the reeducation of palsied muscles. Patient can alternatively wear the two types of glasses with sectors. Authors decrease sector's size when motility improves. They are stopped if regression, and in other cases, patients can wait surgical time with maximum comfort. Clinical examples are presented: VI, III and IV cranial nerve palsies and generalized diplopia.

Superior division paresis of the oculomotor nerve.

Five patients with isolated ptosis and a paresis of ocular elevation in abduction consistent with an isolated superior division III nerve palsy are reported. In all instances the III nerve appeared to be involved before its reported anatomic bifurcation into a superior and an inferior division, in the anterior cavernous sinus.

Synkinesis between facial nerve and oculomotor nerve. A case report.

We present a case history of a young man suffering from a facial-oculomotor synkinesis. The findings speak in favour of an acquired synkinesis due to trauma. Most probably the injury occurred in the midbrain, in the area of the vertical gaze control center and/or the area of the levator palpebrae motoneurons. A congenital synkinesis due to embryonic malformation seems to be unlikely, because at birth no restriction of the eye-ball motility was present.

Isolated Hypertrophic Neuropathy of the Oculomotor Nerve.

BACKGROUND: Hypertrophic neuropathy is a rare entity commonly associated with peripheral nerve, characterized by onion bulb formations. Its cranial nerve involvement is very rare; only 7 cases have been found in the literature. CASE DESCRIPTION: A 32-year-old white man with a 5-year history of intermittent right retro-orbital headache and mild right ptosis presented to the Neurosurgery Clinic. A magnetic resonance imaging of his brain demonstrated an enhancing lesion associated with the right third nerve. He underwent biopsy of the lesion, and its pathology report confirmed the diagnosis of h

Resection of an oculomotor nerve cavernous angioma.

BACKGROUND: Cavernous angiomas (CAs) of cranial nerves are rare, and their occurrence on the third cranial nerve is particularly rare. Surgical management of such CAs involving the third nerve is controversial. We describe a case of a symptomatic CA of the oculomotor nerve and review the literature in order to ascertain the relevance of surgical intervention. CASE DESCRIPTION: A 71-year-old male patient presented with a 2-month history of progressive oculomotor nerve paralysis. CA of the oculomotor nerve was suspected on magnetic resonance imaging (MRI). The patient underwent complete resecti

Neurinoma of the oculomotor nerve--case report.

Only 10 cases of oculomotor nerve neurinoma have been reported previously. The authors report the rarity of a neurinoma originating from Schwann cells of the oculomotor nerve. The diagnosis was based on the initial sign of oculomotor nerve paresis without involvement of other cranial nerves, neuroradiological and surgical findings, and histological features of the specimen obtained at surgery.

Neurinoma of the oculomotor nerve. Case report.

A 15-year-old boy with a recurrent third nerve palsy who was found to have a neurinoma of the third nerve is described. Unusual features of his presentation and review of the literature relative to this rare tumor are discussed. Tumors derived from Schwann cells, which wrap around the axons of peripheral nerves, are called neurinomas, neurofibromas, plexiform neurofibromas, neurilemomas, and schwannomas. Neurinomas are usually localized in the peripheral nerves. The most frequent site of occurrence of an intracranial schwannoma is the acoustic nerve; however, the tumor may occur along the seco

Oculomotor Nerve Palsy Due to Unusual Causes.

BACKGROUND: An isolated oculomotor nerve (CN III) palsy is a diagnostic concern because of the potential for serious morbidity or life-threatening causes. We present 5 unusual causes of oculomotor nerve palsy that escaped initial diagnosis in order to raise awareness of their associated features that will facilitate correct diagnosis. METHODS: This study consisted of a retrospective analysis of clinical features and imaging of 5 patients who were referred for neuro-ophthalmologic evaluation with presumed diagnosis of oculomotor nerve palsy of unknown reasons. RESULTS: A complete CN III palsy
