Clinical features and functional recovery of traumatic isolated oculomotor nerve palsy in mild head injury with sphenoid fracture.

OBJECT: The aim of this study was to provide information about long-term functional outcome in patients with isolated oculomotor nerve palsy following minor head injury and to discuss surgical treatment of these patients, especially those with accompanying sphenoid fracture. METHODS: A retrospective analysis was made of 26 patients with traumatic isolated oculomotor nerve palsy. The severity of oculomotor nerve palsy and the functional recovery were evaluated based on extraocular muscle movement, eyelid movement, and pupil size. On average, patients were evaluated 3.6 days after the initial i

Guillain-Barré Syndrome with Incomplete Oculomotor Nerve Palsy after Traumatic Brain Injury: Case Report and Literature Review.

Guillain-Barré syndrome (GBS) is a severe peripheral neuroinflammatory demyelinating disease characterized by symmetrical progressive limb weakness, which can be accompanied by cranial nerve and sensory disturbances. There is usually a history of bacterial or viral infection prior to onset. GBS is rarely seen after traumatic brain injury (TBI). We report a case of a 66-year-old male patient who presented with dilated pupils, followed by respiratory failure and symmetrical quadriplegia during a conservative treatment for TBI. He was eventually diagnosed with GBS and was treated with intravenous

Isolated oculomotor nerve palsy following mild traumatic brain injury: diagnostic challenges and insights from High-Resolution MRI.

BACKGROUND/PURPOSE: Isolated oculomotor nerve palsy (ONP) following mild traumatic brain injury (TBI) is rare and often presents diagnostic challenges. Typically associated with diffuse axonal injury and poor prognosis, ONP lacks comprehensive radiological documentation when no skull base fractures along the course of the 3rd Nerve or brainstem injuries are evident. This study explores the diagnostic utility of contrast-enhanced magnetic resonance imaging (CEMRI) in identifying ONP cases in mild TBI patients. METHODS: A retrospective analysis was conducted on six patients diagnosed with isola

An isolated oculomotor nerve palsy and brainstem hemorrhage in a 53-year-old male patient with mild traumatic brain injury: illustrative case.

BACKGROUND: Even though head injury is one of the possible causes for oculomotor nerve palsy (ONP), it is uncommon to find an isolated ONP in a patient who sustained a traumatic brain injury (TBI). Both posttraumatic ONP and brainstem hemorrhage are more frequently seen in patients with severe TBI, and the overall outcome of ONP after head injury is poor. OBSERVATIONS: A 53-year-old male patient presented after he was hit by a speeding car from behind while he crossing a road, and then he fell face first to the ground. His Glasgow Coma Scale score was 14 of 15. He had complete ONP of the left

Traumatic lesions of the optic, oculomotor, trochlear, and abducens nerves--computer tomographic findings.

Traumatic lesions of the oculomotor and optic nerves are usually associated with multiple injuries which pose difficulties for radiological examination. In 54 patients with functional disturbances of the optic, oculomotor, trochlear, or abducens nerve following severe multiple injuries, the initial computer tomographic examination was unable to document conclusively direct nerve damage. Indirect signs of nerve damage in the form of orbital and midface fractures were often found in the presence of optic and oculomotor nerve impairments, but seldom in the case of abducens nerve deficits. The cau

Traumatic oculomotor nerve avulsion with subarachnoid hemorrhage identified using magnetic resonance imaging.

Traumatic injury to the oculomotor nerve is a serious condition and generally carries a poor prognosis.Herein, we report a case of oculomotor nerve disruption with traumatic subarachnoid hemorrhage that was identified using an imaging technique. A 63-year-old female patient was brought to our emergency department following a traffic accident. Conservative treatment was initiated, leading to an improvement in her level of consciousness, although her right oculomotor nerve palsy symptoms remained unalleviated. She was discharged 2 weeks later with persistent symptoms of right oculomotor nerve pa

MRI Findings of Isolated Oculomotor Nerve Palsy after Mild Head Trauma in a Pediatric Patient: Case Report.

INTRODUCTION: Traumatic oculomotor nerve injury is usually caused by severe head trauma and is generally associated with other neurological deficits such as basilar skull fracture, orbital injury, or subarachnoid hemorrhage. Isolated traumatic oculomotor nerve injury after minor head trauma and its MRI findings are rarely reported. CASE PRESENTATION: We report a case of a 13-year-old girl with mydriasis, limited inferior and medial movement of the left eyeball, and left ptosis after a mild bump of the left forehead and eye into an electricity pole. The symptoms suggested left oculomotor nerve

Traumatic oculomotor nerve palsy.

Traumatic isolated oculomotor nerve palsy is not common. Oculomotor nerve palsy without internal ophthalmoplegia (pupil sparing) is extremely rare. We report a case of this condition in a child. An 11-year-old boy was transferred to our hospital after a head injury in a traffic accident. Neuro-ophthalmic examination showed that the left eye had limited adduction, supraduction, and infraduction, incomplete convergence and left ptosis, but no internal ophthalmoplegia. Magnetic resonance imaging indicated mild bending of the ipsilateral oculomotor nerve at the posterior petroclinoid ligament. One

Localization of traumatic oculomotor nerve palsy to the midbrain exit site by magnetic resonance imaging.

PURPOSE: To present the magnetic resonance imaging findings for a patient with traumatic oculomotor nerve injury. METHODS: We examined a patient with a right pupil-involving oculomotor nerve palsy after severe closed head trauma. RESULTS: Magnetic resonance imaging of the brain demonstrated marked signal hypointensity on gradient-echo T2*-weighted images consistent with hemorrhage at the midbrain exit site of the right oculomotor nerve. CONCLUSIONS: Distal fascicular damage or partial rootlet avulsion is a mechanism of injury in some traumatic oculomotor nerve palsies. Gradient-echo T2*-wei

Post-traumatic Oculomotor Nerve Palsy due to Intraneural Hemorrhage: A Case Report.

Oculomotor nerve palsy following traumatic brain injury is a rare entity. A head injury can result in diffuse neuronal axonal injury with subsequent microbleed within the nerve tissue. We report an unusual case of a patient who presented with complete isolated right third nerve palsy following a road traffic accident. In this patient, magnetic resonance imaging (MRI) revealed swelling and edema of the right oculomotor nerve along its cisternal segment with contrast enhancement. The acquisition of susceptibility-weighted images on MRI helped to clinch the diagnosis of intraneural hemorrhage as

Isolated oculomotor nerve palsy from minor head trauma.

Isolated third cranial nerve palsies in head trauma patients can be the result of direct or indirect damage to the oculomotor nerve. They are usually associated with severe head trauma. We reported a case of isolated oculomotor nerve palsy associated with minor head injury. No initial loss of consciousness was recalled. Computed tomography (CT), magnetic resonance imaging (MRI), and magnetic resonance angiography (MRA) of the brain were normal. Previous reports in the literature were reviewed and the possible mechanism of injury was discussed. Head injuries are commonly seen in sports settings

Pathogenesis of primary internal ophthalmoplegia after head injury.

Primary internal ophthalmoplegia is a rare ocular syndrome after head injury. Its pathogenesis remains unclear and it is only a hypothesis that the pupillomotor fibres on the ventromedial aspect of the oculomotor nerve are slightly contused against the petroclinoid ligament at the moment of impact. We observed a patient after head injury who presented with a fixed, dilated pupil and died within 15 hours of the accident. The postmortem examination revealed an isolated group of fibres on the medial aspect of her oculomotor nerve, which had been torn out of the midbrain. This suggests an alternat

Traumatic avulsion of the oculomotor nerve documented by high-resolution magnetic resonance imaging.

Autopsy studies have described definitive traumatic avulsion of the oculomotor nerve from the brainstem; however, detailed characterization of mechanisms and localization of traumatic nerve injury has yet to be definitively described in vivo. We report the case of a 13-year-old girl in whom high-resolution magnetic resonance imaging confirmed irreversible injury to the left oculomotor nerve after trauma.

Pupil-sparing oculomotor nerve palsy caused by upward compression of a large posterior communicating artery aneurysm. Case report.

A 69-year-old woman without diabetes or hypertension presented with a large posterior communicating artery aneurysm projecting beneath the oculomotor nerve manifesting as a 2-week history of progressive diplopia. Neurological examination revealed external ophthalmoplegia and blepharoptosis without pupil involvement. Neuroimaging showed a large aneurysm in the left internal carotid artery projecting postero-inferiorly. Craniotomy and neck clipping of the aneurysm revealed the origin at the junction of the internal carotid artery and posterior communicating artery, and elevation of the oculomoto

Diplopia as a sign of third nerve palsy due to intracranial aneurysm: a case report.

Third nerve palsies that result from head injuries or intracranial aneurysms may sometimes show symptoms of aberrant regeneration and only partially heal. The usual characteristics of oculomotor nerve palsy, caused by compression of the third cranial nerve, are severe ptosis, deficiencies in elevation, depression, and adduction, and a dilated, poorly responding pupil on the afflicted side. The parasympathetic fibres are usually spared from a vasculopathic lesion and impacted by compressive lesions because they are situated in the peripheral segment of the oculomotor nerve as it leaves the brai

Aberrant regeneration in idiopathic oculomotor nerve palsy: case report.

Aberrant regeneration of the oculomotor nerve usually follows injury to the nerve by posterior communicating arty aneurysms or trauma. A case of idiopathic third nerve palsy with pupillary involvement occurred in an otherwise healthy 38-year-old man. Follow-up examination 32 months later showed evidence of oculomotor function with aberrant regeneration.
