Lower motoneurons (MNs) show varied vulnerability in amyotrophic lateral sclerosis (ALS): those of non-ocular brainstem nuclei and most of those of the spinal cord are highly vulnerable, while those of extraocular brainstem nuclei are quite resistant. Results of our former study on the immunoexpression of the survival of motor neuron protein (SMN) and Gemins 2-4 in cervical spinal cord anterior horn -MNs of sporadic ALS patients suggested that a relative deficit in Gemin2 may play some role in the pathomechanism of the disease. Here, we tested this idea further by comparing immunoexpression p

Movements of the eye are produced by six extraocular muscles innervated by three cranial nerves: the oculomotor (III), the trochlear (IV), and the abducens (VI). These cranial nerves are discussed together because of the interrelated nuclear origins, neural pathways, and motor functions. The normal anatomic pathway of these three nerves is presented. The clinical and pathologic manifestations of lesions producing both isolated and complex palsies of these nerves are discussed along with imaging correlation.

The visceral, namely the parasympathetic component of the oculomotor complex which directly innervates ciliary muscle was investigated using horseradish peroxidase (HRP) in three cats and three monkeys. A unilateral total iridectomy to abolish innervation to the iris was made in all animals. After 1 month to allow fiber degeneration in the iris, HRP was injected into the ciliary muscle of ipsilateral eye. Distribution of the labeled cells at the midbrain and at the ciliary ganglion was examined. Labeled cells were present in the ciliary ganglion and midbrain in all three cats and three monkeys

Mitochondrial disorders can be linked to mutations in both mitochondrial and nuclear deoxyribonucleic acid, corresponding to various clinical phenotypes. Mutations in nuclear genes, including NDUFV1, have been associated with severe encephalomyopathies in infants, but genotype-phenotype correlations have remained elusive. This report details the complete clinical, biochemical, and molecular data of a 7-year-old male who presented at the age of 7 months with progressive ophthalmoplegia and later developed cerebellar ataxia, spasticity, and dystonia. Complex I deficiency was demonstrated in musc

Diplopia, a very common ophthalmic complaint, is a potential first sign of severe pathology. Here, we present a case of an atypical midbrain infarction targeting the lateral subnucleus of the oculomotor nuclear complex that manifested as diplopia with no additional symptoms of a stroke episode. Axial diffusion-weighted and coronal T2-weighted magnetic resonance imaging showed an infarct in the rostral midbrain affecting the subnucleus of the medial rectus located ventrally. Diffusion-weighted imaging was used to diagnose the medial rectus nucleus infarct.

Brain nuclei are spatially organized collections of neurons that share functional properties. Despite being central to vertebrate brain circuitry, little is known about how nuclei are generated during development. We have chosen the chick midbrain oculomotor complex (OMC) as a model with which to study the developmental mechanisms of nucleogenesis. The chick OMC comprises two distinct cell groups: a dorsal Edinger-Westphal nucleus of visceral oculomotor neurons and a ventral nucleus of somatic oculomotor neurons. Genetic studies in mice and humans have established that the homeobox transcripti

Oculomotor deficits, such as insufficiencies in accommodation, convergence, and saccades, are common following traumatic brain injury (TBI). Previous studies in patients with mild TBI attributed these deficits to insufficient activation of subcortical oculomotor nuclei, although the exact mechanism is unknown. A possible cause for neuronal dysfunction in these regions is biomechanically induced plasma membrane permeability. We used our established porcine model of head rotational TBI to investigate whether cell permeability changes occurred in subcortical oculomotor areas following single or r

PURPOSE: Convergence insufficiency is a very common disorder that can have significant adverse effects on school performance. When reading, children with this disorder often experience diplopia and headaches. We have recently obtained a rhesus monkey with a naturally occurring impairment of vergence eye movements. In the companion paper, we report behavioral testing that shows a pattern of impairments similar to what clinicians observe in human children with convergence insufficiency, including a receded near point, an exophoria that increases as target distance decreases, and difficulty maint

A clinical evaluation of visual and central oculomotor defects are presented in a series of 56 patients who were HIV carriers, of whom 47 fulfilled criteria for AIDS. Changes were detected in 22% of the asymptomatic patients (2/9), in 36% of the AIDS patients with no neurological complications and in 93% (15/16) of the AIDS patients with neurological involvement. Some changes were seen in 55% of the patients with AIDS. The commonest defects were alterations of saccadic movements and visual pursuit (35% respectively) and of opticokinetic nystagmus (28%). Only one homonymous visual defect was fo

Intraoperative clinical effects and the results of destruction were demonstrated in 31 patients with various forms of torticollis spastica subjected to operation on the interstitial nucleus of Cajal. The intraoperative clinical complex of symptoms was formed of changes of the toxic and clonic activity of the neck muscles and transient oculomotor and vestibular signs. The favourable postoperative results are evidence of the direct relation of the nuclear structures in the region of the posterior commissure to the mechanisms regulating the posture of the head and neck. Their exclusion may be one

The Edinger-Westphal nucleus of the oculomotor nuclear complex provides preganglionic parasympathetic innervation to the pupil. We labelled its retinal input by transneuronal autoradiography after an eye injection of [3H]proline in the macaque monkey. The primary retinal projection to the pretectum terminated in the ipsilateral and contralateral olivary nuclei. These nuclei were intensely labelled and sharply delimited, with a mean diameter of 590 microm and a rostrocaudal length of 2.52 mm. The caudal half of the olivary nucleus on each side broke into multiple clumps of label. Fragments of l

The midbrain represents the uppermost portion of the brainstem, containing numerous important nuclei and white matter tracts, most of which are involved in motor control, as well as the auditory and visual pathways. Notable midbrain nuclei include the superior and inferior colliculus nuclei, red nucleus, substantia nigra, oculomotor nuclear complex, and trochlear nucleus. In addition, white matter tracts include the brachium conjunctivum, medial and lateral lemniscus, spinothalamic tracts, and the fiber tracts within the cerebral peduncles. Although neurologically vital, many of these small mi

Our previous study showed that oculomotor internuclear neurons project to precerebellar populations lying in the paramedian tract cell groups, in addition to projecting to the abducens nucleus. Consequently, we posited that the abducens internuclear neurons might have similar projections, beyond their well-recognized projection to the oculomotor nucleus. With this in mind, we examined macaque monkey cases with abducens nucleus injections of anterograde tracers. As with oculomotor internuclear neurons, we found terminal fields in paramedian tract cell groups. These abducens nucleus injections,

The role of internuclear neurons residing in the medial rectus subdivision of the oculomotor nucleus and projecting to the abducens nucleus is poorly understood. As a step toward remedying this, we labeled this projection in macaque monkeys by injecting the oculomotor nucleus with anterograde tracers. Labeled terminal arbors present within the main body of the abducens nucleus were diffusely distributed and mainly directed at motoneurons. However, a dense terminal field was present along the dorsomedial aspect of the nucleus, designated the infragenual region, and immediately dorsal to the fac

Mammalian extraocular muscles contain singly innervated twitch muscle fibers (SIF) and multiply innervated nontwitch muscle fibers (MIF). In monkey, MIF motoneurons lie around the periphery of oculomotor nuclei and have premotor inputs different from those of the motoneurons inside the nuclei. The most prominent MIF motoneuron group is the C group, which innervates the medial rectus (MR) and inferior rectus (IR) muscle. To explore the organization of both cell groups within the C group, we performed small injections of choleratoxin subunit B into the myotendinous junction of MR or IR in monkey

Little is known about the mechanisms at play in nerve regeneration after nerve injury. Personal studies are reported regarding motonuclear changes after regeneration of injured cranial nerves, in particular of the facial and oculomotor nerves, as well as the influence that the natural molecule acetyl-L-carnitine (ALC) has on post-axotomy cranial nerve motoneuron degeneration after facial and vagus nerve lesions. Adult and newborn animal models were used. Massive motoneuron response after nerve section and reconstruction was observed in the motonuclei of all nerves studied. ALC showed to have s

A 65-years-old female was hospitalized 24 h after experiencing the sudden onset of subjective reduction in visual acuity and hypersomnia. On admission to the neurological ward, she presented isolated downgaze palsy. A Magnetic Resonance Imaging of the brain disclosed a discrete, ovalar hyperintensity involving the left paramedian thalamic-mesencephalon junction. The lesion was consistent with infarction. Isolated downgaze palsy has been described in thrombosis of Artery of Percheron leading to infarction of bilateral paramedian thalami along with structures from the mesencephalic-diencephalic

Biallelic variants in NADH (nicotinamide adenine dinucleotide (NAD) + hydrogen (H))-ubiquinone oxidoreductase 1 alpha subcomplex 13 have been linked to mitochondrial complex I deficiency, nuclear type 28, based on three affected individuals from two families. With only two families reported, the clinical and molecular spectrum of NADH-ubiquinone oxidoreductase 1 alpha subcomplex 13-related diseases remains unclear. We report 10 additional affected individuals from nine independent families, identifying four missense variants (including recurrent c.170G > A) and three ultra-rare or novel predic
