The treatment of olecranon fractures in adults.

Olecranon fractures are common. They are usually managed surgically with open reduction and either tension band wiring or plate fixation. Currently, there are few studies comparing fracture treatments. We aim to review the available literature to guide the orthopaedic surgeon on the management of these fractures. A literature review of peer-reviewed publications in international orthopaedic journals detailing olecranon fracture treatment was conducted. An additional focus was placed on the evidence base for and surgical outcomes of tension band wiring for common two-part fractures. Our novel i

Surgical techniques of olecranon fractures.

Olecranon fractures are common upper extremity injuries. The vast majority are treated with operative fixation. Many treatment techniques have been described including tension band and plating. This review covers the most commonly used fixation techniques in detail, including pearls and pitfalls with case examples of both successful treatments and potential complications.

Olecranon Fractures Have Features of Osteoporotic Fracture.

BACKGROUND: To determine whether olecranon fractures have osteoporotic features such as age-dependent, low bone attenuation and low-energy trauma as a cause of injury. METHODS: Elbow computed tomography (CT) and medical record review were performed in 114 patients (53 males and 61 females) with acute olecranon fractures. The mean age was 57 years. Bone attenuation was measured on the central part of the olecranon on sagittal CT images avoiding the fracture, and on the distal humerus (distal metaphysis and medial and lateral condyles) on coronal CT images. We compared bone attenuation and caus

Wide-Awake Anesthesia for Olecranon Fracture Fixation.

Introduction: Wide-awake local anesthesia and no tourniquet (WALANT) has come a long way. It has been reported to be successful in the surgery of distal radius and ulna fractures. We report a case of olecranon fracture plating under WALANT. Methods: Surgery was performed with the patient fully conscious where tumescent anesthesia was injected into the surgical site without application of tourniquet 30 minutes before the first incision. Posterior approach to the elbow was used, and the fracture was fixed with anatomical locking plates. Results: The surgery was successfully completed without pai

Cyclic loading of olecranon fracture fixation constructs.

BACKGROUND: Despite the good results that are usually reported after fixation at the sites of olecranon fractures and osteotomies, problems such as loss of fixation, nonunion, and the need for revision surgery are still encountered. Various types of fixation have been recommended, but few have been evaluated with use of clinically relevant cyclic load testing at appropriate levels of stress. The purpose of the present study was to test multiple olecranon fixation techniques under physiologic cyclic loads. METHODS: We studied ten cadaveric elbows with use of cyclic loading that simulated (1) a

Suture Repair of Simple Transverse Olecranon Fractures and Chevron Olecranon Osteotomy.

Tension band wiring of simple transverse olecranon fractures results in a high reoperation rate due to hardware problems. A technique using high-strength braided polyester and polyethylene suture through a bone tunnel has been piloted. This technique is suitable for simple transverse olecranon fractures or olecranon osteotomy with stability of the ulnohumeral articulation. A transverse drill hole was made in the ulna with a 2.5-mm drill. Fracture fixation was achieved using 2 braided synthetic sutures passed through the bone tunnel and grasping the insertion of the triceps tendon. The outcome

Displaced apophyseal olecranon fracture in a healthy child.

Olecranon apophyseal fractures in children are uncommon. The bulk of these injuries are nondisplaced and therefore can be treated nonoperatively. Few published reports of children with displaced fractures of the olecranon apophysis exist, and the large majority of reports describe children with osteogenesis imperfecta. We report our experience with the case of an 11-year-old, healthy, male patient without osteogenesis imperfecta who sustained a displaced olecranon apophyseal fracture during a fall.

Olecranon Osteotomy Exposure for Distal Humeral Fracture Treatment.

BACKGROUND: Olecranon osteotomy (OO) is commonly utilized to improve exposure when treating intra-articular distal humeral fractures. A chevron-shaped osteotomy facilitates reduction and increases surface area for healing1. Following distal humeral fracture reduction and fixation, the OO fragment is fixed with a precontoured plate. The OO technique yields comparable outcomes to alternative techniques1,2. DESCRIPTION: The technique is performed as follows. (1) Imaging is reviewed and preoperative planning is performed. (2) The patient is positioned in the lateral decubitus position with the op

Fracture through an enthesophyte on the olecranon process.

The patient was a 54-year-old man who was currently serving in the military. He was evaluated by a physical therapist in a direct-access capacity for a chief complaint of left elbow pain that began 3 weeks prior. The physical therapist ordered radiographs, which revealed a linear lucency through a large enthesophyte at the insertion of the triceps tendon on the olecranon process that was consistent with a fracture.

Treatment of comminuted olecranon fractures with olecranon plate and structural iliac crest graft.

Severely comminuted olecranon fractures are challenging injuries. Commonly used tension band wiring exerts excessive compressive forces causing olecranon shortening, joint incongruity and humeral trochlea subluxation. We report a retrospective study of 3 patients who underwent surgery for a severely comminuted olecranon fracture, with open reduction and fixation with a bridging rigid locking plate and intercalary tricortical structural iliac bone graft. Joint stability was restored allowing early mobilization and good functional outcome. Patients' mean age was 54 years. Mean follow-up was 23 m

Olecranon fractures : current treatment concepts.

Despite being one of the most common injuries around the elbow, the optimal treatment of olecranon fractures is far from established and stimulates debate among both general orthopaedic trauma surgeons and upper limb specialists. It is almost universally accepted that stable non-displaced fractures can be safely treated nonoperatively with minimal specialist input. Internal fixation is recommended for the vast majority of displaced fractures, with a range of techniques and implants to choose from. However, there is concern regarding the complication rates, largely related to symptomatic metalw

Tension Suture Fixation of Olecranon Fractures.

BACKGROUND: Mayo type-IIA olecranon fractures are characterized by a transverse or short oblique fracture without articular comminution or ulnohumeral instability. Traditionally, these fractures are treated with a tension-band wiring technique. Despite good rates of fracture union, tension-band wiring is associated with a reoperation rate of 30% to 60%, usually for removal of prominent metalwork. The tension suture technique was developed as an alternative method of fixing Mayo type-IIA fractures using only high-tensile braided nonabsorbable number-2 sutures, with the aim of reducing the reope
