Trial watch: dexmedetomidine in cancer therapy.

Dexmedetomidine (DEX) is a highly selective α2-adrenoceptor agonist that is widely used in intensive and anesthetic care for its sedative and anxiolytic properties. DEX has the capacity to alleviate inflammatory pain while limiting immunosuppressive glucocorticoid stress during major surgery, thus harboring therapeutic benefits for oncological procedures. Recently, the molecular mechanisms of DEX-mediated anticancer effects have been partially deciphered. Together with additional preclinical data, these mechanistic insights support the hypothesis that DEX-induced therapeutic benefits are media

Anesthesia Management of Awake Craniotomy for Excision of a Supratentorial Hemorrhagic Brain Tumor Affecting Speech and Memory.

BACKGROUND Awake craniotomy is a neurosurgical approach used for resecting brain tumors in eloquent areas, where real-time neurological monitoring is critical to preserving function. Selecting the appropriate anesthesia approach is essential when preoperative language deficits or patient anxiety may limit intraoperative participation. While motor mapping is an established component of awake craniotomy, the use of a speech-language pathologist for real-time intraoperative monitoring remains underutilized. CASE REPORT A 31-year-old woman presented with a 1-year history of seizures, headaches, an

Intraoperative and Perioperative Outcomes of Breast-Conserving Surgery After Neoadjuvant Systemic Therapy: A Retrospective Cohort Study.

PURPOSE: Neoadjuvant chemotherapy (NAC), with or without immunotherapy (IO), is increasingly used in patients with breast cancer and may facilitate breast-conserving surgery (BCS). Because these therapies may alter physiologic reserve and perioperative care needs, we evaluated their associations with anesthetic management and postoperative recovery. PATIENTS AND METHODS: This retrospective cohort study included 374 patients with stage II-III breast cancer undergoing BCS at a high-volume ambulatory cancer center. Patients were categorized as receiving NAC, NAC + IO, or no neoadjuvant therapy.

Protocol for a cohort study: evaluating the effect of neoadjuvant immunochemotherapy on intraoperative remifentanil consumption during IoC2-guided anesthesia in elderly patients with esophageal cancer.

BACKGROUND: The influence of neoadjuvant immunochemotherapy (nICT) on intraoperative opioid consumption during esophagectomy for cancer is not well understood. IoC2 enables real-time antinociceptive depth monitoring. This study assesses nICT's effect on remifentanil requirements in elderly patients using IoC2. METHODS: This prospective observational cohort study will be conducted at Jiangsu Cancer Hospital. Sixty patients scheduled for esophagectomy will be enrolled. Patients will be divided into the nICT group (n = 30) or the treatment-naive group (n = 30). All patients will receive a standa

Anaesthetic management for pulmonary bilobectomy in a patient with previous contralateral lobectomy and total laryngectomy with tracheostomy.

Pulmonar lobectomy by video-assisted thoracoscopic surgery (VATS) usually requires collapse of the operative lung and ventilation of the contralateral lung only. The complexity of anaesthetic management in such procedures is further increased by patient comorbidities and previous pathologies. We report the case of a patient scheduled for right lower lobectomy by VATS with a history of previous left lower lobectomy and total laryngectomy with permanent tracheostomy after a glottic carcinoma. A left-sided double-lumen tube was used in combination with a bronchial blocker in order to allow select

Navigating complexity: anaesthetic management of renal cell carcinoma with inferior vena cava thrombus for radical nephrectomy and thrombectomy.

Renal cell carcinoma (RCC) with inferior vena cava (IVC) tumour thrombus represents a high-risk surgical and anaesthetic scenario requiring meticulous multidisciplinary coordination. We report the peri-operative management of a patient in his late 60s with right-sided clear-cell RCC extending to level III of the IVC, who underwent open radical nephrectomy with tumour thrombectomy under continuous transoesophageal echocardiographic monitoring. The procedure was complicated by a total blood loss of 6.4 L and a 26-min period of supra-hepatic IVC clamping. Haemodynamic stability was maintained thr

Ketamine administration and postoperative mortality after major cancer surgery: a propensity-matched population-based study.

BACKGROUND: Ketamine is commonly used perioperatively as an anaesthetic and analgesic adjunct and has demonstrated potential antitumour effects in preclinical studies. However, clinical and epidemiological data on its association with long-term outcomes after oncologic surgery remain limited. METHODS: We conducted a retrospective cohort study using the TriNetX US Collaborative Network, including adults (≥18 yr) undergoing major solid tumour cancer surgery between January 1, 2014, and April 1, 2023. The primary exposure was ketamine administration on the day of surgery. Propensity score matchi

High-flow nasal cannula oxygen therapy in post-anesthesia care unit (PACU) reduces postextubation atelectasis in patients undergoing esophageal cancer surgery: A randomized controlled trial.

BACKGROUND: Esophageal cancer surgery frequently leads to post-extubation atelectasis. The efficacy of high-flow nasal cannula (HFNC) oxygen therapy in the post-anesthesia care unit (PACU) for these patients remains unvalidated. METHODS: This randomized controlled trial enrolled 100 patients after esophageal cancer surgery, allocated to a control group (conventional oxygen therapy at 5 L/min, n = 50) or an HFNC group (heated and humidified oxygen at 40% FiO₂, 37°C, 10 L/min, n = 50). The primary outcome was the lung ultrasound score at pre-extubation (T1), 30 minutes post-intervention (T2), a

Combined Epidural-General Anesthesia is Associated with Increased Postoperative Complications in Patients Receiving Neoadjuvant Chemotherapy Followed by Gastrointestinal Radical Surgery: A Propensity Score Weighting Study.

BACKGROUND: Combined epidural-general anesthesia (TEA+GA) can enhance postoperative recovery after gastrointestinal surgery. However, in patients receiving preoperative neoadjuvant chemotherapy (NACT), a known risk factor for postoperative complications, it remains unclear whether TEA+GA improves postoperative outcomes. METHODS: This retrospective cohort study included 365 patients with NACT followed by radical resection of gastrointestinal cancer at Zhongshan Hospital between January 2020 and December 2022. TEA was initiated before the induction of GA. The primary outcome was the incidence o

Intraoperative tumor lysis syndrome in a child with Wilms' tumor.

Tumor lysis syndrome in an onco-metabolic emergency resulting from massive lysis of rapidly proliferating malignant cells seen commonly in patients with hematological malignancies such as acute lymphocytic leukemia and Burkitt's lymphoma and is quite rare in solid tumors. Spontaneous development of tumor lysis has been described among other trigger factors such as corticosteroid therapy, anesthesia, tumor manipulation during surgery and pyrexia. We describe such a case in a 5-year-old boy posted for excision and staging of a massive Wilms' tumor who developed a hyperkalemic cardiac arrest duri

Recovery quality with regional anesthesia and dexmedetomidine sedation versus general anesthesia for ambulatory breast cancer surgery: A randomized trial.

BACKGROUND: Advances in regional anesthesia techniques and the widespread use of ultrasound guidance have enabled safe and effective anesthesia for ambulatory breast cancer surgery, eliminating the general anesthesia-associated systemic effects. We therefore compared regional anesthesia with dexmedetomidine sedation to general anesthesia on the 15-item postoperative quality of recovery (QoR-15). METHODS: Ninety-six patients scheduled for breast-conserving surgery and sentinel lymph node biopsy were randomly assigned to regional anesthesia with sedation or general anesthesia. In patients assig

Perioperative Anesthetic Management in Cancer Patients Receiving Strong Opioids: A Retrospective Study from a Single Center with Palliative Care Team Involvement.

There are no clinical guidelines for the perioperative management of cancer pain among opioid users, and a lack of literature exists concerning this topic. This study aimed to retrospectively investigate the actual perioperative management of patients with cancer who received general anesthesia while using strong opioids. We conducted a chart review of the perioperative management of patients with cancer who received general anesthesia while using strong opioids. The primary outcome was the number of additional administration of analgesics during the first 24 h after surgery, except for PCA (p

Opioid-sparing utility of subcutaneous analgesia systems in pediatric oncology patients following major tumor resection surgery: A matched case-control study.

BACKGROUND: For pediatric oncology patients undergoing major tumor surgery, subcutaneous analgesic systems (SAS) have been suggested as a potential pain-relief alternative to epidural analgesia. Studies randomizing the two modalities have not demonstrated benefit of SAS but suffer from inherent bias due to frequent contraindications to epidural analgesia in this population. We sought to quantify the incremental analgesic benefit of SAS versus standard intravenous analgesia following major pediatric oncology surgeries, adjusting for potentially confounding factors. METHODS: We performed a retr

DAMPs and PAMPs in the Perioperative Period: Danger Signaling, Immune Dysfunction, and Oncologic Implications.

Background and Objectives: The perioperative period is characterized by marked biological stress responses that extend beyond direct tissue injury. Innate immune activation during surgery is largely driven by molecular danger signals. Surgical trauma, ischemia-reperfusion injury, blood transfusions, mechanical ventilation, and perioperative infections are critical events that result in the release of danger signals: damage-associated molecular patterns (DAMPs) and pathogen-associated molecular patterns (PAMPs). This narrative review examines perioperative DAMP and PAMP sources, their molecular

Differential effects of sevoflurane versus propofol on calmodulin expression in breast cancer patients.

BACKGROUND: Evidence from experimental and clinical studies suggests that anesthetic drugs may modulate tumor biology by influencing immune regulation, inflammation and intracellular calcium signaling. However, the extent to which sevoflurane or propofol alters calcium-related molecular expression in human breast cancer tissue remains unclear. OBJECTIVES: This study aimed to compare the effects of sevoflurane- versus propofol-based anesthesia on calcium signaling pathway-related gene and protein expression in breast cancer tissues and to evaluate their association with early -postoperative re
