Sweet's syndrome: a revisit for hematologists and oncologists.

The diagnosis of Sweet's syndrome rests on a combination of clinical symptoms and characteristic physical and pathologic features. Patients typically have fever and tender erythematous skin lesions (papules, nodules, or plaques). Neutrophilia, high levels of serum inflammatory markers, and diffuse mature neutrophil infiltration localized to the upper dermis are the most important findings. Sweet's syndrome was first described by Robert Sweet in 1964, whose sentinel paper described 8 women with fever, leukocytosis, and erythematous plaques infiltrated by neutrophils. Subsequently, extracutaneou

Nutrition and Aging: a Practicing Oncologist's Perspective.

Malnutrition is common in patients with cancer and is associated with a variety of negative outcomes. These can include reduced treatment tolerance and worsened cancer prognosis. Various aspects of aging, including sensory, physical, or psychosocial changes, place older patients at a particularly high risk for malnutrition, and these geriatric factors must be identified early and addressed. Despite the lack of available evidence on the optimal nutritional interventions for older adults with cancer, the oncologist must be prepared to address the common nutritional concerns that arise in both ad

Pharmacologic Pain Management: What Radiation Oncologists Should Know.

Individuals with cancer experience a host of symptoms, especially when the malignancy is advanced. Pain occurs from the cancer itself or related treatments. Undertreated pain contributes to patient suffering and lack of engagement in cancer-directed therapies. Adequate pain management includes thorough assessment; treatment by radiotherapists or anesthesia pain specialists; anti-inflammatory medications, oral or intravenous opioid analgesics, and topical agents; and attention to the emotional and functional effects of pain, which may involve social workers, psychologists, speech therapists, nu

How well do medical oncologists' perceptions reflect their patients' reported physical and psychosocial problems? Data from a survey of five oncologists.

BACKGROUND: Modern cancer treatments can cure or prolong patients' lives. However, the associated physical and psychosocial problems can detrimentally affect patients' compliance with treatment and, ultimately, their outcomes. Therefore, oncologists need to recognize the problems experienced by their patients and, when possible, help resolve these problems. METHODS: The authors conducted a cross-sectional survey of physical symptoms, anxiety, depression, and perceived needs among 204 consenting patients visiting an outpatient medical oncology department. Immediately following consultations wi

Evaluating Oncologists' Communication Strategies Regarding Molecular Testing and Immunotherapy.

PURPOSE: In metastatic colorectal cancer (mCRC), testing for mismatch repair deficiency (dMMR) can identify patients who will benefit from immune checkpoint inhibitors (ICIs). Communication regarding molecular testing and ICI is challenging, with studies demonstrating that oncologists have varied knowledge of genomic testing and frequently use technical jargon, and patient understanding is poor. We aimed to characterize discussions of molecular testing and ICIs to identify effective communication strategies. METHODS: We conducted an observational qualitative study from 2022 to 2023. We recrui

Radiomics systematic review in cervical cancer: gynecological oncologists' perspective.

OBJECTIVE: Radiomics is the process of extracting quantitative features from radiological images, and represents a relatively new field in gynecological cancers. Cervical cancer has been the most studied gynecological tumor for what concerns radiomics analysis. The aim of this study was to report on the clinical applications of radiomics combined and/or compared with clinical-pathological variables in patients with cervical cancer. METHODS: A systematic review of the literature from inception to February 2023 was performed, including studies on cervical cancer analysing a predictive/prognosti

Curability of metastatic cancer: a survey of medical oncologists.

BACKGROUND: Patients with metastatic cancer are living longer due to treatment advances. We explored oncologists' perceptions about curability in metastatic cancer. METHODS: We invited medical oncologists to complete a 21-item online survey. We conducted descriptive analyses and thematically analyzed free-text responses. RESULTS: A total of 126 respondents completed the survey. The median age was 39 years (range = 27-75). Most respondents worked in Australian (64%), metropolitan (88%), public practices (56%). The most frequently treated cancer types were breast (55%), lung (52%), and colorec

Hyperbaric oxygen therapy: A practical guide for gynecologic oncologists.

Hyperbaric oxygen therapy (HBOT) has several promising uses in the setting of gynecologic malignancy. Treatment for gynecologic malignancy frequently includes radiation, which can cause significant cell and tissue injury leading to long-term toxicities for patients. HBOT involves creating a high-pressure atmosphere in which the patient breathes 100% oxygen, which results in increased arterial pO2, vasodilation of hypoxic tissue, and decreased inflammatory cascades. This has been shown to be beneficial in treatment of common long-term toxicities associated with radiation. Response rates to HBOT

Systemic anticancer therapy for urothelial carcinoma: UK oncologists' perspective.

Urothelial carcinoma (UC) is a common cancer associated with a poor prognosis in patients with advanced disease. Platinum-based chemotherapy has remained the cornerstone of systemic anticancer treatment for many years, and recent developments in the treatment landscape have improved outcomes. In this review, we provide an overview of systemic treatment for UC, including clinical data supporting the current standard of care at each point in the treatment pathway and author interpretations from a UK perspective. Neoadjuvant cisplatin-based chemotherapy is recommended for eligible patients with m

Investigation of Radiation Oncologists' Awareness of Online Reputation Management.

BACKGROUND: Online reputation management (ORM) is an emerging practice strategy that emphasizes the systematic and proactive monitoring of online reviews relating to one's professional reputation. OBJECTIVE: We developed this survey project to assess whether radiation oncologists are aware of ORM and how it is utilized in their practices. We hypothesized that ORM is largely unknown by most practicing radiation oncologists and that little time is spent actively managing their reputations. METHODS: An online survey was submitted to 1222 radiation oncologists using the Qualtrics research platfo

[Psycho-oncology and the present tasks of psycho-oncologists].

This article aims to contribute to a better apprehension by oncologists of the work done by psycho-oncologists, by diminishing misconceptions and misunderstandings end promoting collaboration between them. First, we describe the main aspects of their practice, which is still too poorly apprehended, the landmarks they use (when they support a patient they take into account his/her body, social and professional position, personal and family history, objectives, psychopathology and the family), and the tools they have at their disposal. We then describe how psycho-oncology is currently organised,

When should the medical oncologist call the surgeon?

Head and neck squamous cell carcinoma (HNSCC) is a pathology that necessitates collaborative care and multidisciplinary management. There is a role for timely surgical involvement which evolves at different stages of disease progression. Upfront surgery remains the standard of care for resectable disease at certain subsites to provide locoregional control and definitive pathologic information for accurate staging and guidance of adjuvant therapies. Surgery also fits into advancing treatment paradigms with neoadjuvant systemic approaches. This requires early assessment of surgical candidacy to

A review of histopathologic assessment for head and neck oncologists.

GOAL OF REVIEW: With a deeper understanding of histopathologic assessment, head and neck oncology specialists (surgical oncologists, radiation oncologists, and medical oncologists) will be better equipped to address the increasing complexity encompassing head and neck cancer management. INTRODUCTION: Histopathologic assessment of surgical specimens imparts crucial information that is essential for post-operative treatment planning and prognostication for patients with head and neck squamous cell carcinoma (HNSCC). Herein, we discuss the most current guidelines and recommendations to elucidate

Evaluation and Treatment of Iron Deficiency for the Practicing Oncologist.

Iron deficiency is highly prevalent in patients with cancer and has major impacts on morbidity and quality of life. The etiologies of cancer-related iron deficiency are multifactorial, and unlike in the noncancer population, most patients with cancer-related iron deficiency have functional iron deficiency due to cancer-related inflammation. Diagnosis of iron deficiency in patients with cancer requires different thresholds compared with patients without cancer. Treatment of iron deficiency requires adequate dosing of iron repletion and specific understanding of patient context (ie, perioperativ

How an Oncologist's Intuition Compares to a Geriatric Assessment.

BACKGROUND: A geriatric assessment (GA) is recommended before treating older cancer patients. The goal of this study was to investigate if the additional information from a GA and discussion by a multidisciplinary board (MB) change the treatment recommendations. PATIENTS AND METHODS: Older cancer patients (n = 421) were prospectively assessed by an oncologist for eligibility for chemotherapy. A multidimensional GA was performed and a discussion about each patient was held by a MB including a geriatrician. Differences between the judgment made by the oncologist, that of the MB, and the Balducc
