Automatic matching of surgeries to predict surgeons' next actions.

OBJECTIVE: More than half a million surgeries are performed every day worldwide, which makes surgery one of the most important component of global health care. In this context, the objective of this paper is to introduce a new method for the prediction of the possible next task that a surgeon is going to perform during surgery. MATERIAL AND METHOD: We formulate the problem as finding the optimal registration of a partial sequence to a complete reference sequence of surgical activities. We propose an efficient algorithm to find the optimal partial alignment and a prediction system using maximu

Nonoperating Room Anesthesia and Procedural Sedation in Interventional Radiology: A Collaborative Framework for Safe Growth.

Interventional radiology (IR) has become one of the fastest expanding areas of procedural care, with increasing demand for image-guided diagnostic, vascular, oncologic, and emergency interventions. This growth has created parallel demands for both procedural sedation (PS) and anesthesiology services, the so-called nonoperating room anesthesia (NORA) within the IR suite. These services should not be viewed as competing models, but as different points on a continuum in which the level of sedation or anesthesia is matched to the patient, procedure, location, level of care, clinicians' qualificati

Nanophotonic Design of Germanium-Silicon Single-Photon Detectors for Integrated Quantum Photonic Circuits.

Scalable on-chip quantum photonic systems require single-photon detectors that simultaneously provide high detection efficiency, low timing jitter, high bandwidth, and low noise while remaining compatible with silicon photonics and room-temperature operation. Germanium-silicon (Ge/Si) single-photon avalanche detectors (SPADs) offer a promising CMOS-compatible platform for telecom-band photon detection; however, existing implementations often suffer from limited photon-detection efficiency, excess timing jitter, and challenges in monolithic integration with quantum photonic circuits. Here, we p

Care transition of trauma patients: Processes with articulation work before and after handoff.

While care transitions influence quality of care, less work studies transitions between hospital units. We studied care transitions from the operating room (OR) to pediatric and adult intensive critical care units (ICU) using Systems Engineering Initiative for Patient Safety (SEIPS)-based process modeling. We interviewed twenty-nine physicians (surgery, anesthesia, pediatric critical care) and nurses (OR, ICU) and administered the AHRQ Hospital Survey on Patient Safety Culture items about handoffs, care transitions and teamwork. Care transitions are complex, spatio-temporal processes and invol

Programmable Topological Magnetic Textures in Above-Room-Temperature 2D Ferromagnet Fe3GaTe2.

Topological invariants identify distinct topological states by remaining unchanged under continuous deformations, which reflects the robustness of topological phases against external perturbations. In magnetic systems, they characterize deformation-resistant spin textures and endow non-trivial configurations with exceptional stability; however, they also create inherent challenges in precise modulation of topological magnetic states. Here, we demonstrate the generation and programmable modulation of arbitrary topological numbers in a magnetic skyrmion bag system that is thermally stable in the

A simplified building airflow model for agent concentration prediction.

A simplified building airflow model is presented that can be used to predict the spread of a contaminant agent from a chemical or biological attack. If the dominant means of agent transport throughout the building is an air-handling system operating at steady-state, a linear time-invariant (LTI) model can be constructed to predict the concentration in any room of the building as a result of either an internal or external release. While the model does not capture weather-driven and other temperature-driven effects, it is suitable for concentration predictions under average daily conditions. The

Action recognition in medical environments for robotic assistance.

PURPOSE: Teamwork is fundamental to medical practice and relies on seamless collaboration among professionals with different tasks. Integrating robotic systems into this environment demands smooth interactions. Human action recognition, which infers a person's state without explicit input, can support this. We focus on handovers between medical staff, using the actions as implicit cues for robotic assistance to replace the giving party in such scenarios. METHODS: Skeletal information processed with differing machine learning algorithms makes it possible to derive actions out of sequential ima

Audit of intensive care unit admissions from the operating room.

An audit of 265 intensive care unit (ICU) admissions from the operating room was performed for the year 1991. In a quality assurance exercise we identified 34 unanticipated ICU admissions (UIAs) by a retrospective peer review of the medical charts. Of these UIAs, 16 were deemed predictable and seven preventable. Five of the seven potentially preventable UIAs were judged to have had inappropriate intravenous fluid management. This has prompted changes in our education programme. In an assessment of our resource management, we evaluated prospectively collected data on the Apache II scores on the

Resection of intraventricular tumors via a computer-assisted volumetric stereotactic approach.

Intraventricular tumors present a major challenge for neurosurgeons because of their depth and the important structures around them. Between 1984 and 1991, we performed 60 computer-assisted volumetric stereotactic resection procedures in 58 patients with intraventricular tumors (30 patients with third ventricular tumors and 28 patients with lateral ventricular tumors). The pathological findings of the tumors were as follows: colloid cyst in 27, giant cell astrocytoma in 5, central neurocytoma in 4, pilocytic astrocytoma in 4, meningioma in 3, subependymoma in 3, metastatic tumor in 3, oligoden

Improving efficiency in robotic-assisted surgery: a practical guide for surgeons.

The integration of robotic systems into minimally invasive surgery offers significant advantages, including enhanced precision, reduced invasiveness, and improved patient outcomes. However, sustaining these benefits requires optimization of operational efficiency to justify the substantial resource investment associated with robotic technologies. This review examines strategies to enhance efficiency in robotic-assisted surgery using a four-pillar framework: (1) team structure and leadership; (2) organization of care; (3) procedural standardization; and (4) parallel task overlap. Evidence from

Cost Implications of Varying the Surgical Technique, Surgical Setting, and Anesthesia Type for Carpal Tunnel Release Surgery.

PURPOSE: Carpal tunnel release (CTR) is a common surgical procedure, representing a financial burden to the health care system. The purpose of this study was to test whether the choice of CTR technique (open carpal tunnel release [OCTR] vs endoscopic carpal tunnel release [ECTR]), surgical setting (operating room vs procedure room [PR]), and anesthetic type (local, monitored anesthesia care [MAC], Bier block, general) affected costs or payments. METHODS: Consecutive adult patients undergoing isolated unilateral CTR between July 2014, and October 2017, at a single academic medical center were

Picometer scale vibrometry in the human middle ear using a surgical microscope based optical coherence tomography and vibrometry system.

We have developed a highly phase stable optical coherence tomography and vibrometry system that attaches directly to the accessory area of a surgical microscope common to both the otology clinic and operating room. Careful attention to minimizing sources of phase noise has enabled a system capable of measuring vibrations of the middle ear with a sensitivity of < 5 pm in an awake human patient. The system is shown to be capable of collecting a wide range of information on the morphology and function of the ear in live subjects, including frequency tuning curves below the hearing threshold, maps
