PURPOSE: The variations of origin of ophthalmic artery are extremely rare. Here, we aimed to present a patient had a rare variation of the ophthalmic artery.
CLINICAL PRESENTATION: The patient had a history of Moyamoya disease. The imaging studies revealed ophthalmic artery origination from basillar artery. In addition, M1-segment fusiform aneurysm, vasculitis and, as a variation, middle meningeal artery originating from the ophthalmic artery were also detected. An unusual case of post-traumatic exophthalmos without pulsation, bruit or thrill, due to missile injury is presented. Arterialization of the ophthalmic vein represents one of the very few situations where an eye injury can lead to exsanguination and dealth unless the diagnosis is made early. Prompt carotid arteriography is advised to document the pathologic process and guide appropriate therapy.

A 39-year-old woman presented with left visual disturbance and diplopia. Magnetic resonance imaging revealed a well-enhanced tumor in the left tentorium, cavernous sinus, and suprasellar region. Angiography demonstrated an abnormal origin of the ophthalmic artery from the anterior cerebral artery. The tumor was partially removed by left frontotemporal craniotomy with orbitozygomatic osteotomy. Intraoperatively, the anomalous origin of the ophthalmic artery was confirmed. This anatomical variation is extremely rare.

BACKGROUND: Nearly 10% of patients with Crohn's disease develop ocular complications. Most complications are inflammatory in nature, and involve both the anterior and the posterior segments of the eye. Episcleritis, uveitis and keratopathies are the most frequently described. Occlusions of retinal and optic nerve vessel are less common. We believe this to be an important observation as the ocular involvement was the patients primary complaint, thus allowing the correct diagnosis to be made.
