Artificial Intelligence-Assisted Diagnosis of Retinal and Neuro-Ophthalmic Diseases: a Comparative Evaluation of ChatGPT, Google Gemini and An Ophthalmologist.

OBJECTIVES: This study evaluated the diagnostic performance of two large language models (LLMs), ChatGPT and Google Gemini, to identify common retinal and optic nerve diseases benchmarked against an experienced ophthalmologist. METHODS: Thirty standardized case vignettes, each comprising a brief clinical history and a high-resolution fundus image, were independently evaluated by ChatGPT, Google Gemini and an ophthalmologist. Ten retinal and optic nerve diseases were included. Diagnostic accuracy was calculated against a gold standard defined by consensus of two retina specialists. Inter-rater

Reversible bilateral optic tract edema following pipeline-assisted coiling of a large ophthalmic aneurysm: A case report.

Edema coursing the optic apparatus has traditionally been associated with sellar and para-sellar tumors. However, postoperative aneurysmal volume expansion following endovascular treatment has been reported to induce cranial neuropathies, such as vision loss in rare instances. Here, we present a case report of worsening optic tract edema associated with bilateral visual acuity deficit following treatment of a large left paraophthalmic aneurysm with pipeline-assisted coiling. Rapid resolution of visual deficit was observed following administration of corticosteroids. A 42-year-old female with a

[Ophthalmic surgical robot-assisted retinal puncture and injection for submacular hemorrhage caused by polypoid choroidal vasculopathy: a case report].

A 65-year-old man presented with decreased visual acuity in the left eye for 1 month. The diagnosis of hemorrhagic retinal detachment (submacular hemorrhage), which was caused by idiopathic polypoid choroidal vasculopathy, was confirmed by the ultra-wide-angle fundus examination, optical coherence tomography, and B-ultrasound. A vitrectomy combined with an ophthalmic surgical robot-assisted retinal puncture and injection was performed. The recombinant tissue plasminogen activator was injected accurately by the ophthalmic surgical robot between the retinal nerve epithelium and retinal pigment e

Coil-Assisted Onyx Embolization of an Ophthalmic Artery-Supplied Anterior Cranial Fossa Dural Arteriovenous Fistula Mimicking an Intracranial Aneurysm.

Dural arteriovenous fistulas are acquired intracranial vascular malformations characterized by abnormal shunting between dural arterial feeders and venous structures. Anterior cranial fossa and ethmoidal dural arteriovenous fistulas are uncommon but clinically important lesions because they frequently drain directly into cortical veins and may be associated with venous ectasia or aneurysmal venous dilatation. When venous variceal dilatation is present, these lesions may mimic intracranial aneurysms on noninvasive vascular imaging, making digital subtraction angiography essential for definitive

Micro-balloon-assisted embolization of anterior cranial fossa dural arteriovenous fistula via a trans-ophthalmic approach - a technical report and case series.

PURPOSE: Dural arteriovenous fistulas (dAVF) account for approximately 10-15% of all intracranial arteriovenous abnormalities. dAVFs carry a significant risk of mortality, particularly in cases of acute hemorrhage, of up to 10%. A small proportion of these dAVFs are found in the anterior cranial fossa (ACF), of which the rate of hemorrhage can be as high as up to 91%. The Scepter Mini (SM) is the smallest dual-lumen micro-balloon (MB) available for neurointerventional practice. It consists of a 2.8 French outer diameter, with a 2.2 mm × 9 mm semi-compliant balloon providing a working length of

Agreement between novice and experienced trachoma graders improves after a single day of didactic training.

BACKGROUND/AIMS: Prevalence estimates and treatment decisions for trachoma are based entirely on ocular clinical examination. The aim of the current study is to demonstrate that ophthalmic assistants can be trained and certified to provide trachoma grading within a single day. METHODS: Conjunctival photographs from an area with endemic trachoma were randomised into two sets of 60 cases. Photographs were graded for trachomatous inflammation-follicular (TF) and trachomatous inflammation-intense (TI) by three experienced graders. Inter-rater reliability of eight ophthalmic assistants and three e

Rapid epidemiologic assessment of cataract blindness. The Aravind Rapid Epidemiologic Assessment Staff.

A major constraint to obtaining reliable information about blindness and its causes in developing countries is the limited availability of ophthalmologists for diagnosis in population-based surveys. This study in rural south India assessed the feasibility of using non-ophthalmologists to make diagnoses in a population-based survey. Ten men in their early twenties with 12 years of schooling were recruited and trained as ophthalmic assistants through a six week course in basic ophthalmology. All people aged 40 and older in households in 24 villages were enumerated and invited to have an eye exam

Primary Eye Care in Eastern Nepal.

Purpose: Vision 2020 and the Global Action Plan 2013-2019 prioritise primary eye care (PEC) as an important component of reducing avoidable blindness. Studies in sub-Saharan Africa have demonstrated that current PEC provision is poor. There has been no evaluation of the current practice of PEC among primary health care workers (PHCWs) in Nepal. Methods: A mixed methods descriptive cross-sectional study with semi-structured interviews and focus group discussions (FGDs) was carried out in Eastern Nepal. Government employed PHCWs working at health posts in three districts were invited to take part

Barriers to accessible ophthalmic imaging device uptake: an exploratory study of ophthalmology care provider perspectives.

BACKGROUND AND OBJECTIVES: Clinical ophthalmology often depends on imaging to support accurate diagnoses. However, traditional tabletop imaging machines often fail to accommodate patients with mobility impairments. Despite this, few clinical settings routinely use accessible ophthalmic imaging devices to address these concerns. This study aims to identify perceived barriers to the uptake of accessible ophthalmic imaging devices to inform changes and improve patient care. METHODS: In this exploratory mixed-methods survey study, a questionnaire was distributed to ophthalmology care providers ac

Large language models for ophthalmic examination understanding: from information extraction to clinical decision support.

Ophthalmology relies on heterogeneous examination outputs, including device-generated reports, structured measurements, fundus photographs, optical coherence tomography (OCT), visual field plots, corneal imaging, and multimodal clinical data. Publicly available large language models (LLMs) and multimodal large language models (MLLMs) can process these inputs without study-specific ophthalmic training, creating opportunities for information extraction, interpretation, and clinical decision support. This Mini Review synthesizes 37 peer-reviewed studies evaluating such models across ophthalmic ex

Neuro-ophthalmological manifestations of sarcoidosis.

Neuro-sarcoidosis has important ophthalmic and neuro-ophthalmic manifestations. Sarcoidosis most commonly affects the uveal tract (iris, ciliary body, and choroid) however the optic nerve is commonly involved. Sarcoid related optic neuritis is an important differential diagnosis in optic neuritis especially in atypical presentations. The use of multimodal imaging techniques available in the ophthalmic setting can enable the detection of choroidal or optic nerve granulomas and aid the diagnosis. Efferent manifestations of neuro-sarcoidosis are broad and can range from isolated cranial neuropath
