High dose rate brachytherapy for early stage oral tongue cancer.

BACKGROUND: High dose rate (HDR) interstitial brachytherapy of the oral tongue is a new treatment modality. Our study evaluates the outcomes of patients with early stage oral tongue cancer as treated by HDR interstitial implant. METHODS: We reviewed the records of 19 patients who were seen between 1994 and 2000 with carcinoma of the oral tongue and whose primary tumors were treated solely with interstitial implant using HDR remote afterloading technique. Ten patients had T1 N0 disease, and the remaining 9 had T2 N0 disease. Elective neck treatment was withheld for 12 patients. The remaining s

Pathomorphologic features of the ulcerative stage of oral aphthous ulcerations.

Macroscopic, histopathologic, and immunohistochemical features of eight 1- to 7-day-old minor (Mikulicz) aphthae, one herpetiform ulcer, and one ulcer from a patient with Behcet's syndrome were studied. In addition to light and electron microscopy, methods included the peroxidase-antiperoxidase (PAP) technique to disclose binding of IgA, IgG, IgM, Clq, and C3. Observations revealed the presence and distribution of extravasates of erythrocytes at and around the ulcers, extravascular neutrophilic granulocytes undermining the oral epithelium of the ulcer margin, the presence of numerous macrophag

Incorporation of grade into stage in oral cavity squamous cell carcinoma: A novel staging schema.

BACKGROUND: While numerous cancer staging systems have incorporated grade into stage, the impact of grade on oral cavity carcinoma (OCC) prognosis has been conflicting. We investigated grade as a prognostic determinant in OCC staging. METHODS: Multivariable Cox regression models of OCC patients identified via U.S. cancer registry data were constructed to determine associations between grade (G1 = low-grade, G2 = intermediate-grade, G3 = high-grade) and overall survival (OS). Recursive partitioning analysis (RPA) was used to derive staging schema. RESULTS: Overall, 46,789 OCC cases were ident

Oral microbiome profiles by periodontitis stage in a Korean population.

BACKGROUND: Periodontitis is a chronic inflammatory disease driven by oral microbial dysbiosis. Although the oral microbiome has been characterized in diverse populations, comprehensive profiling across periodontal disease stages defined by the 2018 AAP/EFP classification remains limited in Korean adults. METHODS: In this pilot prospective cross-sectional study, oral microbiome profiles were characterized in 74 participants classified into three groups: healthy controls (n = 24), Stage I-II periodontitis (n = 12), and Stage III-IV periodontitis (n = 38). Mouthwash samples were collected and s

Calcitriol administration in end-stage renal disease: intravenous or oral?

1,25-Dihydroxyvitamin D deficiency plays an important role in the pathogenesis of secondary hyperparathyroidism, and adequate replacement of this hormone is considered essential to normalize parathyroid gland function and restore bone homeostasis in patients with advanced renal failure. Although initial uncontrolled clinical trials suggested the superiority of intravenous calcitriol treatment, more recent controlled investigations show that different routes (oral versus intravenous), frequency (daily versus intermittent), and dosing (physiological versus pharmacological) of calcitriol administ

Early Stage Oral Submucous Fibrosis is Characterized by Increased Vascularity as Opposed to Advanced Stages.

INTRODUCTION: The degree of vascularity of the diseased mucosa in Oral Submucous Fibrosis (OSMF) has always been a matter of debate with conflicting results. Knowledge of this aspect is important to understand pathogenesis of OSMF, which in future could be translated into therapeutic strategies. AIM: In the present study, attempt has been made to investigate parameters like Mean Vascular Density (MVD), Total Vascular Area (TVA) and Mean Vascular Area (MVA) using CD34 antibody. MATERIALS AND METHODS: Forty five previously untreated histopathologically diagnosed cases of OSMF were retrieved fr

Detecting Early-Stage Oral Cancer from Clinically Diagnosed Oral Potentially Malignant Disorders by DNA Methylation Profile.

Clinically, early-stage oral cancers are difficult to distinguish from oral potentially malignant disorders (OPMDs), and invasive tissue biopsy should be performed to determine a treatment strategy. Previously, we focused on gargle fluid as a noninvasive testing method and reported aberrant methylation in gargle fluid in patients with oral cancer. This study aimed to distinguish early-stage oral cancer from clinically diagnosed OPMDs using gargle fluid samples. We collected gargle fluid samples from 40 patients who were clinically diagnosed with OPMDs in the training set; among them, 9 patient

Hypoxia and angiogenesis across clinical stages of oral submucous fibrosis: implications for oral carcinogenesis.

BACKGROUND: "Oral Submucous Fibrosis" (OSF) is a progressive "potentially malignant disorder" which is characterized by fibrosis and epithelial atrophy, resultant to hypoxia. The resulting hypoxic microenvironment is thought to trigger angiogenic responses that promote disease progression and malignant transformation. This study assessed whether hypoxia (HIF-1α) and angiogenesis (CD105) correlate with clinical stages in OSF. METHODS: A total of 105 tissue samples (30 normal oral mucosa; 75 OSF) were analysed using immunohistochemistry. HIF-1α immunoreactice score and CD105 based microvascular

Assessment of correlation between clinical staging, functional staging, and histopathological grading of oral submucous fibrosis.

BACKGROUND: Oral submucous fibrosis (OSMF) is a premalignant condition with a greater prevalence in countries such as India. Various classifications have been put forth by multiple authors to determine the clinical, functional, and histopathological grade of the disease. The classification systems have greatly helped to determine the treatment modality for the patients. Understanding the correlation between the various classifications will help us determine the course of the disease, management, and prognosis of OSMF. This study assesses the correlation between clinical, functional, and histop

Controversies in the treatment of early-stage oral squamous cell carcinoma.

The treatment of early-stage oral squamous cell carcinoma (OSCC) is still a controversial issue. Thanks to the 8th edition of TNM by AJCC there is a better distinction between the stages of OSCC. However, Stages I and II still share the same treatment protocol, even if the prognosis is radically different. A retrospective study has been conducted including 70 previously untreated patients with Stage I or II OSCC, treated with tumorectomy and selective neck dissection. The study focuses on the link between pT1/2 and various other factors, particularly histological grading, vascular and perineur

Advanced stage of oral myiasis in children: a clinical case report.

Human myiasis is characterized by larval infestation of body tissues or cavities of live individuals. Although most frequently observed in underdeveloped and tropical countries, reports of human myiasis have been encountered throughout the world. Conventional treatment is the mechanical removal of the larvae, one by one, which is painful and embarrassing and repugnant to both health professional and patient. Even though human myiasis is not considered rare, most dental professionals have very little knowledge of the diagnosis and treatment of this pathology. The purpose of this article, theref
