Orbital spread of conjunctival squamous cell carcinoma following evisceration: a reminder of the importance of thorough evaluation before and after destructive surgery.

Evisceration is a neglected surgery usually performed by the most junior ophthalmologists in unsalvageable eyes with panophthalmitis. It is universal knowledge that evisceration is contraindicated in eyes with intraocular malignancy. We report 2 patients who presented to us with squamous cell carcinoma (SCC) in eviscerated sockets. While one of them was diagnosed with SCC on histopathological examination of the eviscerated contents, the other had history suggestive of conjunctival SCC prior to evisceration. The second patient presented to us with complaints of poorly fitting prostheses. We con

Self-limiting Orbital Emphysema Following Ocular Evisceration and Demonstration of Nasolacrimal-Oropharyngeal Communication in a South African Ostrich (Struthio camelus australis).

A 25-year-old, male, sanctuary-owned, South African ostrich (Struthio camelus australis) was evaluated for orbital emphysema after evisceration of a nonvisual and chronically irritated eye. On initial ophthalmic examination, the ostrich's left eye displayed severe corneal fibrosis, broad anterior synechia, and a shallow anterior chamber, all suggestive of a previous corneal perforation. Conjunctival hyperemia and eyelid crusts were also present, reportedly associated with chronic rubbing. Evisceration of the left eye was performed by excising the eyelid margins, conjunctiva, nictitans, cornea,

The use of autologous dermis grafts for the reconstruction of the anophthalmic socket.

PURPOSE: The main objectives of enucleation, evisceration or secondary orbital implants are to replace orbital volume and obtain good motility and adaptation of the implant and the external prosthesis. We describe our experience using autologous dermis graft sutured to Tenon and conjunctiva following evisceration, enucleation and any reconstruction requiring either a primary or secondary orbital implant, even those with large tissue loss. MATERIALS AND METHODS: A retrospective case series of patients who received an autologous dermis graft to assist in closure of a Tenon's capsule and conjunc

Repair of the anophthalmic cavity of rats with synthetic hydroxyapatite.

We placed spheres of synthetic hydroxyapatite (calcium chloride combined with sodium phosphate) in the eviscerated or enucleated orbital cavity of rats in order to evaluate the biocompatibility of this material with the orbital cavity. The study was conducted on 50 albino rats, 25 of which were submitted to enucleation and 25 to evisceration of one eye. The animals were sacrificed 7, 15, 21, 30 and 60 days after surgery and the orbital content was submitted to histopathological examination. A reaction of the young granulation tissue type was observed first. The hydroxyapatite was gradually sur

Modified four incomplete petals evisceration technique: A 14 year case series.

PurposeThe aim of this article is to describe the outcomes of a case series using our own variation of the four-petal evisceration technique which may minimize the rate of orbital implant exposure through a simple approach.MethodsOur new technique consists of incompletely dividing the first two scleral flaps, thus creating four incomplete petals that can be sutured over the orbital implant. A case series with a minimum follow-up of 12 months is reported.ResultsOne hundred and twenty-two eyes of 121 patients who underwent evisceration surgery using the described technique are included. No cases

Congenital mature orbital teratoma: A case report.

Teratomas are gonadal or extragonadal neoplasms, derived from the three embryonic tissues, composed of germ cells of the neuroectoderm, mesoderm and ectoderm. Congenital orbital teratoma (OCT) commonly affects the left orbit, primarily affecting women over men at a ratio of 2:1. We present the case of a female patient of 9 days of extrauterine life with a left mature congenital orbital teratoma. The orbit and oculoplastic service performed an orbital exenteration and total resection of the tumor mass.

Silicone Sphere Implant Extrusion From Orbital Granulomatosis With Polyangiitis: A Rare Complication in the Anophthalmic Socket.

Orbital implant extrusion is a known complication following evisceration and enucleation. In this case report, we present a 45-year-old woman who presented with a left silicone implant exposure and infection 2 years following evisceration with saddle nose on examination. CT of the maxillofacial bones without contrast showed bilateral soft tissue infiltration around the superior recti muscles, as well as a nasal septum perforation from extensive sinus disease. Left orbitotomy revealed a small fibrotic mass near the orbital roof. Biopsy and serology results were consistent with granulomatosis wi

Transorbital Approach to the Cavernous Sinus After an Exenteration.

BACKGROUND AND OBJECTIVES: To describe a novel technique of transorbital access to the lateral wall of the cavernous sinus (CS) after exenteration. METHODS: Cadaveric dissection study. Seven heads (13 orbits) were dissected after total orbital exenteration. The technique was centered on creation of an osteotomy within the greater wing of sphenoid, bordered by the superior and inferior orbital fissures to access the middle cranial fossa. V2 within the foramen rotundum was used as a guide to enter the interdural plane of the lateral CS wall. Results were expressed as the mean value ±1 SD. RESU

Orbital exenteration: an updated review with perspectives.

Orbital exenteration is a radical and disfiguring surgery mainly performed in specialized tertiary care centers. Orbital exenteration has long been considered the treatment of choice for managing periocular tumors invading the orbit or primary orbital malignancies. Over the past decades, attention has been directed toward reducing the perioperative morbidity by developing new surgical devices and new strategies and promoting cosmetic rehabilitation by providing adequate facial prostheses. Despite these advances, several studies have questioned the role of orbital exenteration in improving over
