Orbital fractures in children.

PURPOSE: To describe the demographics, etiologic factors, clinical presentations, and outcomes of orbital fractures in children. METHODS: This was a retrospective case series of 96 consecutive patients under 18 years of age with orbital fractures presenting to the Massachusetts Eye and Ear Infirmary, including both hospitalized and nonhospitalized patients. RESULTS: Orbital fractures in children were most frequently the result of sports, assault, or motor vehicle accident. The majority of patients did not require hospitalization and were treated as outpatients. The medial wall and floor of t

Orbital fractures in children.

OBJECTIVE: To determine if the pattern of orbital fractures may be influenced by the changing craniofacial ratio of the growing child, as the orbit is the boundary between the face and the cranium. DESIGN: Retrospective case series of 40 patients between the ages of 1 year and 16 years with orbital fractures. SETTING: The Albany (NY) Medical Center Hospital, a tertiary level 1 trauma center. OUTCOME MEASURES: The sex, age, site, and mechanism of injury, associated injury, and treatment methods for children admitted to the Albany Medical Center Hospital with orbital fractures between July 19

Sequelae of orbital fractures.

Many injuries and complications of orbital fractures are instantaneous and unpreventable, some develop over time, and some are a result of surgery. Most complications can either be managed or prevented and are anticipated based on the fracture location and size. A thorough search for probable complications is mandatory, especially ocular injuries and intracranial complications. Most patients are carefully followed for 7-14 days, and the significant residual sequelae are managed. Some complications, however, require urgent care.

Naso-ethmoid-orbital fractures.

This report reviews the experience with naso-ethmoid-orbital fractures at a major medical center. Thirty-three patients were treated between 1 October 1976 and 1 January 1979, who met our definition of naso-ethmoid-orbital fractures. The majority sustained trauma in a motor vehicle accident and 60% of the patients had associated nonfacial injuries. All patients had other facial injuries and fractures; orbital rim or floor, 94%; complex maxillary (LeForte II-III), 72%; and mandible fractures, 25%. Slightly over one half of the group of naso-ethmoid fractures were open; the remainder were closed

Orbital fractures in children.

In children, differences in the properties and proportions of bone in the craniofacial skeleton and the lack of development of the paranasal sinuses result in orbital fractures that present differently from those in adults. Facial growth may be disturbed by such injuries and also by surgical intervention, which should therefore be as conservative as possible. However, urgent operation is needed to prevent irreversible changes when fractures of the orbital floor involve entrapped muscle. We present an approach to such injuries.

[Orbital fractures in children].

The aim of this article is to review data concerning paediatric orbital fractures. These fractures exhibit strong specificities because they occur in a growing face. Due to the craniofacial growing pattern and the peumatization of paranasal sinuses, there are differences in the anatomical location of orbital fracture with the age: before the age of seven they are mostly orbital roof and after seven they involve the orbital floor. The clinical diagnosis is confirmed with a computed tomography scan (CT scan), gold standard for the imaging in the orbital fractures. The magnetic resonance imaging

Unilateral orbital floor fractures.

Ninety patients operated on, on suspicion of unilateral orbital floor fractures are described. Age, sex, and etiology are given. Operative indication is examined and related to the operative findings. Fifty-six of the patients had fractures requiring transplantation, 24 had fractures which did not. Ten patients had no fractures. Only autogenous bone graft was used. The patients were examined three months and one to 11 years postoperatively (questionnaire). Of 82 operated patients who answered the questionnaire diplopia was found in eight, enophthalmos in two, hypesthesia in 18, reduced mobilit

Pediatric Orbital Floor Fractures: Outcome Analysis of 72 Children with Orbital Floor Fractures.

BACKGROUND: Orbital floor fractures are uncommon in the pediatric population. The aim of this study was to review the presentation, management, and outcomes for children with these injuries. METHODS: A retrospective review was performed on 72 consecutive children with orbital floor fractures over a 21-year period. RESULTS: Seventy-two patients with 76 fractures were identified. Mean follow-up time was 14.2 ± 4 months. The majority (50 percent) of patients suffered minimally displaced fractures, whereas 17 percent (13 of 76) suffered blowout fractures and 5 percent (four of 76) suffered trapd

Orbital fractures: pathophysiology and implant materials for orbital reconstruction.

Among midfacial fractures, the frequency of orbital injuries is surpassed only by nasal fractures. A clear understanding of orbital anatomy and the pathophysiology of these injuries is critical to accurate diagnosis, precise surgical reconstruction, and successful clinical outcomes. This chapter reviews the mechanism of injury and pathophysiology of orbital fractures as well as the implant materials that are currently used for surgical reconstruction.

[Orbital volume measurement of enophthalmos of orbital blowout fractures].

OBJECTIVE: To measure the volumetric changes of enophthalmos of orbital blowout fractures, and to study the relation between the change of the orbital volume and the degree of enophthalmos. METHODS: In 16 patients with enophthalmos of orbital blowout fractures, the measurement of orbital volume was carried out during 3 months to 2 years after injury by using the computed tomography (CT), computer image processing and computer orbital three-dimensional measuring method. The relation between the orbital volume discrepancy and enophthalmos was assessed by using Pearson correlation coefficients.

Orbital emphysema: a potentially blinding complication following orbital fractures.

A case of visual loss due to orbital emphysema secondary to a blow-out fracture of the orbit is presented. Because vision returned to 20/20 following an optic nerve decompression procedure, we hypothesize that our patient developed a compressive optic neuropathy with ischemia due to the emphysema. Essential instructions concerning the injury that the emergency physician should give the patient suffering an orbital blow-out are also presented.

Orbital Fractures and Air Travel.

UNLABELLED: Previous surveys indicate that the majority of clinicians recommend avoidance of air travel for a period of time following an acute orbital fracture. This advice has largely been based on the theoretical risk that in-flight pressure variation may exacerbate orbital emphysema and produce visual compromise, though no previous reviews have been conducted on the evidence underpinning this advice. The purpose of this study was, therefore, to conduct a systematic review of the literature pertaining to the safety of air travel in the setting of an acute orbital fracture. A systematic revi
