Carcinoma of the tongue and tonsil (oropharynx).

In this article, the multidisciplinary approach to the management of carcinomas of the tongue and oropharynx has been emphasized. The decision for the primary mode of therapy will depend on the multiple factors discussed herein. However, a surgeon skilled in the management of head and neck tumors, including major reconstructive techniques, should lead the team. Most patients with head and neck carcinoma present with lesions that are relatively far advanced, and the combination of surgical excision and radiation therapy has proved to be the best method of controlling this disease. Even when cur

[Accessory external ear in the oropharynx].

This report describes the case of a one-day old girl with an accessorial ear lobe in the region of the left soft palate. The girl was referred to us by the children's clinic because of shortness of breath and difficulties in swallowing. On inspecting the cavity of the mouth and the pharynx a pedunculated, ball-shaped, whitish, membranous tumour was identified. The size of the tumour was comparable to a cherry, hanging down from the left velum into the pharynx. It was possible to remove the tumour from its base by using the sling technique. The operation itself did not present any complications

[Solitary extramedullary plasmocytoma of oropharynx].

Report on a solitary extramedullary plasmacytoma case, an entity rarely diagnosed in daily practice, because of its low incidence. When the process occurs is seen fundamentally located at upper aero-digestive tract. Our patient was a 34-year-old man, who was diagnosed as oropharyngeal plasmacytoma with the peculiarity that the lesion feigned a benign one wether for the clinical features or the macroscopical aspect. The patient underwent surgery and postoperative rontgentherapy, after discarding a systemic disease. Till now the evolution proved good.

Controversies in the management of oropharynx cancer.

Squamous cell carcinomas of the oropharynx account for approximately 25% of all head and neck squamous cell malignancies. Most patients present with locally advanced tumors and require a multimodality approach to treatment, with input from qualified surgeons and radiation and medical oncologists. For organ preservation, concurrent chemoradiotherapy is usually preferred over surgery with adjuvant radiotherapy. Controversies regarding management of particular populations of locally advanced oropharyngeal tumors exist, including whether to include induction chemotherapy before chemoradiation, the

Radiation-induced leiomyosarcoma of the oropharynx.

Leiomyosarcoma is a malignant mesenchymal tumor originating from smooth muscle cells, which most frequently develops in the myometrium and in the gastro-intestinal tract. Reviewing the international literature, radiation-induced sarcoma arise in 0.035 to 0.2% of all irradiated patients. Especially in the head and neck region, radiation-induced leiomyosarcoma is an extremely rare lesion. The authors report a case of a radiation-induced leiomyosarcoma of the tonsillar region of the oropharynx in a 51-year-old male patient, who had undergone radiation therapy of this region 38 years before. The l

Carcinoma of the oral cavity and oropharynx.

A high index of suspicion in smokers, coupled with inspection and palpation, is necessary for early diagnosis of cancer of the oral cavity and the oropharynx. Surgery or radiotherapy, or a combination of those modalities, is usually successful, except in advanced cases of cancer of the tongue base and the oropharynx.

Carcinoma of the oropharynx: soft palate.

This article reviews the experience of The Princess Margaret Hospital in the management of squamous cell carcinoma of the soft palate and uvula. Between 1970 and 1979, 70 patients with this diagnosis received primary radiation therapy. The overall five year survival is 43%. The primary tumor was controlled in 80% by radiation. Advanced local (T3) and nodal (N2/3) disease proved difficult to control with radiation alone. The development of second malignancies in patients cured of their primary cancer remains a distressing problem.

Oropharynx cancer after sleep apnea surgery.

Surgery can treat sleep apnea. An elderly male underwent lingual/palatine tonsillectomy for OSA. He was then found to have T3N2 p16+ squamous cell carcinoma. He is receiving chemoradiation. Recognition of occult malignancy in tonsillectomy specimens may facilitate early diagnosis and treatment for patients following sleep apnea surgery.

Adult Lymphangioma of the Oropharynx: A Case Report.

Lymphangioma insults in adults are uncommon and the occurrence in the oropharynx is extremely rare. Although disease history and clinical symptoms may help in distinguishing it from malignant tumors, the pathophysiology of lymphangioma via histological examinations should be established for convenient and precise diagnosis. We present a 20-year-old male with lymphangioma in the right oropharynx, and the multiple-angle examination and treatment plan used. The case further emphasizes the need to consider lymphangioma in the differential diagnosis of an oropharyngeal mass. Awareness that lymphang

[Primary localized amyloidosis in nasopharynx and oropharynx].

Primary localized amyloidosis (PLA) is a rare benign disease defined by the presence of extracellular deposits of insoluble amyloid fibrils without systemic involvement. We report a case story of a patient with a tumour in pharynx. It was excised and based on histopathology the diagnosis PLA was confirmed. The patient was examined for systemic amyloidosis (SA). All tests were normal except for an elevated lactate dehydrogenase level. Using PET/CT we found a hypermetabolic lymph node, but the ultrasound examination was normal. The patient continued follow-up and has so far no sign of recurrence

Hyalinizing clear cell adenocarcinoma of the oropharynx.

BACKGROUND: Hyalinizing clear cell adenocarcinoma (HCCA) is a rare salivary gland malignancy. To our knowledge, there are no published reports of primary clear cell carcinoma of the vallecula. METHODS: A 59-year-old man with progressive dysphagia and otalgia underwent direct laryngoscopy with biopsy of a vallecular mass. The pathology results returned as clear cell adenocarcinoma. He underwent a wide local excision for his T₂N₀M₀ stage II tumor. RESULTS: Clear cell adenocarcinoma of the oropharynx typically presents as a painless neck mass and it can be difficult to distinguish from other cl

Transoral robotic photodynamic therapy for the oropharynx.

Photodynamic therapy (PDT) has been used for head and neck carcinomas with little experience in the oropharynx due to technical challenges in achieving adequate exposure. We present the case of a patient with a second right tonsil carcinoma following previous treatment with transoral robotic surgery (TORS) and postoperative chemoradiation for a left tonsil carcinoma. Repeat TORS for the right tonsil carcinoma reviewed multiple positive surgical margins. The power output from the robotic camera was modified to facilitate safe intraoperative three dimensional visualization of the tumor bed. The

Rosai-Dorfman disease presenting in the oropharynx.

BACKGROUND: We report a case of Rosai-Dorfman disease (RDD) presenting as an oropharyngeal mass, and we provide a detailed discussion of this rare clinical entity. METHODS: An initial biopsy, described as being consistent with lymphoma, was reviewed at a second outside institution and was thought to be reactive lymphoid hyperplasia. The patient then presented to our institution for a second opinion. Direct laryngoscopy revealed a firm 1- to 2-cm lesion involving the left soft palate and superior tonsillar pillar. RESULTS: The diagnosis of RDD was confirmed with immunohistochemical staining o

Squamous cell carcinoma of the oropharynx and hypopharynx.

Oropharyngeal and hypopharyngeal squamous cell carcinomas require an interdisciplinary approach to manage patients appropriately. Tumor stage and histology, functional outcome, and patient comorbidities are important factors to consider. Various surgical approaches as well as chemotherapy and radiation therapy alone or in combination remain the mainstay of therapy.

Carcinoma of the oropharynx: factors affecting outcome.

OBJECTIVES/HYPOTHESIS: To assess the value of both patient- and tumor-related factors of oropharyngeal squamous cell carcinoma in predicting patient outcome, with respect to the three primary subsites of the disease. It was hypothesized that the subsite has a significant impact on outcome. STUDY DESIGN: Historical cohort study. METHODS: A chart review was conducted of 126 patients diagnosed with squamous cell carcinoma of the oropharynx over a 10-year period. The oropharynx was divided into the following subsites: 1) base of tongue, 2) tonsil and pillars, and 3) uvula, soft palate, and poste

Peripheral facial palsy and dysfunction of the oropharynx.

The facial nerve contributes to the oropharyngeal phase of deglutition via the buccinator, perioral, digastricus posterior, and stylohyoid muscles. The gustatory and salivatory functions of the facial nerve are also known to contribute to swallowing. The relation between peripheral facial nerve palsy (PFP) and swallowing dysfunction has never been studied systematically. Forty four patients with unilateral Bell's palsy (acute or chronic stages) and 20 normal control subjects were investigated. In 66% of patients with PFP, oropharyngeal swallowing was disturbed as demonstrated electrophysiologi
