OBJECTIVE: In passive middle ear prosthetics, rigid implants have proven successful in reconstructing the ossicular chain. However, these cannot fully replicate the physiology of the ossicular chain. Pressure fluctuations cause high stresses in rigid passive prostheses, which can result in dislocation, protrusion, and pre-tension in the annular ligament resulting in unsatisfactory hearing results.
METHODS: In collaboration with MED-EL, we developed a new passive middle ear prosthesis that features a balanced, centered ball joint between the headplate and shaft of the prosthesis. We compared t

OBJECTIVES: Microbial biofilms have been associated with poor outcomes with a variety of biomedical implants; however, this relationship has not been established with ossicular chain reconstruction prostheses (ORPs). The purpose of this study was to determine if biofilms are present on ORPs in patients undergoing revision ossicular chain reconstruction and if their presence correlates with middle ear scarring or hearing outcomes.
STUDY DESIGN: Prospective and blinded.
SETTING: Tertiary referral center.
PATIENTS: Patients undergoing revision ossicular chain reconstruction with previous ORP p

OBJECTIVE: Since 1993, titanium prostheses have been used in Europe as a biocompatible implant for ossicular reconstruction. More recently, the titanium ossicular prosthesis has become more widely available in the United States as a newer means of surgical restoration of hearing. We will review the hearing results of patients who have undergone tympano-ossiculoplasty using a titanium total ossicular replacement prosthesis (TOP). We will compare these results to our previously published results using the porous polyethylene TOP.
METHODS: Retrospective review in a tertiary otologic practice.

OBJECTIVE: To show the efficiency of using transmastoid atticotomy (TMA) endoscopy on the outcome of ossiculoplasty in patients with cholesteatoma. TMA is often performed as part of the surgical management of patients with middle ear cholesteatoma extending to the epitympanum. TMA can also be used as an access for endoscopic view to confirm the right alignment and stability of the ossicular prosthesis because the reconstruction of the tympanic membrane will obscure the visualization of the prosthesis.
METHODS: A retrospective study was done at a tertiary referral institute, including 133 ears

Experienced otologists achieve better functional outcomes and fewer complications post-surgery. Otology requires high surgical precision, with risks including total and permanent hearing loss in the operated ear. Although otologic surgical simulators exist, most are sophisticated and expensive. The main objective of this study was to develop and assess a reproducible, affordable, and effective middle ear surgical simulator, defined by its ability to improve residents' anatomical understanding, confidence, and basic technical skills in prosthesis placement. A simulator model was 3D-printed to a

The author describes a new prosthesis used in functional rehabilitation when the chain is destroyed. It consists of a tripod part in gold, anchored in the attic region with its foot resting on the plate of the stapes. This prosthesis is covered with a piece of autologous cartilage freshly removed from the tragus which forms the junction with the tympanic membrane or covering fascia. 60 cases are presented, some with a follow-up of 4 years. All of the patients were suffering from the sequelae of chronic otorrhoea, with varying degress of destruction of the chain and tympanic membrane, leaving a

OBJECTIVES/HYPOTHESIS: The performance of an ossicular replacement prosthesis (ORP) is influenced by its alignment and appropriate tension between the tympanic membrane and the stapes footplate. A novel ORP with a flexible element that potentially allows for length adjustment in situ is presented and tested for acoustic performance.
STUDY DESIGN: Laser Doppler vibrometry in fresh human cadaveric temporal bones was used to test the acoustic performance of the adjustable ORP relative to standard prostheses used for ossiculoplasty.
METHODS: The three-dimensional (3D) velocity of the stapes post

BACKGROUND: Glaucoma is the leading cause of irreversible blindness worldwide. Normal tension glaucoma (NTG) is a distinct subtype characterized by intraocular pressures (IOP) within the normal range (< 21 mm Hg). Due to its insidious onset and optic nerve damage, patients often present with advanced conditions upon diagnosis. NTG poses an additional challenge as it is difficult to identify with normal IOP, complicating its prediction, prevention, and treatment. Observational studies suggest a potential association between NTG and abnormal lipid metabolism, yet conclusive evidence establishing

CONCLUSIONS: Titanium proved to be a valuable alternative to ossicles in ossiculoplasty procedures. Complication rates are comparable to those obtained by other authors with titanium and nontitanium prostheses. Hearing results were worse compared with other reports; however, no conclusion can be drawn on the basis of the literature because of the heterogeneity of the populations studied.
OBJECTIVES: To evaluate the results and complications of total (TORP) and partial (PORP) titanium ossicular replacement prostheses in middle ear cholesteatoma.
METHODS: Fifty-seven patients affected by acqui

OBJECTIVE: In tympanoplasty, surgical reconstruction of the tympanic membrane and ossicular chain is well-established; however, its hearing results still require improvement. Custom 3D printing of individualized ossicular prostheses seems to be an attractive solution for optimal prosthesis adjustment and better hearing results.
AIM: The aim was to design a custom ossicular prosthesis using a 3D printing method based on Cone-beam Computed Tomography (CBCT) scans and assess the acoustic conduction properties of such prosthesis.
MATERIAL AND METHODS: A cadaver fresh frozen temporal bone was use

BACKGROUND: In the further development of alloplastic prostheses for use in middle ear surgery, the Dresden and Cologne University Hospitals, working together with a company, introduced a new partial ossicular replacement prosthesis in 2011. The ball-and-socket joint between the prosthesis and the shaft mimics the natural articulations between the malleus and incus and between the incus and stapes, allowing reaction to movements of the tympanic membrane graft, particularly during the healing process.

OBJECTIVES: Sometimes performing PORP adequately is challenging when the stapes is tilted or the suprastructure is partially damaged owing to inflammation or infection. In such cases, the implementation of a TORP bypassing the stapes can be a useful alternative. This study aimed to investigate whether bypassing the stapes suprastructure during total ossicular replacement prosthesis (TORP) affects postoperative complications or audiological outcomes.

BACKGROUND: The principal objective of ossiculoplasty is to reconstruct the sound-conducting system in the ear to the best possible degree after the elimination of pathological lesions from the middle ear. The ossicular chain is reconstructed with the use of the patient's own properly modelled ossicles and synthetic prostheses. The objective of the study was to assess the degree of hearing improvement after tympanoplasty in patients for whom a ventilation tube was used as a partial ossicular replacement prosthesis (PORP).
MATERIAL AND METHODS: Between 2009 and 2012, 387 patients underwent ear
