Ossification of the posterior longitudinal ligament (OPLL) is a disease of progressive ectopic calcification of the PLL of the spine. It occurs most frequently in the cervical spine, followed by the thoracic spine. The disease was first described in the Japanese population, and the prevalence of OPLL is highest in Japan at a rate of 1.9%-4.3%. Note, however, that OPLL is also seen and is a known cause of cervical myelopathy in other Asian countries and in the white population. Research into the underlying cause of OPLL over the past few decades has shown that it is a multifactorial disease wit

Ossification of the cervical posterior longitudinal ligament (OPLL) represents a continuum beginning with hypertrophy of the posterior longitudinal ligament (PLL) followed by progressive coalescence of centers of chondrification and ossification. Early OPLL mimicking disc disease appears opposite multiple disc spaces associated with significant retrovertebral extension, helping to differentiate it from spondylosis. On computerized tomography examinations, the single- and double-layer signs indicate possible dural penetration with the increased potential for an intraoperative cerebrospinal flui

BACKGROUND CONTEXT: Lumbar ossification of the posterior longitudinal ligament (L-OPLL) has been underrecognized and remains poorly characterized clinically. We hypothesized that L-OPLL constitutes a distinct phenotype within the broader OPLL spectrum, sharing features of obesity and diffuse spinal ligament ossification.
PURPOSE: To evaluate the clinical and radiographic features of L-OPLL and assess their relationship with diffuse spinal ligament ossification and obesity-related factors.
STUDY DESIGN: Cross-sectional study with a replication cohort.
PATIENT SAMPLE: A total of 186 patients

Combinations of varying degrees of spondylosis and/or ossification of the posterior longitudinal ligament (OPLL), and ossification of the yellow ligament (OYL) contribute to thoracic and lumbar neural compression in North Americans. Preoperative magnetic resonance and computed tomography examinations dictated the surgical approaches used to address spondylosis/OPLL in 11 patients, OYL in 12 patients, and spondylosis/OPLL and OYL in 3 patients. Myelopathy (4 patients), radiculopathy (13 patients), and cauda equina dysfunction (11 patients) were observed, 2 patients showing combined deficits. Ou

Ossification of the posterior longitudinal ligament (OPLL) can be defined as an ectopic ossification in the tissues of spinal ligament showing a hyperostotic condition. OPLL is developed mostly in the cervical spine and clinical presentations of OPLL are majorly myelopathy and/or radiculopathy, with serious neurological pathology resulting in paralysis of extremities and disturbances of motility lowering the quality of life. OPLL is known to be an idiopathic and multifactorial disease, which genetic factors and non-genetic factors including diet, obesity, physical strain on the posterior longi

Ossification of the posterior longitudinal ligament (OPLL) is a kind of disease that involves a variety of factors leading to ectopic bone deposition of the spinal ligament. Although the detailed mechanism is not clear, genetic factors play important roles in the development of this disease. Noncoding RNA (ncRNA) refers to an RNA molecule that is not translated into a protein but participates in the regulation of gene expression. Functionally important types of ncRNA associated with OPLL include long noncoding RNA, microRNA, and circular RNA. We listed the differentially expressed ncRNAs in OP

Ossification of the posterior longitudinal ligament (OPLL) occurs as heterotopic bone forms in the posterior longitudinal ligament, resulting in neural compression, myelopathy, and radiculopathy. OPLL is most commonly observed in East Asian populations, with prevalence rates of 1.9% to 4.3% reported in Japan. OPLL rates are lower in North American and European patients, with reported prevalence of 0.1% to 1.7%. Patients typically develop symptoms due to OPLL in their cervical spines. The etiology of OPLL is multifactorial, including genetic, metabolic, and anatomic factors. Asymptomatic or sym

Starting in the 1960s, ossification of the posterior longitudinal ligament (OPLL) became more commonly diagnosed in Japan. The disease is characterized by a gradual increase in calcification of the posterior longitudinal ligament with the eventual sequelae of cervical canal stenosis and myelopathy. Surgical interventions to relieve stenosis and neurologic symptoms are performed to decompress the cervical canal. Studies demonstrate continued ossification of the OPLL in both nonsurgical and surgically treated patients. In this review, the authors evaluate the epidemiology, pathophysiology, and l

Ossification of the posterior longitudinal ligament in the cervical spine may be a cause of cervical myelopathy. This ossification has often been encountered in Japan, but only sporadically among the Caucasian races. It is therefore probable that racial factors are relevant to the pathology. During the past four years, due to the routine use of computerised axial tomography (CAT) in the pre-operative study of cervical myelopathy, we were able to show that in 13 cases stenosis of the vertebral canal was due to ossification of the posterior longitudinal ligament. This was visible in the standard

Ossification of the posterior longitudinal ligament of the spine (OPLL) is a common disease after the middle age. OPLL frequently causes serious neurological problems due to compression of the spinal cord and/or nerve roots. OPLL occurs in patients with monogenic metabolic diseases including rickets/osteomalacia and hypoparathyroidism; however most of OPLL is idiopathic and is considered as a multi-factorial (polygenic) disease influenced by genetic and environmental factors. Genomic studies for the genetic factors of OPLL have been conducted, mainly in Japan, including linkage and association

Ossification of the posterior longitudinal ligament and ossification of the yellow ligament are the main causes of spinal canal stenosis. This article describes a case of ossification of the posterior longitudinal and yellow ligaments on the lumbar spine. The patient presented with gradually worsening left lower-extremity ache and pain. The deep tendon reflex was hyperreflexia in the lower extremities. Disturbances existed in the blade and bowel. The ossified lesion of ossification of the posterior longitudinal ligament was observed at L5-S1, and plain lateral radiographs and computed tomograp

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) can result in significant myelopathy. Surgical treatment for OPLL has been extensively documented in the literature, but less data exist on conservative management of this condition.
METHODS: The authors conducted a systematic review to identify all reported cases of OPLL that were conservatively managed without surgery.
RESULTS: The review yielded 11 published studies reporting on a total of 480 patients (range per study 1-359 patients) over a mean follow-up period of 14.6 years (range 0.4-26 years). Of these 480 patients, 3

OBJECT: The authors assessed the clinical course in patients with a narrowed cervical spinal canal caused by ossification of the posterior longitudinal ligament (OPLL), but who have no or only mild myelopathy. Additionally, the authors analyzed the factors contributing to the development and aggravation of myelopathy in patients with OPLLinduced spinal canal stenosis.
METHODS: Between 1997 and 2004, the authors selected treatments for patients with cervical OPLL in whom the residual space available for the spinal cord was < or = 12 mm. Treatment decisions were based on the severity of myelopa

Ossification of the posterior longitudinal ligament (OPLL), which causes marked spinal cord compression, occurs more frequently in Japan than in other parts of the world. To investigate the mechanism of this ossification, we examined two cases of mixed type of OPLL inducing severe neurological signs due to spinal cord compression and three cases of segmental OPLL type without neurological signs. Ossification of the ligament begins at the upper end of posterior margin of the vertebral body and distends mainly downward along the posterior longitudinal ligament (PLL) and finally replaces the PLL

The clinical material presented by the authors includes 24 observations of patients with ossification of the posterior longitudinal ligament. In all cases there was a cervical spine injury in the patients', past history. The diagnosis was confirmed roentgenologically and by the method of computer tomography. The treatment is surgical and consists in anterior decompression of the spinal cord. Recovery was observed in 10 patients (42 +/- 10%), partial regress of myelopathy in 9 patients (29 +/- 10%) and discontinuation (12 +/- 6%); there was no effect in 2 patients of progress of the disease in

A case is presented of cervical myelopathy with acute posttraumatic onset, due to ossification of the posterior longitudinal ligament. This disease is rare outside Japan. The progressive ossification appears to be secondary to multiple ankylosing degenerative discopathies.

Ossification of the posterior longitudinal ligament of the spine (OPLL) is an intractable disease leading to severe neurological deficits. Its etiology and pathogenesis are primarily unknown. The relationship between OPLL and comorbidities, especially type 2 diabetes (T2D) and high body mass index (BMI), has been the focus of attention; however, no trait has been proven to have a causal relationship. We conducted a meta-analysis of genome-wide association studies (GWASs) using 22,016 Japanese individuals and identified 14 significant loci, 8 of which were previously unreported. We then conduct
