[Diagnosis and treatment of osteoid osteoma].

OBJECTIVE: To study the clinical features and surgical treatment of osteoid osteoma and improve the diagnostic therapeutic level. METHODS: Clinical data of 35 patients (25 males and 10 females) with osteoid osteoma diagnosed and treated between January 1997 to October 2009 were retrospectively reviewed. The average age was 21 years (ranged, 6 to 49 years). The average interval time between onset of symptoms and diagnosis was 12 months (ranged, 1 to 24 months). The most common sites were the tibia (13 patients) and the femurs (7 patients). The most common presenting complaints for patients wit

Osteoid osteoma and osteoblastoma of the spine.

Eleven patients with spinal osteoid osteoma and six patients with spinal osteoblastoma treated between 1975 and 1990 were reviewed to characterize the tumors as they affect the spine and to define the important differences between the two tumors. All patients with cervical osteoid osteoma presented with pain, limited range of motion of the neck, and torticollis. Four osteoblastomas had soft-tissue components in the epidural space, necessitating dissection of the tumor from the dura. No soft-tissue component was found in any of the osteoid osteomas. Our results were similar to a metaanalysis of

The natural history of osteoid osteoma.

A long-term follow-up of three patients suffering from osteoid osteoma who have not undergone surgery is reported. Only eight similar cases have been reported in the literature. It is shown that over a period of years the pain of osteoid osteoma eventually disappears but the radiological appearance may remain unaltered or gradually resolve. Surgical management of osteoid osteoma is advised because of the prolonged symptoms.

Osteoid osteoma simulating an osteocartilaginous exostosis.

We describe a case of osteoid osteoma in the tibia of a 3-year-old patient who presented with a clinical and radiographic picture that suggested an exostosis. The formation of osteoid osteoma with a radiographic picture similar to that of osteophytes or exostosis has been previously documented only rarely. The authors hypothesize that the exostosis-like formation observed was actually the calcification of soft tissues that formed after the intense periosteal inflammatory reaction caused by the osteoid osteoma. As a result of its peculiar clinical and radiographic presentation, diagnosis of thi

Pseudo-ataxia due to Osteoid Osteoma.

BACKGROUND: Ataxia is diagnosed by typical features on examination suggestive of a cerebellar etiology and can invoke extensive diagnostic testing. Osteoid osteomas (OOs) are benign bone tumors of the lower limbs that occasionally present with focal neurological signs. CASE REPORT: A 3-year-old male presented with apparent progressive gait ataxia and non-specific leg pain. Initial imaging was unremarkable. However, 12 months later, a lesion was identified in the distal right femur, which was found to be an OO. The gait disorder and pain resolved after surgery. DISCUSSION: This case highlight

Osteoid osteoma--the CT vessel sign.

OBJECTIVE: To evaluate whether the presence of a feeding vessel in proximity to osteoid osteomas of long bones on multidetector CT (MDCT) can be an adjuvant clue for the diagnosis of osteoid osteoma. MATERIALS AND METHODS: Forty-nine CT scans of patients with radiological and clinical diagnosis of osteoid osteoma of long bones and a control group of 20 CT scans of patients with cortical-based lesions other then osteoid osteoma were analyzed. Two radiologists evaluated the CT images in consensus for the presence of a blood vessel in the same axial slices in which the nidus of osteoid osteoma w

[Chronic monoarthritis caused by osteoid osteoma].

Osteoid osteoma is a nonmalignant tumour that rarely localizes intraarticularly. When this happens, the tumour provokes arthritis and its recognition is delayed from months to years. We report the case of a 34 year old man with a previously known HIV infection, but no evidence of immunosuppression. He develops a chronic monoarthritis of the left elbow that is initially interpreted as infectious disease. CT provides diagnostic suspicion of osteoma, that is confirmed by pathologic examination two years after the onset of the clinical complaints. CT is the radiologic technique of election in the

Osteoid osteoma: excision with scintimetric guidance.

The definite diagnosis of osteoid osteoma relies on the demonstration of the nidus, best shown by CT, which also provides precise preoperative localization of the nidus. While bone-block excision to remove the nidus is feasible in the long bones, there may be unacceptable sequelae in the vertebral column and small bones of the hands. By precisely localizing the nidus, radionuclide scintimetry permits excellent therapeutic results, with minimal morbidity.

Transoral approach to osteoid osteoma of axis.

BACKGROUND: Pediatric osteoid osteomas of the cervical spine are rare, representing a unique diagnostic and therapeutic challenge due to their proximity to critical neurovascular structures. Although benign, these bone-forming tumors can cause significant pain and functional limitation, often necessitating intervention. Lesions involving the axis, particularly at the base of the dens, require careful management given their relationship to the spinal cord and stabilizing elements of the craniovertebral junction. CASE DESCRIPTION: We present a 9-year-old female with a 1-year history of progress

A misdiagnosed case of osteoid osteoma of L5.

Clinically, it is difficult to differentiate osteoid osteoma, more than 50% of which occur in the fibia or tibia, from other diseases, i.e. spinal degenerative diseases, inflammatory and noninflammatory arthritis. In this case report, we presented an unusual case of lumbar osteoid osteoma in a 38-year-old male, who experienced low back pain and sciatica as initial symptoms. The patient was initially misdiagnosed as lumbar disc herniation for more than 10 years. With the usage of computed tomography (CT) and magnetic resonance imaging (MRI), the patient was finally diagnosed as osteoid osteoma

Arthroscopic Excision of Osteoid Osteoma of the Talus.

Osteoid osteoma is a benign tumor that can cause significant pain and disability. Excision of the tumor can be accomplished with open surgery or, as advocated in recent years, with computed tomography (CT)-guided radiofrequency ablation. In this article, a unique arthroscopic approach to excise an osteoid osteoma of the talus is presented. This was possible by relying on a clear intra-articular prominent osteophyte, which was used as a landmark to indicate tumor location in accordance with preoperative CT views. This technique enabled excision of the tumor with concomitant arthroscopic decompr

Osteoid osteomas and osteoblastomas of the occipitocervical junction.

STUDY DESIGN: We describe our surgical experience to remove osteoid osteomas and osteoblastomas of the occipitocervical junction. In this location, vertebral artery vicinity requires special consideration. OBJECTIVES: We illustrate our surgical approaches to remove lesions confidently, while minimizing bone resection to preserve stability. SUMMARY OF BACKGROUND DATA: Up until now, osteoid osteomas and osteoblastomas of the occipitocervical junction are reported as case reports. Several treatment methods have been described to treat osteoid osteomas. Nevertheless, surgery is the treatment of

Osteoid osteoma of the patella: a case report.

Though osteoid osteoma is a common primary benign lesion of the bones, intra-articular involvement is rare and poses diagnostic difficulties when it affects middle-aged patients. We present the case of a 51-year-old woman with a 2.5 year history of anterior knee pain that was misdiagnosed as osteochondritis dissecans. Radiological findings were absent, whereas MRI showed a well-circumscribed lesion. A local excisional biopsy was performed and microscopic appearance confirmed diagnosis.

Atypical presentation of an osteoid osteoma in a child.

Osteoid osteomas are benign bone tumors that most commonly involve the long bones of the lower extremities, but do occur in the spine with some frequency. Patients with lesions in the spine typically present with back pain, scoliosis, and, less commonly, varying degrees of radicular leg pain. We report the case of a child with complaints of nonradicular leg pain. A combination of plain radiographs, scintigraphy, and axial imaging showed a lesion in the sacral spine consistent with an osteoid osteoma. After failed medical management, our patient was treated with intralesional excision, with com

Osteoid osteoma of a cervical vertebral body.

An osteoid osteoma in the anterior part of the body of the fourth cervical vertebra occurred in a 22-year-old female. The patient's main complaint was neck pain and occasional numbness of the extremities. The pain was relieved by analgesics. Plain radiography and a 99Tcm MDP bone scan showed a non-specific abnormality. CT suggested the pathological diagnosis with reasonable certainty. The unusual location of the lesion and the role of various diagnostic modalities are discussed and the literature reviewed.

Osteoid osteoma of the metacarpus. A case report.

The authors present a case of osteoid osteoma which is of interest not only because of the extreme rarity of its localisation in the 1st metacarpal, but also because of its unusual clinical features. These consisted of atypical pain, which was constant and did not respond to salicylates. There was marked local reaction, particularly in the soft tissues surrounding the 1st metacarpal, with a fine superficial venous reticulum. It was possible to follow the complete radiographic development of the tumour from its earliest stage to its final rather atypical appearance, and this allowed us to make

Scintigraphic patterns in osteoid osteoma and spondylolysis.

A retrospective review was performed of radionuclide bone scans and radiographs of 54 children with diagnoses of spondylolysis or osteoid osteoma/osteoblastoma. The findings indicate that skeletal scintigraphy shows imaging patterns that can be used to help distinguish these two conditions. Most cases of spondylolysis and all cases of osteoid osteoma or osteoblastoma demonstrated focally increased activity on the delayed skeletal scintigrams. Evaluation of the bone scan images obtained during the immediate postinjection period revealed intense activity with osteoid osteoma and osteoblastoma, b
