High tibial osteotomy: osteotomy in minus or hemicallotasis with EAF?

Between January 1988 and November 1994 a total of 29 patient were submitted to high tibial osteotomy to treat varus knee. Osteotomy in minus stabilized by staples and plaster was used in 13 cases, a monoaxial external fixator (AEF) associated with hemicallotasis was used in 16. After a period of time ranging from 11 to 58 months satisfactory results were obtained in 90% of the cases in the first group, and in 95% of those in the second. As compared to the first surgical procedure the second was characterized by: a) a simpler surgical technique; b) reduction in time required for surgery and sur

Bilateral Obturator Osteotomy: A Novel Osteotomy for Bladder Exstrophy Closure.

BACKGROUND: Abdominal and pelvic closure remains a challenge during bladder exstrophy initial repair. We aimed to report on the feasibility and results of a novel technique of bilateral obturator osteotomy. METHODS: Retrospective study of prospective collected data of children who underwent single-stage delayed bladder exstrophy closure combined with RSTM (Radical Soft Tissue Mobilization) for BEEC (Bladder Exstrophy Epispadias Complex) by the same team at different institutions between December 2017 and May 2021. When pubic approximation was not feasible at the end of the procedure, bilatera

[Distal osteotomy for the treatment of hallux valgus (Chevron osteotomy)].

Distal osteotomies, like the Chevron osteotomy, is indicated for mild to moderate hallux valgus deformities. Splayfoot, painful pseudoexostosis, and transfer metatasalgia are observed in the clinical examination. Radiographic examination should be done with weight bearing in two planes. Preoperatively the intermetatarsal (IM), hallux valgus, and distal metatarsal articular (DMAA) angles should be measured. The operative technique is based on soft tissue and bony correction. Modifications of the osteotomy allow a shortening, lengthening, or neutral correction of the first metatarsal. With a mod

Patient selection criteria for periacetabular osteotomy or rotational acetabular osteotomy.

BACKGROUND: Hip dysplasia is the most common cause of secondary osteoarthritis (OA). Periacetabular osteotomy (PAO) or rotational acetabular osteotomy (RAO) has been used as a joint-preserving procedure. However, the patient selection criteria are not clearly defined. QUESTIONS/PURPOSES: Based on a systematic review, we identified reported patient selection criteria for PAO or RAO. METHODS: We performed a systematic review of RAO and 18 studies met our inclusion criteria. For the PAO, the systemic review performed by Clohisy et al. was used. WHERE ARE WE NOW?: For patients with symptomatic h

[Imaging and preoperative planning of osteotomy of tibial head osteotomy].

Valgus-producing osteotomy of the proximal tibia is a well-established treatment for medial femorotibial osteoarthritis in the varus knee. The ideal patient is active, under 55 years of age, has a stable varus deformity of less than 10 degrees, a good bone stock, and an osteoarthritis stage that is restricted to the medial compartment of the knee. Coventry reported a failure rate in proximal tibial osteotomy to be significantly higher when the postoperative alignment was less than 8 degrees of anatomical valgus. Hernigou noted better clinical long-term results in cases with a precise correctio

A Novel Modification of the Sagittal Split Osteotomy as an Access Osteotomy.

Pathologies present in the central zone of the mandible are difficult to access, primarily because of the presence of the inferior alveolar nerve (IAN) and the need to remove a large corticocancellous component to reach the area of interest. Many times, this bony window is replaced as a free graft and there is complete resorption in the long term or even rejection of the graft causing a bony defect which can weaken the mandible. Furthermore, the damage to the IAN is profound. To try and avoid these comorbidities the traditional sagittal split osteotomy was modified to access a central osteoma

Evans Calcaneal Osteotomy.

Pes planovalgus is a prevalent deformity treated globally by foot and ankle surgeons. Surgical correction is indicated when a patient has failed conservative therapy or the deformity is unsuitable for conservative treatment. While correction of pes planovalgus often requires multiple procedures, the Evans osteotomy is widely utilized due to the robust triplanar correction it offers. This article aims to review the indications and surgical technique of the Evans osteotomy, while expanding on its indications that are not traditionally considered.

Spherical Periacetabular Osteotomy.

UNLABELLED: Various techniques for periacetabular osteotomy have been reported to prevent the progression of osteoarthritis in dysplastic hips1-5. Bernese periacetabular osteotomy, which involves the use of an anterior approach, is widely performed throughout the world because it offers preservation of the blood supply to the bone fragment and lateral pelvic muscles. However, Bernese periacetabular osteotomy has potential complications, such as nonunion at the osteotomy site, postoperative fracture, nonunion of the pubis and ischium, and damage to the main trunk of the obturator artery. Pedicle Subtraction Osteotomy.

UNLABELLED: Pedicle subtraction osteotomy (PSO) was originally performed in cases of ankylosing spondylitis. This procedure was invented because it was safer than trying to lengthen the anterior column via osteoclasis, which risked vascular injury and death1-4. PSO involves the removal of the posterior elements and the use of a vertebral body wedge to shorten the spine posteriorly and achieve sagittal-plane correction5,6. PSO has been used to correct sagittal-plane deformities not only in patients with ankylosing spondylitis but also in those with degenerative conditions or those who have prev

Rotational Acetabular Osteotomy.

UNLABELLED: Hip dysplasia is a common cause of secondary osteoarthritis. To prevent the early onset of secondary osteoarthritis, rotational acetabular osteotomy has been proposed1. DESCRIPTION: The approach combines the anterior iliofemoral and posterior approaches through a single skin incision. The pubic bone is cut independently, and the ilium and ischium are cut in continuity. The rotation of the acetabulum provides a more horizontal weight-bearing area and, at the same time, returns the superiorly subluxated femoral head to a more normal position. ALTERNATIVES: The Ganz periacetabular o

Evans Osteotomy Complications.

In 1975, Evans published an article describing the surgical management of the "calcaneo-valgus deformity," pointing out that the deformity was due to relative shortening of the lateral column of the foot. Correction involved "equalizing" both columns by performing an osteotomy in the neck of the calcaneus 1.5 cm from the calcaneocuboid joint, where a trapezoidal wedge of tricortical bone was placed. Although it was considered a success, there were complications, including sural nerve injury, surgical wound dehiscence, undercorrection, and graft subsidence. The osteotomy grew in popularity. Ind

Lesser Metatarsal Osteotomy.

Central metatarsal osteotomy is an effective approach in alleviating pain oriented to the forefoot. The procedures individually are straightforward in the isolated scenario. A working knowledge of the specific and unique pathobiomechanics is imperative when considering surgical intervention for the given pathologic scenario. Treating only the pain focus generally underserves the pathology and does not address the high point of the deformity. The surgeon must be cognizant of the complication potpourri, prepare the patient expectations, and engage a level of proactivity against sequelae to ensur

Vertical symphyseal osteotomy.

For the treatment of mild crossbite with increased bigonial distance, the authors performed a vertical symphyseal osteotomy on six patients in the last 3 years. Three of these patients had cleft lip deformities and the others had operations for orthodontic or aesthetic reasons. After exposing the mandible through the buccal mucosal incision, both premolars were extracted with or without conventional segmental osteotomy. The two vertical symphyseal osteotomies were performed with approximately 1 cm between them, and the central part of the mandibular bone was discarded. The bilateral segments o

Upper tibial osteotomy.

Arthritis of the knee usually produces a change in the coronal plane of the normal femorotibial axial alignment. This change occurs if the arthritis is mostly unicompartmental. Most unicompartmental degenerative arthritis involves the medial compartment, producing a varus deformity. In rheumatoid patients, however, a valgus deformity is commonly produced. For at least the last 25 years, upper tibial osteotomy has been used to correct this malalignment. The rationale is to unload the more involved compartment and transfer the load to the less involved side. While the vast majority of upper tibi
