The aim of this study was to evaluate the stress distributions and deformations of the temporomandibular joint (TMJ) during different periods before and after sagittal split ramus osteotomy (SSRO). A three-dimensional finite element model of the mandible and TMJ was established, based on the preoperative CT of a patient with mandibular prognathism. Numerical SSRO was performed and the models of three postoperative periods were established. Contact elements were used to simulate the interaction between the articular discs and the articular cartilages. Nonlinear cable elements were used to simul

The sagittal split ramus osteotomy (SSRO) has been performed mainly on an inpatient basis because of the duration of anaesthesia and the potential risk of postoperative complications, such as bleeding, pain, nausea, and vomiting. However, advances in both surgical and anaesthetic management have enabled the reduction of these risks and shortened the length of hospital stay. Thus, the SSRO may be feasible even in the ambulatory setting in elective cases. The clinical records of all patients who underwent an outpatient SSRO between August 2011 and September 2020 at Lilla Craniofacial Clinic were

The new orthognathic treatment strategy of non-segmental fixation (non-fix) sagittal split ramus osteotomy (SSRO) with jaw exercise initiation on the second postoperative day, the so-called physiological positioning strategy (PPS), induces good skeletal stability with few temporomandibular joint (TMJ) symptoms after surgery in skeletal class III. This study aimed to clarify whether non-fix SSRO with modified PPS can be applied to skeletal class II. This retrospective study included skeletal class II patients who underwent non-fix SSRO to correct mandibular retrognathia. Jaw exercise was initia

Bilateral sagittal split ramus osteotomy (BSSRO) is commonly used in orthognathic surgery. Although abnormal sensation in areas that are innervated by the inferior alveolar nerve is a well-known neurological complication of mandibular osteotomy, facial palsy is rare postoperatively. We present a case of peripheral facial palsy that developed the day after BSSRO to correct a mandibular protrusion in a 42-year-old man. Oral prednisolone was begun on the second day postoperatively, and was gradually tapered off over time. One month after operation, he had gradually recovered all movements in his

Aneurysmal bone cyst is a benign pseudocystic osseous lesion characterized by a fibrous connective tissue stroma with cellular fibrous tissue, multinucleated giant cells, and large blood-filled spaces with no endothelial lining. The entity is uncommon in facial bones, and it rarely involves the mandibular condyle. Resection of the lesion is the most accepted treatment. The present case is the 11th reported case of aneurysmal bone cyst of the mandibular condyle in the existing literature and the first where, rather than using conventional extra oral approach, sagittal split ramus osteotomy was

The sagittal split ramus osteotomy (SSRO) is commonly used to correct dentofacial deformities. This study compared fracture patterns, maximum tensile load, displacement, time, and thickness of ex-vivo mandibles subjected to the Epker or Wolford SSRO techniques. Sixty-one dried adult cadaveric mandibles were sectioned, with each hemimandible randomly assigned to one of two groups: Epker, which involved a horizontal beveled medial cut above the lingula, and Wolford, which added an additional cut along the inferior mandibular rim. Each hemimandible was mounted on a wooden base, and a metallic dev

Sagittal mandibular ramus osteotomy (SSRO) has undergone some modifications in terms of the position of the osteotomy and its extension, and it continues to be a versatile and efficient procedure for the treatment of facial deformities. The usual tools to perform the osteotomy include the motor with rotary drills and the reciprocating saw, and recently tools like the laser or the piezoelectric system have been included. The piezoelectric system shows advantages for the safety in the osteotomy and has allowed the development of new techniques that improve the execution and the prognosis of the

The purpose of this study was to elucidate the effects of mandibular anatomy and osteotomy technique on lingual fracture patterns in SSRO. The predictor variables were: length of horizontal medial osteotomy; type of border osteotomy; buccolingual width; and vertical length of the basal cortex. The outcome variable was the type of lingual split pattern. This was categorized into four types according to a lingual split scale (LSS): LSS 1, true Hunsuck; LSS 2, fracture line to posterior border of the ramus; LSS 3, through to mandibular canal; LSS 4, unfavorable fracture pattern. Data were analyze

Trigeminocardiac reflex (TCR) is a brainstem reflex triggered by the stimulation of any branch of the fifth cranial nerve along its course, presenting as a reduction in heart rate and blood pressure. Oculocardiac reflex is a well-known subtype of TCR. In the case reported here, remarkable arrhythmia followed by bradycardia occurred suddenly in a healthy patient undergoing orthognathic surgery. The heart rhythm recovered when the surgical manipulation ceased, but bradycardia was reproduced when the surgery resumed. This case of TCR is unique in that remarkable arrhythmia first appeared and led

Adjustable osteosynthesis miniplates are used to facilitate positioning of the mandible after bilateral sagittal split osteotomy (BSSO) to avoid skeletal relapse and occlusal discrepancies. The short Obwegeser BSSO reduces neurosensory disturbances. Adjustable osteosynthesis plates suited for the Obwegeser BSSO are not commercially available. This study tested adjustable miniplates for the short Obwegeser BSSO in advancement of the mandible and correction of facial asymmetry, assessing (1) sensitivity impairment of the lower lip and (2) skeletal stability.A prototype of L-shaped, 6-hole, 2.0-m
