Chronic suppurative otitis media: a review.

OBJECTIVES: Chronic suppurative otitis media (CSOM) remains one of the most common childhood chronic infectious diseases worldwide. Although microbial, immunological, and genetically determined factors, as well as Eustachian tube characteristics, are supposed to be involved in the pathogenesis of CSOM, many aspects of the pathogenesis of CSOM still need to be clarified. Optimal treatment strategy has not been established yet. The objective of this review is to present and evaluate the current state of knowledge of CSOM. DESIGN: Systematic narrative review. METHODS: A PubMed search (1966-Janu

Bacteriology of chronic suppurative otitis media.

Aspiration of exudate through an open perforation was performed in 183 patients with chronic otitis media. The pus was cultured aerobically and anaerobically. Aerobes only were isolated from 71 patients (39%); 20 patients (11%) had only anaerobes; and 91 patients (50%) had both aerobes and anaerobes. Only 1 specimen had no growth. There were 259 aerobic isolates. Pseudomonas aeruginosa was recovered from 68 patients. Other aerobes commonly recovered included Staphylococcus aureus and Klebsiella pneumoniae. There were 178 anaerobic isolates. Only anaerobic gram-positive cocci were isolated in 2

Hearing in chronic suppurative otitis media.

In individuals with chronic otitis media, mixed hearing impairments are common but it is unclear whether the raised bone conduction thresholds are a reflection of the pathologic process affecting the inner ear or a combination of the high prevalence of sensorineural hearing impairments in the population along with the artificial elevation of bone conduction thresholds associated with a conductive defect. A total of 395 ears with chronic otitis media but without cholesteatoma were studied. In them, once the artificial elevation (Carhart effect) of the bone conduction thresholds, which occurs wh

Epidemiology of acute suppurative otitis media.

The incidence of acute otitis media in childhood reported in the literature varies considerably, particularly for children in the first year of life. In three cohorts of otherwise healthy children, followed from birth to the age of 9 years, the authors have attempted to determine more accurately the incidence and prevalence of acute otitis media in childhood. The incidence is highest in the first year of life, at about 22 per cent, after which time it gradually decreases from 15 per cent in the second and 10 per cent in the third and fourth years of life to 2 per cent in the eighth year.
